Sources of multidrug-resistant Acinetobacter baumannii and its role in respiratory tract colonization and nosocomial pneumonia in intensive care unit patients

Background Multidrug-resistant Acinetobacter baumannii （MDRAB） is an important and emerging hospital-acquired pathogen worldwide. This study was conducted to identify the sources of MDRAB and its role in respiratory tract colonization and nosocomial pneumonia in intensive care unit （ICU） patients. Methods We conducted a prospective active surveillance study of MDRAB in three ICUs at a Chinese hospital from April to August 2011, to identify the sources of MDRAB and its role in respiratory tract colonization and nosocomial pneumonia. Results One hundred and fourteen （13.0%） MDRAB isolates were detected from 876 specimens, with a sensitivity of 11.6% （55/474） in screening of the pharyngeal and tracheal swabs, and 14.7% （59/402） of the sputum/endotracheal aspirates. MDRAB colonization/infection was found in 34 （26.8%） of 127 patients, including 16 （12.6%） cases of pure colonization and 18 （14.2%） cases of pneumonia （two pre-ICU-acquired cases of pneumonia and 16 ICU-acquired cases of pneumonia）. Previous respiratory tract MDRAB colonization was found in 22 （17.3%） patients： eight （6.3%） were pre-ICU-acquired colonization and 14 （11.0%）ICU-acquired colonization. Of eight pre-ICU-colonized patients, five were transferred from other wards or hospitals with hospitalization 〉72 hours, and three came from the community with no previous hospitalization. Overall, 6/22 colonized patients presented with secondary pneumonia; only two （9.1%） colonized MDRAB strains were associated with secondary infections. Respiratory tract MDRAB colonization had no significant relationship with nosocomial pneumonia （P=0.725）. In addition, acute respiratory failure, mechanical ventilation, renal failure, and prior carbapenem use were risk factors for MDRAB colonization/infection. Conclusions A high proportion of cases of MDRAB colonization/infection in ICU patients were detected through screening cultures. Mantle cell lymphoma （MCL） is an aggressive nonHodgkin＇s lymphoma, originating from naive B-cells. The blastoid MCL tumors often show complex cytogenetic aberrations. In this review, we summarized the data available on immunoglobulin heavy-chain （IgH） genes rearrangement for their importance in suggesting the MCL normal counterpart B-cell. Some new data suggesting an antigen selection process were also presented in this review.

Eradication of Helicobacter pylori increases childhood growth and serum acylated ghrelin levels

AIM: To determine whether Helicobacter pylori (H. pylori)infected children have reduced body weight (BW) and height (BH) growth, and if H. pylori eradication may restore growth while improving serum acylated ghrelin. METHODS: This longitudinal cohort study with oneyear follow-up enrolled 1222 children aged 4 to 12 years old into an observation cohort (18 with and 318 without H. pylori ) and intervention cohort (75 with and 811 without). The 7-d triple therapy was used for eradication in the intervention cohort. The net increases of BW and BH as well serum acylated ghrelin after oneyear follow-up were compared between successful eradicated H. pylori-infected children and controls. RESULTS: In the observation cohort, the H. pylori - infected children had lower z score of BW (-1.11 ± 0.47 vs 0.35 ± 0.69, P = 0.01) and body mass index (BMI) (0.06 ± 0.45 vs 0.44 ± 0.73, P = 0.02) at enrollment and lower net BW gain after one-year follow-up (3.3 ± 2.1 kg vs 4.5 ± 2.4 kg, P = 0.04) than the non-infected controls. In the intervention cohort, the H. pylori - infected children had lower z score of BMI (0.25 ± 1.09 vs 0.68 ± 0.87, P = 0.009) and serum acylated ghrelin levels (41.8 ± 35.6 pg/mL vs 83.6 ± 24.2 pg/mL, P < 0.001) than the non-infected controls. In addition to restoring decreased serum ghrelin levels (87.7 ± 38.0 pg/mL vs 44.2 ± 39.0 pg/mL, P < 0.001), the H. pylori - infected children with successful eradication had higher net gains (P < 0.05) and increase of z scores (P < 0.05) of both BW and BH as compared with non-infected controls after one-year follow-up. CONCLUSION:H. pylori-infected children are associated with low serum acylated ghrelin and growth retardation. Successful eradication of H. pylori restores ghrelin levels and increases growth in children.

Quality of life in head and neck cancer survivors： A concept analysis

Objective： To identify and clarify the existing definitions of quality of life in head and neck cancer survivors. Thisconcept analysis will inform healthcare professionals of promoting nursing care for head and neck cancer survivors andeventually improve their quality of life. Method： Concept analysis based on Walker and Avant’s framework. Results：Quality of life is a subjective and personally derived cognitive evaluation of global well-being by perceiving a feeling ofoverall life satisfaction in his/her own value, happiness, and the independence to manage daily activities and lifeexperience. Three defining attributes for quality of life were identified： a feeling of overall life satisfaction in his/hervalue, global personal well-being, and independence. An improvement of quality of life means an improvement ofclinical outcome. However, the variety of instruments for measuring life quality makes it difficult for healthcareprofessionals to select a standard one for head and neck cancer survivors. Conclusion： By proposing a comprehensivedefinition of the concept, this analysis contributes to the advancement of knowledge about quality of life in the context ofhead and neck cancer survivorship. Through a sound understanding of the phenomenon, healthcare professions willrecognize or measure head and neck cancer survivors’ life conditions in a more accurate way and to implementintervention where necessary, eventually, improve patients’ life quality in survivorship.

目的探索1985—2010年中国城乡儿童肥胖和超重率性别差异的长期趋势。方法研究对象来自全国学生体质与健康调研，该调研是中国最大的可代表全国学龄儿童的调研，包括1280239名7～18岁儿童。按照世界卫生组织（wH0）和国际肥胖工作组（[OTV）的标准对肥胖和超重进行判定。使用Logistic回归对不同年度超重、肥胖率的性别差异进行估计。结果过去25年以来，中国城乡儿童肥胖率迅速增长（WHO标准：从0．10％上升到4．3％；IOTF标准：从0．03％上升到2．2％），在每一调查年男生肥胖率均高于女生（P〈0．01）。各年龄组均出现增长趋势（P〈0．01）。尽管肥胖率在男女生中都出现增长，但是男生的增长速度快于女生。肥胖率的男女生性别比有随时间增加的趋势，在每一调查年，城市地区肥胖率的男女生性别比都高于乡村地区。结论在城乡地区逐步增加的性别差异显示男生的肥胖和超重，特别是城市男生的肥胖和超重，导致了儿童肥胖和超重的比例不断增加。

The role of antiangiogenic monoclonal antibodies combined to EGFR-TKIs in the treatment of advanced non-small cell lung cancer with activating EGFR mutations: acquired resistance mechanisms and strategies to overcome them

As of today, only two antiangiogenic monoclonal antibodies plus epidermal growth factor receptor-tyrosine kinaseinhibitor (EGFR-TKI) combinations are FDA and EMA-approved and are recommended by American Society ofclinical Oncology, European Society for Medical Oncology, and National Comprehensive Cancer Network for thefirst-line treatment of EGFR+ advanced non-small cell lung cancer patients: erlotinib plus bevacizumab anderlotinib plus ramucirumab. However, all treated patients eventually become unresponsive to such drugs, due toseveral different acquired resistance mechanisms, mainly represented by T790M substitutions and METamplifications. While osimertinib treatment in T790M+ patients still represents the only approved treatment,MET-TKIs will likely change this status quo in the near future. In fact, existing clinical data strongly support a rolefor MET-TKI-based combinations in EGFR+ MET-amplified patients, possibly revolutionizing our current treatmentalgorithm. Chemotherapy plus immunotherapy plus antiangiogenic therapy combinations could also representanother useful addition.
