Diabetic corneal neuropathy and diabetic retinopathy are ocular complications occurring in the context of diabetes *** corneal neuropathy refers to the progressive damage of corneal *** retinopathy has traditionally been considered as damage to the retinal ***,growing evidence suggests that diabetic retinopathy is a complex neurovascular disorder resulting from dysfunction of the neurovascular unit,which includes both the retinal vascular structures and neural *** retinopathy is one of the leading causes of blindness and is frequently screened for as part of diabetic ocular ***,diabetic corneal neuropathy is commonly overlooked and underdiagnosed,leading to severe ocular surface *** studies have found that these two conditions tend to occur together,and they share similarities in their pathogenesis pathways,being triggered by a status of chronic *** review aims to discuss the interconnection between diabetic corneal neuropathy and diabetic retinopathy,whether diabetic corneal neuropathy precedes diabetic retinopathy,as well as the relation between the stage of diabetic retinopathy and the severity of corneal *** also endeavor to explore the relevance of a corneal screening in diabetic eyes and the possibility of using corneal nerve measurements to monitor the progression of diabetic retinopathy.

Background:Online physician reviews increase transparency in health care,helping patients make informed de-cisions about their *** processing techniques can quantify this data and allow providers to better understand patients'experiences,perspectives,and *** objective of this study was to assess patient satisfaction and understand the aspects of care that are valued by patients seeking refractive care using sentiment and word frequency ***:Written reviews and Star ratings for members of the Refractive Surgery Alliance Society practicing in the United States were collected from Healthgrades,a popular physician rating *** with at least one written review were included in the *** were scored from-1(most negative)to+1(most positive)using Valence Aware Dictionary sEntiment Reasoner(VADER).Reviews were stratified by demographic charac-teristics,namely gender,region,and years in *** frequency analysis was applied to find the most common words and ***:A total of 254 specialists and 3104 reviews were analyzed,with an average of 4.4/5 stars and mean 48 ratings *** physicians had positive reviews(96%,average VADER=0.69).Younger physicians(20 years)(P<0.001).A similar trend was observed in VADER score(0.71 vs 0.69),although not statistically significant(P=0.06).No statistical differences were observed between Stars rating and VADER score by gender(P=0.66,P=0.83)or by geographical region(P=0.74,P=0.07)."Staff"(n=1269),"professional"(n=631),"office"(n=523),"questions"(n=424),and"friendly"(n=386)were frequently used in reviews,along with phrases such as"the staff"(n=273)and"my questions"(n=174)."Surgery"(n=719),"staff"(n=576),"procedure"(n=251),"experience"(n=243),and"professional"(n=240)were the most common words in positive reviews,while"surgery"(n=147),"office"(n=86),"staff"(n=54),"time"(n=47),and"insurance"(

AIM:To analyse the changes in magnitude and orientation of astigmatism after suture removal(SR)in keratoplasty eyes as measured by refraction,tomography,and ***:Twenty-six eyes of 25 patients after optical keratoplasty requiring SR to reduce the astigmatism during the follow-up period were prospectively *** with poor quality topography scans or if there were no sutures in the steepest semi meridian were *** distance visual acuity(CDVA),manifest refraction,corneal tomography and aberrometr y were per formed on all patients before and after ***:The mean age of the patients was 40.8±14.4 *** keratoplasty was performed in 23 eyes(89%)and deep anterior lamellar keratoplasty was done in 3 eyes(11%).There was a statistically significant reduction in the magnitude of refractive,tomographic and aberrometry astigmatism after SR(Pcorneal tomography compared to refractive astigmatism(P<0.05).There was no statistically significant change in refractive astigmatism between 2 h and 2 mo after SR(P=0.55).Vector calculations demonstrated a greater amount of undercorrection in the tomography group and the rotational error was more towards counterclockwise *** monocular log MAR CDVA improved from 0.57 D to 0.49 D after SR(P=0.002).CONCLUSION:The net reduction in the magnitude of astigmatism after SR is greater in the tomography and aberrometry *** one episode of SR,there is no difference in the aberration profile.

AIM:To compare the difference of capsulotomy produced by precision pulse capsulotomy(PPC),manual(M-CCC),and femtosecond laser assisted capsulotomy(FLAC)in relation to intraocular lens(IOL)centration,circularity and its effect on visual ***:Prospective,non-randomized comparative study conducted at LV Prasad eye institute,Hyderabad,*** eyes of 52 patients were grouped into 3(FLAC,PPC and M-CCC)based on capsulotomy techniques *** consecutive eyes with uneventful phacoemulsification and with no comorbidities affecting the capsulotomy or visual outcome were included in each *** main outcome measure was IOL centration in relation to capsulotomy and *** outcome measures were post-operative visual acuity,manifest refraction and aberration profile between ***:At 5 wk the visual,refractive outcomes and endothelial cell density were comparable between the 3 *** median circularity index of FLAC was statistically significantly different to M-CCC or PPC(1-10)groups(P<0.01)but PPC(11-20)was comparable to *** of IOL center in relation to capsulotomy was seen only between the PPC(1-10)group and FLAC group(P=0.02).The IOL was well centered in relation to the pupil in all the groups(P=0.46).The quality of vision parameters like the higher order aberrations,spherical aberration,coma,trefoil,modular transfer function,and Strehl ratio were comparable between the ***:Our study shows that despite differences in the morphology of capsulotomy produced by PPC,M-CCC,FLAC a well-centered IOL can be *** measured capsular morphology parameters do not affect visual outcomes.

AIM:To investigate the incidence,risk factors,clinical course,and outcomes of corneal epithelial defects(CED)following vitreoretinal surgery in a prospective study ***:This was a post-hoc analysis of all participants in DISCOVER intraoperative optical coherence tomography *** with CED 1 d after surgery without intraoperative corneal debridement was defined as the postoperative CED *** who underwent intraoperative debridement were defined as intraoperative debridement *** were matched 2:1 with controls(eyes without postoperative CED)for comparative *** primary outcomes were the incidence of CED on postoperative day one and the incidence of required intraoperative *** outcomes included time to defect closure,delayed healing(>2 wk),visual acuity(VA)and presence of scarring at one year and cornea ***:This study included 856 eyes that underwent vitreoretinal *** corneal debridement was performed to 61(7.1%)subjects and postoperative CED developed spontaneously in 94(11.0%)*** factors associated with postoperative CED included prolonged surgical duration(P=0.003),diabetes mellitus(P=0.04),postoperative ocular hypotension(P2 wk)was associated with corneal scar formation at the end of the 1 y in all epithelial defect subjects(Pcorneal scarring for all eyes undergoing vitrectomy was 1.8%.CONCLUSION:Prolonged duration of surgery is the strongest factor associated with both intraoperative debridement and spontaneous postoperative *** defect closure is associated with a greater risk of corneal scarring at one *** overall rate of corneal scarring following vitrectomy is low at<2%.

laser refractive surgery is one of the most commonly performed procedures *** laser refractive surgery,Femtosecond laser in Situ Keratomileusis and Refractive Lenticule Extraction have emerged as promising alternatives to microkeratome laser in Situ Keratomileusis and Photorefractive *** laser refractive surgery,the corneal nerves,epithelial and stromal cells release neuromediators,including neurotrophins,neuropeptides and ***,nerve growth factor,substance P,calcitonin gene-related peptide and various cytokines are important mediators of neurogenic inflammation and corneal nerve *** in neuromediator profiles and ocular surface parameters following laser refractive surgery are attributed to the surgical techniques and the severity of tissue insult *** this review,we will discuss the(1)Functions of neuromediators and their physiological and clinical significance;(2)Changes in the neuromediators following various laser refractive surgeries;(3)Correlation between neuromediators,ocular surface health and corneal nerve status;and(4)Future directions,including the use of neuromediators as potential biomarkers for ocular surface health following laser refractive surgery,and as adjuncts to aid in corneal regeneration after laser refractive surgery.

Background:To report a case of bilateral benign reactive lymphoid hyperplasia(BRLH)of the conjunctiva treated with oral doxycycline and perform review of the literature evaluating the presentation,treatment and risk of transformation to *** presentation:A case report is described and review of the literature from January 1975 to January 2019 was performed.A 30-year-old man presented with bilateral enlarging fleshy pink medial canthal conjunctival *** biopsy revealed *** doxycycline was initiated(100 mg two times a day)for a total of 2 *** lesions decreased in size significantly at the patient’s two-month follow up *** residual lesion in the right eye was excised along with an adjacent pterygium and the patient has been free of recurrence for the past 1.5 *** lesion in the left eye has remained stable in size after cessation of the oral doxycycline.A total of 235 cases of conjunctival BRLH were identified in our literature *** mean age at diagnosis was 35.2 years(range,5 to 91 years).BRLH lesions were unilateral in 75%of patients and bilateral in 25%of *** patients(2.9%)had a concurrent Epstein-Barr virus(EBV)infection at the time of lesion *** most common treatments were surgical excision(155/235 or 65.9%)and corticosteroids(30/235 or 12.7%),while 14%(33/235)of the patients were observed and 4.6%(11/235)received external beam radiotherapy *** occurred in ten patients(10/235 or 4.2%),of whom five had undergone surgical excision alone,two excision followed by external beam radiotherapy,one excision and oral corticosteroids,one radiotherapy alone and one had been treated with topical ***,only 2 of the 235 reported cases(0.8%)developed malignancy,one localized to the conjunctiva and one ***:Benign reactive lymphoid hyperplasia is one of the lymphoproliferative disorders of the conjunctiva and ocular *** literature review shows that m

corneal neuromas,also termed microneuromas,refer to microscopic,irregula rly-shaped enlargements of terminal subbasal nerve endings at sites of nerve damage or *** formation of corneal neuromas results from damage to corneal nerves,such as following corneal pathology or corneal or intraocular surge ***,denervated areas of sensory nerve fibers become invaded by sprouts of intact sensory nerve fibers,and later injured axons regenerate and new sprouts called neuromas *** recent years,analysis of corneal nerve abnormalities including corneal neuromas which can be identified using in vivo confocal microscopy,a non-invasive imaging technique with microscopic resolution,has been used to evaluate corneal neuropathy and ocular surface *** neuromas have been shown to be associated with clinical symptoms of discomfort and dryness of eyes,and are a promising surrogate biomarker for ocular surface diseases,such as neuropathic corneal pain,dry eye disease,diabetic corneal neuropathy,neurotrophic keratopathy,Sjogren's syndrome,bullous keratopathy,post-refra ctive surgery,and *** this review,we have summarized the current literature on the association between these ocular surface diseases and the presentation of corneal microneuromas,as well as elaborated on their pathogenesis,visualization via in vivo confocal microscopy,and utility in monitoring treatment *** current quantitative analysis on neuromas mainly relies on manual annotation and quantification,which is user-dependent and labor-intensive,future direction includes the development of artificial intelligence software to identify and quantify these potential imaging biomarkers in a more automated and sensitive manner,allowing it to be applied in clinical settings more *** imaging and molecular biomarkers may also help elucidate the associations between corneal neuromas and ocular surface diseases.
