Elevated serum uric acid level as a predictor for cardiovascu-lar and all-cause mortality in Chinese patients with high cardiovascular risk

Objective To assess the predictive value of serum uric acid levels for cardiovascular and all-cause mortality in a large prospective population based *** The study was based on 3648 participants in Shanghai and Beijing,who were inpatients with high cardiovascular(CV) risk at baseLine (2004.7 to 2005.1),and blood was ***-up for death from cardiovascular disease and any cause was complete until January 1,*** The mean follow-up was 1 *** were 303 deaths during follow-up,of which 121 were *** mortality rates were 8.3 % for all patients,6.8% for female patients (116/1715),and 9.7% (187/1933) for male *** men,patients in the lower and higher uric acid groups had increased cardiac and overall mortality risks compared with patients in the normal uric acid *** relation was found in women but not statistically *** adjusting for other conventional risk factors (age,diabetes,hypertension,diuretic use and smoking),baseline uric acid level was still associated with increased risk for death from cardiovascular disease (P=0.005),or death from all causes (P=0.014) Conclusion Our data suggest that abnormal serum uric acid levels are independently and significantly associated with risk of cardiovascular and all-cause mortality.(J Geriatr Cardiol 2008;5:15-20)

Valvular heart disease （VHD） refers to the disorder of cardiac valves leading to valvular regurgitation or stenosis. Severe VHD will result in chronic heart failure and cardiac death. The only large population-based epidemiologic study involving echocardiography was performed in the United States in 2006, In this study, the prevalence of VHD in the United States was estimated to be 2.5%.for As a developing country, VHD caused by rheumatic heart disease is still a huge burden in China. It is estimated that there were around 7.07 million patients with rheumatic heart disease in China in 2015 In addition, as life expectancy increases, the prevalence of the degenerative valvular disease increases sharply according to a regional survey in Xinjiang, which reported that the prevalence of degenerative valvular disease in Hart elderly （mean age 67.7 years old） was 30.2%.

The Trend and Cause of Mortality Burden in Infancy—China,1990–2019

Introduction:China has made remarkable achievements in reducing infant deaths over the past 30 ***,the trend and cause composition of infant deaths have significantly changed,and the high-level years of life lost(YLLs)presents challenges to public *** study analyzes the temporal trend and cause of infant mortality burden in China from 1990 to ***:Based on the updated estimates from the 2019 Global Burden of disease Study,we used death and YLLs to describe the infant mortality *** percentage changes were applied to illustrate the temporal variation over the period ***:In 2019,the estimated infant deaths and YLLs in China were 101.9(95%uncertainty interval:88.6-116.4)thousand and 9.0(7.9-10.3)million,respectively,which decreased by 89.9%compared with *** infant deaths occurred in the neonatal period,especially the first 7 days of ***’s infant mortality rates for all causes decreased from 1990 to 2019,whereas certain preventable causes such as preterm birth and congenital heart anomalies accounted for increasing proportions during this *** and Implications for Public Health Practice:The present study reveals the major preventable causes of infant deaths in *** neonatal mortality and improving health status of children at early stage of life have great significance on public *** is also a solution to achieve the goal of“ending preventable child deaths”of the 2030 Sustainable Development Agenda.

Quantitative Parameters Analysis for Prenatally Echocardiographic Diagnosis of Atrioventricular Septal Defects

Background:Atrioventricular septal defects(AVSDs)are screened and diagnosed usually rely on the imaging characteristics of fetal echocardiography(FE).However,diagnosis on images is heavily depended on sonographers’experience and the quantitative data are rarely ***:This study aimed to realize the prenatal diagnosis of AVSDs by analyzing the quantitative data on ***:One hundred and thirteen cardiac quantitative data was analyzed in 370 normal and 49 AVSDs fetuses *** top six with the highest diagnostic accuracy rate were acquired according to the area under the curve(AUC),and the diagnostic value of six variables was ***:Six parameters obtained on the four-chamber view(4CHV),including the atrial to ventricular length ratio in end-diastole(AVLR-ED),AVLR-ED combined with the atrial to ventricular length ratio in end-systole(AVLR-ES),quantile score(Q score)of AVLR-ED,Q score of AVLR-ES,Q score of ventricle length in end-diastole(VL-ED),and AVLR-ES,were the top six with the highest diagnostic value,and the AUC was 0.99(95%CI 0.99–1.00),0.99(95%CI 0.99–1.00),0.99(95%CI 0.98–1.00),0.95(95%CI 0.91–0.99),0.93(95%CI 0.87–0.99),and 0.91(95%CI 0.83–1.00),*** within the 20%false positive rate,the diagnostic sensitivity was greater than 100%,100%,100%,90%,90%,and 88%,***:Six variables could be used for prenatal diagnosis of *** them,AVLR-ED and Q score of AVLR-ED,obtained on the 4CHV,were more convenient to acquire and had higher diagnostic accuracy.

Infliximab does not increase colonic cancer risk associated to murine chronic colitis

AIM To explore the influence of Infliximab(IFX) on cancer progression in a murine model of colonic cancer associated to chronic *** AOM/DSS model was induced in C57BL/6 mice. Mice were injected with IFX(5 mg/kg) during each DSS cycle while control mice received saline. Body weight, occult blood test and stool consistency were measured to calculate the disease activity index(DAI). Mice were sacrificed at week 10 and colons were analyzed macroscopically and microscopically for number of cancers and degree of inflammation. MTT assay was performed on CT26 to evaluate the potential IFX role on metabolic activity and proliferation. Cells were incubated with TNF-α or IFX or TNF-α plus IFX, and cell vitality was evaluated after 6, 24 and 48 h. The same setting was used after pre-incubation with TNF-α for 24 *** IFX significantly reduced DAI and body weight loss in mice compared with controls, preserving also colon length at sacrifice. Histological score was also reduced in treated mice. At macroscopic analysis, IFX treated mice showed a lower number of tumor lesions compared to controls. This was confirmed at microscopic analysis, although differences were not statistically significant. In vitro, IFX treated CT26 maintained similar proliferation ability at MTT test, both when exposed to IFX alone and when associated to TNF-α.CONCLUSION IFX did not increase colonic cancer risk in AOM-DSS model of cancer on chronic colitis nor influence directly the proliferation of murine colon cancer epithelial cells.

Background Dicycloplatin is a relatively safe third generation focused on the effects of dicycloplatin on in vitro proliferation （HASMC） and human aortic endothelial cells （HAEC）. platinum-complex anti-cancer drug. The present study and apoptosis of human aortic smooth muscle cells Methods Proliferation of HASMC and HAEC, DNA content, and cellular levels of proliferation- and apoptosis-related proteins were assessed using the （3-（4,5-dimethylthiazol-2-yl）-5-（3-carboxymethoxyphenyl）-2-（4-sulfophenyl）- 2H-tetrazolium）（MTS） assay, flow cytometry and Western blotting assays, respectively. Results Dicycloplatin at 10 ng/ml significantly inhibited HASMC proliferation, however, 10 μg/ml were required to significantly inhibit HAEC proliferation. Cell cycle analysis showed that dicycloplatin was a non-specific inhibitor of the cell cycle. Although dicycloplatin significantly decreased proliferating cell nuclear antigen （PCNA） expression in HASMC at all concentrations tested, it did not significantly affect PCNA expression in HAEC; Bax and p53 protein expression was upregulated in dicycloplatin groups. Conclusions Dicycloplatin at nanogram concentrations significantly inhibits HASMC proliferation, although the effect is relatively weaker than that of sirolimus. In contrast, the effect of dicycloplatin on inhibition of HAEC proliferation is much less pronounced than that on HASMC. The latter characteristics point to the potential for use of dicycloplatin in drug-eluting stents.

What is the optimal initiation timing of angiotensin converting enzyme inhibitor treatment for maximum benefits in acute myocardial infarction patients？

Randomized clinical trials led to the clinical recommendation that angiotensin-converting enzyme inhibitors （ACEI） should be used as standard therapy in most patients experiencing an acute myocardial infarction （AMI）. However, the optimal initiation timing of treatment, as well as the exact mechanisms, have not been completely resolved, especially with the development of reperfusion strategy after AMI. Earlier initiation of ACEI might be associated with more prompt recovery of left ventricular ejection fraction due to more rapid attenuation of negative remodeling.

Neonatal surgical outcomes after prenatal diagnosis of complex congenital heart disease: experiences of a perinatal integrated diagnosis and treatment program

Background This study aimed to evaluate neonatal surgical outcomes of patients diagnosed with complex congenital heart disease(CHD)during pregnancy and treated by the newly initiated"perinatal integrated diagnosis and treatment program(PIDTP)".Methods We reviewed clinical data of 207 neonates(surgical age≤28 days)who underwent cardiac surgeries in a single center from January 2017 to December 2018,including 31 patients with referrals from the"PIDTP"(integration group)and 176 patients with routine referral treatment(non-integrated group).Results In the integration group,median admission age was 0 days and median age at surgery was 4 *** the non-integrated group,median admission age was 8 days(P=0.001)and median age at surgery was 13 days(P=0.001).The emergency surgery rate in patients with duct-dependent defects was 36%in the integration group and 59%(P=0.042)in the non-integrated group,*** in-hospital mortality was 16%in the integration group and 14%(P=0.78)in the non-integrated *** 2-year cumulative survival rate after surgery was 83.9%±6.6%in the integration group and 80.3%±3.1%(P=0.744)in the non-integrated *** to multivariable regression analysis,independent risk factors for early mortality of overall neonatal cardiac surgery were low body weight,high serum lactate level,postoperative extracorporeal membrane oxygenation(ECMO)support and prolonged cardiopulmonary bypass(CPB)*** PIDTP shortens the postnatal transit interval,reduces the emergency operation rate of neonatal critical CHD,and provides better preoperative status for *** treated by the PIDTP tend to have more complicated anatomical deformity and a greater requirement for the operation and postoperative management,but early outcome and follow-up prognosis are satisfactory.

Background:Aortic root dilatation occurs in adults with tetralogy of Fallot(TOF)after surgical repair,but the longitudinal changes are *** main research aim is to determine the annual dilatation rate of aorta in adults with repaired ***:The present,multicentric,prospective cohort study assessed the rate of aortic diameter change in adults aged 20 years or older with TOF,including pulmonary artery atresia,who underwent surgical *** data,focusing on echocardiograms,were collected at three-year intervals from seven ***:In total,104 patients(58 males;median age:29 years)were *** actual Valsalva sinus(VS)diameter was 34.3±5.8(mean±standard deviation)and 36.1±6.0 mm at the initial andﬁnal examinations,respectively,and the annual dilatation rate was 0.64(0.07,1.33)(median,interquartile)mm/*** corrected diameter at the respective examination was 21.3±3.8 and 22.2±3.7 mm/m^(2),and the annual dilation rate was 0.28(-0.21,0.76)mm/m^(2)/*** regression analysis showed that factors signiﬁcant associated with dila-tation rate of actual VS diameter were the diastolic blood pressure(standardized coefﬁcient-0.22;p=0.04),cardiothoracic ratio(0.28;0.02),and the ratio of early mitral valve inﬂow velocity to early diastolic annular velocity(E/e′)ratio(0.31;0.004).Factors signiﬁcantly associated with corrected VS diameter were diastolic blood pressure(-0.25;0.02)and the E/e′ratio(0.34;0.001).Conclusions:In adults with repaired TOF,the rate of dilatation of the aortic diameter was associated with decreased diastolic blood pressure and left ventricular diastolic dysfunction,possibly reﬂecting decreased aortic wall elasticity.
