Malignant mesotheliomas are rather uncommon neoplasms associated primarily with asbestos exposure; however, they may also arise as second primary malignancies after radiation therapy, with a latency period of 15–25 years. Numerous studies have reported an association between pleural malignant mesothelioma and chest radiation performed for other malignancies; on the other hand, post-irradiation mesotheliomas of the pericardium have been reported in only a few published cases to date, and no homozygous deletion of 9 p21 has been described in such cases. We report the case of a 48-year-old man with a history of Hodgkin's lymphoma and no prior asbestos exposure who developed pericardial malignant epithelioid mesothelioma. We further discuss the cytologic, histologic, immunophenotypic, and fluorescence in situ hybridization findings in this case. To our knowledge, this is the first well-documented case of post-radiation pericardial malignant mesothelioma showing homozygous deletion of 9 p21. Homozygous deletion of 9 p21, the locus harboring the p16 gene, is present in post-irradiation pericardial malignant mesothelioma.

Tumor metastasis is the main cause of death in patients with solid tumors. The epithelial-mesenchymal transition(EMT) process, in which epithelial cells are converted into mesenchymal cells, is frequently activated during cancer invasion and metastasis. MicroRNAs(miRNAs) are small, non-coding RNAs that provide widespread expressional control by repressing mRNA translation and inducing mRNA degradation. The fundamental roles of miRNAs in tumor growth and metastasis have been increasingly well recognized. A growing number of miRNAs are reported to regulate tumor invasion/metastasis through EMT-related and/or non-EMT–related mechanisms. In this review, we discuss the functional role and molecular mechanism of miRNAs in regulating cancer metastasis and EMT.

The DNA damage response(DDR)is a signal transduction pathway that decides the cell’s fate either to repair DNA damage or to undergo apoptosis if there is too much ***-translational modifications modulate the assembly and activity of protein complexes during the DDR ***(miRNAs)are emerging as a class of endogenous gene modulators that control protein levels,thereby adding a new layer of regulation to the *** this review,we describe a new role for miRNAs in regulating the cellular response to DNA damage with a focus on DNA double-strand break *** also discuss the implications of miRNA’s role in the DDR to stem cells,including embryonic stem cells and cancer stem cells,stressing the potential applications for miRNAs to be used as sensitizers for cancer radiotherapy and chemotherapy.

The identification of an ideal cell source for tissue regeneration remains a challenge in the stem cell field. The ability of progeny cells to differentiate into other cell types is important for the processes of tissue reconstruction and tissue engineering and has clinical, biochemical or molecular implications. The adaptation of stem cells from adipose tissue for use in regenerative medicine has created a new role for adipocytes. Mature adipocytes can easily be isolated from adipose cell suspensions and allowed to dedifferentiate into lipidfree multipotent cells, referred to as dedifferentiated fat(DFAT) cells. Compared to other adult stem cells, the DFAT cells have unique advantages in their abundance, ease of isolation and homogeneity. Under proper condition in vitro and in vivo, the DFAT cells have exhibited adipogenic, osteogenic, chondrogenic, cardiomyogenc, angiogenic, myogenic, and neurogenic potentials. In this review, we first discuss the phenomena of dedifferentiation and transdifferentiation of cells, and then dedifferentiation of adipocytes in particular. Understanding the dedifferentiation process itself may contribute to our knowledge of normal growth processes, as well as mechanisms of disease. Second, we highlight new developments in DFAT cell culture and summarize the current understanding of DFAT cell properties. The unique features of DFAT cells are promising for clinical applications such as tissue regeneration.

Penile fracture(PF)is a surgical *** its rarity,we queried a national cohort over an 11-year period to study the temporal and demographic variations in presentation,evaluation,and management of patients with PF compared with a cohort of control *** National Inpatient Sample was queried between the years 2005 and 2016 for patients with a diagnosis of *** patients were selected as a control cohort,given the non-discriminatory nature of this *** and demographic data of the patients were compared with that of *** symptoms,rates of surgical repair,and rates of associated surgical procedures were evaluated in the PF *** the study period,5802 patients were hospitalized for *** annual incidence of PF remained unchanged at 1.0-1.8 cases per 100000 hospitalizations over the study *** with the control cohort,PF patients were more likely to be younger(38.7 years vs 41.2 years,P≤0.001),have lower rates of comorbidities except erectile dysfunction(1.4%vs 0.1%,P≤0.001),and were more likely of Black race(25.4%vs 6.2%,P≤0.001).Notably,PF patients had significantly higher rates of substance abuse(26.4%vs 18.1%,P≤0.001),despite no difference in the diagnosed psychiatric *** rarely presented with hematuria(3.5%);however,urethral evaluation was performed in 23.1%,most commonly with cystoscopy(19.2%).PF occurs more commonly in a younger,healthier male population,and among ***,rates of substance abuse appear to be higher in the PF cohort compared with those of controls.

BACKGROUND: Hepatocellular carcinoma (HCC) is a heterogeneous malignancy with multiple etiologies, high incidence, and high mortality. The standard surgical management for patients with HCC consists of locoregional ablation, surgical resection, or liver transplantation, depending on the background state of the liver. Eighty percent of patients initially presenting with HCC are unresectable, either due to the extent of tumor or the level of underlying hepatic dysfunction. While in patients with no evidence of cirrhosis and good hepatic function resection has been the surgical treatment of choice, it is contraindicated in patients with moderate to severe cirrhosis. Liver transplantation is the optimal surgical treatment. DATA SOURCES: PubMed search of recent articles (from January 2000 to March 2011) was performed looking for relevant articles about hepatocellular carcinoma and its treatment. Additional articles were identified by evaluating references from selected articles. RESULTS: Here we review criteria for transplantation, the types, indications, and role of locoregional therapy in treating the cancer and in downstaging for possible later transplantation. We also summarize the contribution of immunosuppression and adjuvant chemotherapy in the management and prevention of HCC recurrence. Finally we discuss recent advances in imaging, tumor biology, and genomics as we delineate the remaining challenges for the diagnosis and treatment of this disease. CONCLUSIONS: Much can be improved in the diagnosis and treatment of HCC. A great challenge will be to improve patient selection to criteria based on tumor biology. Another will be to incorporate systemic agents post-operatively in patients at high risk for recurrence, paying close attention to efficacy and safety. The future direction of the effort in treating HCC will be to stimulate prospective trials, develop molecular imaging of lymphovascular invasion, to improve recipient selection, and to investigate biomarkers
