Risk assessment model for endpoints of ischemic stroke: A study protocol for a registry study

OBJECTIVE: To evaluate the risk of ischemic stroke endpoints by establishing risk assessment models that combine Traditional chinese medicine(TCM)and modern medicine ***/DESIGN: The proposed study is a registry-based participant survey conducted in seven hospitals nationwide in China. After obtaining informed consent, 3000 patients diagnosed with ischemic stroke will be recruited. One-year follow-ups will be performed on-site in hospitals and by telephone to track endpoint events. Comparative analysis of the prevalence of endpoint events and other TCM or modern medicine features in different groups will be conducted using frequency analysis and χ2tests, and the results will be expressed as composition ratios. Comparative analysis of quantitative scores and related patterns or symptoms will be conducted using a rank-sum test. Correlation analysis of endpoint events and TCM or modern medicine factors will be performed using a multivariate Cox proportional hazard ***: Previous reports have described modern medicine indicator-based risk assessment models for ischemic stroke endpoint events, but no such studies have included TCM features. Our new risk assessment model combines TCM and modern medicine indicators and thus has the potential to facilitate early warning, early intervention, and early control of ischemic stroke endpoint events.

Effect and Safety of Acupuncture for Type 2 Diabetes Mellitus:A Systematic Review and Meta-analysis of 21 Randomised Controlled Trials

Kangfuxin(Periplaneta americana)Lotion for Adjunctive Treatment of Ulcerative Colitis:A Systematic Review and Meta-Analysis of Randomized Controlled Trials

Objective: The objective of the study is to investigate the effect of Kangfuxin lotion as adjunctive therapy for treating ulcerative colitis(UC) through a systematic review and meta?analysis of randomized controlled trials(RCTs). Materials and Methods: RCTs comparing Kangfuxin lotion with no treatment, placebo, western medications or combination therapy for treatment of UC were included in this study. Two English and three chinese electronic databases were searched. All searches ended in December 2017. Data and quality assessment carried out respectively. Rev Man 5.2 software(Copenhagen, The Nordic Cochrane centre, The Cochrane Collaboration, 2014) was used to analyze with effect estimate presented as mean difference(MD), risk ratio(RR) and 95% confidence interval(CI). Results: Fifteen trials with 1050 participants were included in this review. Overall, 13 trials were eligible for meta?analysis. Kangfuxin lotion combined with Mesalazine was better than Mesalazine on clinical cure rate(RR 1.45, 95% CI 1.24–1.70, 7 trials), recurrence rate of UC(RR 0.22, 95% CI 0.07–0.72, 2 trials), or endoscopic mucosal recovery(RR 8.05, 95% CI 2.64–24.57, 2 trials). And Kangfuxin lotion combined with triple live bacteria tablets(Bifidobacterium-Lactobacillus? Streptococcus thermophilus)was superior with regard to symptoms remission rate(RR 5.36, 95% CI 1.73–16.60, 3 trials), endoscopic disease activity score(MD-2.30, 95% CI-2.64–-1.96, 1 trial). There were no reports on the serious adverse events of Kangfuxin lotion. Conclusion: Kangfuxin lotion as adjunctive treatment appears more effective than medications alone on clinical cure rate, recurrence rate, endoscopic mucosal recovery and improvement rate of related symptoms.

Acupoint Herbal Patching with or without Conventional Treatment for Stable Chronic Obstructive Pulmonary Disease: a Systematic Review of Randomized Controlled Trials

Background: Acupoint herbal patching(AHP) alone or as an adjuvant therapy with conventional treatment(CT) has been widely used for prevention and treatment of chronic obstructive pulmonary disease(COPD). However, current clinical evidence from a systematic review of RCTs is ***: To evaluate the effectiveness and safety of AHP with or without CT for people with COPD at stable ***: We searched randomized controlled trials comparing AHP(with or without CT) with no intervention, placebo, or CT from six databases. Two authors selected studies, extracted data and evaluated risk of bias of included trials. Rev Man 5.2 software was used to analysis ***: Twenty one RCTs(2327 participants) were included. AHP of non sanfu applied on no fixed dates with CT significantly decreased the mean frequency of acute exacerbation of COPD(times per year)(MD:-1.24; 95% CI:-2.02 to-0.46; 2 trials), and improved lung function parameters and quality of life. The AHP with CT showed no better effect in 6-minute walking distance(6 MWD) that CT alone. AHP applied at sanfu(specific dates based on lunar calendar) with CT had significant effect for 6 MWD(MD: 11.20; 95% CI: 0.83 to 21.56; I^2= 0%;3 trials). One trial reported skin irritation from AHP. Another trial reported two patients had eye discomfort, which was inferred as the adverse effects of ***: AHP used as an adjunct to CT, appears to be more effective than CT alone in patients with stable COPD. However, further large,rigorously designed trials are warranted to confirm these potential effects.

Biotransformation Effect of Bombyx Mori L. May Play An Important Role in Treating Diabetic Nephropathy

Compared with herbal drugs, medicine processed from animals(animal medicine) was thought to have more bioactive substances and higher activities. Biotransformation effect often plays an important role in their effect. However, researches about effect of animal medicine on diabetic nephropathy and applying animal medicine as natural bio-transformer were seldom reported. The purpose of this paper was to reveal the use of Bombyx Mori L. on diabetic nephropathy from ancient to modern times. The classical literature indicated that Saosi Decoction(缫丝汤), which contains Bombyx Mori L. or silkworm cocoon, was applied to treat disorders congruent with modern disease diabetic nephropathy from the Ming to Qing Dynasty in ancient China. Modern studies showed that Bombyx Mori L. contains four main active constituents. Among these, 1-deoxynojirimycin(1-DNJ) and quercetin showed promising potential to be new agents in diabetic nephropathy treatment. The concentrations of 1-DNJ and the activities of quercetin in Bombyx Mori L. are higher than in mulberry leaves, because of the biotransformation in the Bombyx Mori L. body. However, these specific components need further human and mechanistic studies to determine their therapeutic potential for this challenging condition.
