Modified thin manual Descemet stripping endothelial keratoplasty with air-guided, non-pachymetric donor lenticule dissection: outcomes of graft thickness and complication rate

●AIM:To describe a modified technique of donor lenticule dissection for thin manual Descemet stripping endothelial keratoplasty(TM-DSEK).●METHODS:Donor material was soaked in balanced salt solution(BSS)for 30 min,before being mounted on an artificial anterior chamber(AAC).Rather than BSS,the AAC was filled with filtered air,resulting in a visible reflection at the corneal endothelium-air *** reflection served as a landmark for the depth of the dissection,facilitating the creation of a thin lenticule with low risk of *** was commenced at a standardized depth of 500 microns,with no initial pachymetry *** 29 donor corneas were dissected by a novice TM-DSEK *** time,central graft thickness at 2 mo and complications were analysed.●RESULTS:Results were similar to other endothelial keratoplasty techniques,despite the cases being performed by a novice DSEK *** dissection time was 7 min(range 6-10).One graft perforation occurred(3.45%),but the air tamponaded the break and enabled dissection to be restarted and completed from a different *** central graft thickness after at least two months follow-up was 106 microns(range 25-170).●CONCLUSION:A problem with manual DSEK is the risk of graft perforation by attempting to dissect too thin a lenticule,or creating a thick graft due to fear of *** modified air-guided technique addresses this problem,and is recommended for surgeons either embarking on the learning curve,or who wish to achieve more consistently thin grafts while reducing perforation rates.

25-gauge transconjunctival diagnostic vitrectomy in suspected cases of intraocular lymphoma: a case series and review of the literature

AIM:To report the cytology results of 25-gauge transconjunctival(25G-TSV)diagnostic vitrectomy in cases suspicious for intraocular lymphoma(IOL),and compare the results to those reported in the ***:Clinical and cytopathological records of 18vitreous biopsy specimens obtained via 25G-TSV diagnostic vitrectomy in 12 patients suspicious for IOL were reviewed retrospectively.A review of the literature in regards to the diagnostic yields of vitreous specimens obtained via 25-gauge and 20-gauge diagnostic vitrectomy in suspected cases of IOL was ***:Eighteen eyes from 12 patients with clinical suspicion of IOL underwent diagnostic *** cytopathological investigations demonstrated IOL in 15eyes(83.3%).Vitreous analysis was non-diagnostic in 3eyes(16.7%).CONCLUSION:Twenty-five-gauge diagnostic vitrectomy yields adequate sample for cytological evaluation of the vitreous in cases suspicious for *** diagnostic results of the 25G-TSV in the current study are superior to those reported for 20-gauge vitrectomy but equivalent to those reported for 25G-TSV in the published literature.

AIM:To investigate the incidence,risk factors,clinical course,and outcomes of corneal epithelial defects(CED)following vitreoretinal surgery in a prospective study ***:This was a post-hoc analysis of all participants in DISCOVER intraoperative optical coherence tomography *** with CED 1 d after surgery without intraoperative corneal debridement was defined as the postoperative CED *** who underwent intraoperative debridement were defined as intraoperative debridement *** were matched 2:1 with controls(eyes without postoperative CED)for comparative *** primary outcomes were the incidence of CED on postoperative day one and the incidence of required intraoperative *** outcomes included time to defect closure,delayed healing(>2 wk),visual acuity(VA)and presence of scarring at one year and cornea ***:This study included 856 eyes that underwent vitreoretinal *** corneal debridement was performed to 61(7.1%)subjects and postoperative CED developed spontaneously in 94(11.0%)*** factors associated with postoperative CED included prolonged surgical duration(P=0.003),diabetes mellitus(P=0.04),postoperative ocular hypotension(P2 wk)was associated with corneal scar formation at the end of the 1 y in all epithelial defect subjects(Pcorneal scarring for all eyes undergoing vitrectomy was 1.8%.CONCLUSION:Prolonged duration of surgery is the strongest factor associated with both intraoperative debridement and spontaneous postoperative *** defect closure is associated with a greater risk of corneal scarring at one *** overall rate of corneal scarring following vitrectomy is low at<2%.

Changes in post-keratoplasty astigmatism after suture removal:refraction vs tomography vs aberrometry

AIM:To analyse the changes in magnitude and orientation of astigmatism after suture removal(SR)in keratoplasty eyes as measured by refraction,tomography,and ***:Twenty-six eyes of 25 patients after optical keratoplasty requiring SR to reduce the astigmatism during the follow-up period were prospectively *** with poor quality topography scans or if there were no sutures in the steepest semi meridian were *** distance visual acuity(CDVA),manifest refraction,corneal tomography and aberrometr y were per formed on all patients before and after ***:The mean age of the patients was 40.8±14.4 *** keratoplasty was performed in 23 eyes(89%)and deep anterior lamellar keratoplasty was done in 3 eyes(11%).There was a statistically significant reduction in the magnitude of refractive,tomographic and aberrometry astigmatism after SR(Pcorneal tomography compared to refractive astigmatism(P<0.05).There was no statistically significant change in refractive astigmatism between 2 h and 2 mo after SR(P=0.55).Vector calculations demonstrated a greater amount of undercorrection in the tomography group and the rotational error was more towards counterclockwise *** monocular log MAR CDVA improved from 0.57 D to 0.49 D after SR(P=0.002).CONCLUSION:The net reduction in the magnitude of astigmatism after SR is greater in the tomography and aberrometry *** one episode of SR,there is no difference in the aberration profile.

Achromatopsia caused by novel missense mutations in the CNGA3 gene

AIMTo identify the genetic defects in a Chinese family with *** 2.5-year-old boy, who displayed nystagmus, photophobia, and hyperopia since early infancy, was clinically evaluated. To further confirm and localize the causative mutations in this family, targeted region capture and next-generation sequencing of candidate genes, such as CNGA3, CNGB3, GNAT2, PDE6C, and PDE6H were performed using a custom-made capture ***-lamp examination showed no specific findings in the anterior segments. The optic discs and maculae were normal on fundoscopy. The unaffected family members reported no ocular complaints. Clinical signs and symptoms were consistent with a clinical impression of autosomal recessive achromatopsia. The results of sequence analysis revealed two novel missense mutations in CNGA3, c.633T>A (p.D211E) and c.1006G>T (p.V336F), with an autosomal recessive mode of *** analysis of a Chinese family confirmed the clinical diagnosis of achromatopsia. Two novel mutations were identified in CNGA3, which extended the mutation spectrum of this disorder.
