Diabetes knowledge and self-care practices among people living with type 2 diabetes mellitus in a diabetes clinic in Southwestern Nigeria

Objective:The objective of this study is to assess the knowledge and self-care practices of people living with type 2 diabetes mellitus(T2DM).Materials and Methods:The study was conducted in a clinic in Southwestern *** adopted a cross-sectional *** sample consists of 107 people living with *** were collected using Diabetes Knowledge Questionnaire and Revised Self-care Inventory ***:The mean diabetes knowledge of the participants was 16.2±3.0 and 70.1%had inadequate knowledge of *** mean diabetes self-care practice score among the participants was 49.9±4.9,and 89.7%reported poor diabetes self-care *** is no significant difference in the knowledge of diabetes between male(16.42±3.42)and female(16.12±2.8)participants(t=0.45;P=0.65),and no significant difference in self-care practice of male(49.23±4.51)and female(50.06±5.07)participants(t=0.75,P=0.46).In addition,there is no correlation between age and knowledge of diabetes(r=-0.18,P=0.07)and self-care practices(r=0.08,P=0.38)of *** significant relationship was found between knowledge of diabetes and diabetes selfcare practices(χ^(2)=1.605,P=0.448)Conclusion:Knowledge of diabetes and self-care practices of people living with T2DM in this study were *** should be directed at improving their diabetes knowledge and self-care practices.

Synchrotron radiation(SR)represents a unique and innovative anti-cancer treatment due to its unique physical features,including high flux density,and tunable and collimated radiation *** aim of this work is to assess the dosimetric properties of SR in Shanghai Synchrotron Radiation Facility(SSRF)for potential applications to clinical radiation *** experiments were performed with 34 and 50 keV X-rays on the BL13W biomedical beamline of SSRF and the 6 MV X-rays from ARTISTE linac for the dosimetry *** percentage depth dose(PDD)and the surface dose of the SR X-rays and the 6 MV photon beams were performed in solid water phantom with Gafchromic EBT3 *** curves are normalized to the maximum calculated *** depth of full dose buildup is about 10μm deeper for the monoenergetic X-ray beams of 34 and 50 *** beam transmits through the phantom,with a linear attenuation *** profile in the horizontal plane shows that the dose distribution is uniform within the facula,while the vertical profile shows a Gaussian distribution of the *** penumbra is less than 0.2 mm in the horizontal *** EBT film may be a useful and convenient tool for dose measurement and quality control for the high space and density *** is therefore important to gain a thorough understanding about the physical features of SR before this novel technology can be applied to clinical practice.

Predictors of successfully quitting smoking among smokers registered at the quit smoking clinic at a public hospital in northeastern Malaysia

Objective:The objectives of this study were to determine the proportion of smokers regis-tered at the quit smoking clinic at a public hospital in northeastern Malaysia who successfully quit smoking and the predictive factors for successfully quitting ***:This was a cross-sectional study involving smokers aged more than 18 years old and registered with the clinic from January 1,2012,to October 31,*** were obtained with a designed questionnaire that consisted of sociodemographic information,medical history,smoking characteristics,and type of treatment received by *** who quit smoking 6 months after being registered at the quit smoking clinic were considered as successful *** lo-gistic regression was applied to determine the predictive factors for successfully quitting ***:From a total of 202 respondents,42.6%[95%confidence interval(CI)35.8-49.4%]of them successfully quit *** logistic regression showed that the number of ciga-rettes smoked per day(adjusted odds ratio 2.51,95%CI 1.17-5.41)and a previous quit attempt(adjusted odds ratio 1.88,95%CI 1.03-3.44)were significant predictors for successfully quitting ***:This study shows that the proportion of smokers who successfully quit smoking among smokers registered at the quit smoking clinic was relatively high.A number of cigarettes smoked per day of 20 or fewer and a previous quit attempt significantly predict successful quitting of smoking.

The main manifestations were abdominal pain abdominal mass and emaciation. Cyst and leiomyomas were common in benign tumors. Malignant lymphoma leiomyosarcoma and liposarcoma were predominant in malignant tumors. The morbidity clinicopatholo-gic characteristics diagnosis and differential diagnosis of this group of tumor are discussed.

BACKGROUND Individuals with heart failure(HF) frequently experience limitations in mobility, but specific aspects of these limitations are not well understood. This study investigated the association of HF severity, based on the New York Heart Association(NYHA) classes, with digital mobility outcomes(DMOs) and handgrip strength in older inpatients with *** For this explorative analysis, hospital admission and discharge data from an ongoing, prospective cohort study were used. The sample included older participants with HF and a sub-sample of heart-healthy individuals. Participants were equipped with a wearable inertial measurement unit(IMU) system during mobility performance(balancing, sit-to-stand transfer,walking). We analyzed the association between 17 DMOs and HF severity with multiple linear regression *** The total sample included 61 older participants(65–97 years of age, 55.7% female). Of all DMOs, only sway path in a semi-tandem stance position(m/s^(2)) showed a relevant association with NYHA classes(admission: β =-0.28, P = 0.09;discharge:β =-0.39, P = 0.02). Handgrip strength showed a trend towards a significant association(admission: β =-0.15, P = 0.10;discharge:β =-0.15, P = 0.19).CONCLUSIONS This is to our best knowledge the first analysis on the association of HF severity and IMU-based DMOs. Sway path and handgrip strength may be the most promising parameters for monitoring mobility aspects in treatment of HF.

Vitamin K(VK), which was originally identified as a cofactor involved in the production of functional coagulation factors in the liver, has been shown to be involved in various aspects of physiological and pathological events, including bone metabolism, cardiovascular diseases and tumor biology. The mechanisms and roles of VK are gradually becoming clear. Several novel enzymes involved in the VK cycle were identified and have been shown to be linked to tumorigenesis. The VKs have been shown to suppress liver cancer cell growth through multiple signaling pathways via the transcription factors and protein kinases. A VK2 analog was applied to the chemoprevention of hepatocellular carcinoma(HCC) recurrence after curative therapy and was shown to have beneficial effects, both in the suppression of HCC recurrence and in patient survival. Although a large scale randomized control study failed to demonstrate the suppression of HCC recurrence, a meta-analysis suggested a beneficial effect on the long-term survival of HCC patients. However, the beneficial effects of VK administration alone were not sufficient to prevent or treat HCC in clinical settings. Thus its combination with other anti-cancer reagents and the development of more potent novel VK derivatives are the focus of ongoing research which seeks to achieve satisfactory therapeutic effects against HCC.

Survival rates according to barcelona clinic liver cancer sub-staging system after transarterial embolization for intermediate hepatocellular carcinoma

AIM: To investigate the survival rates after transarterial embolization(TAE).METHODS: One hundred third six hepatocellular carcinoma(HCC) patients [90 barcelona clinic liver cancer(BCLC) B] were submitted to TAE between August 2008 and December 2013 in a single center were retrospectively studied. TAE was performed via superselective catheterization followed by embolization with polyvinyl alcohol or microspheres. The date of the first embolization until death or the last follow-up date was used for the assessment of survival. The survival rates were calculated using the Kaplan-Meier method, and the groups were compared using the log-rank ***: The overall mean survival was 35.8 mo(95%CI: 25.1-52.0). The survival rates of the BCLC A patients(33.7%) were 98.9%, 79.0% and 58.0% at 12, 24 and 36 mo, respectively, and the mean survival was 38.1 mo(95%CI: 27.5-52.0). The survival rates of the BCLC B patients(66.2%) were 89.0%, 69.0% and 49.5% at 12, 24 and 36 mo, respectively, and the mean survival was 29.0 mo(95%CI: 17.2-34). The survival rates according to the BCLC B sub-staging showed significant differences between the groups, with mean survival rates in the B1, B2, B3 and B4 groups of 33.5 mo(95%CI: 32.8-34.3), 28.6 mo(95%CI: 27.5-29.8), 19.0 mo(95%CI: 17.2-20.9) and 13 mo, respectively(P = 0.013).CONCLUSION : The BCLC sub-stagingsystem could add additional prognosis information for postembolization survival rates in HCC patients.

A Prospective Survey Comparing Patients’ Experience of a Nurse-Led clinic and Consultant-Led clinic for Surveillance of Patients with Dilatation of the Intrathoracic Aorta

Background: Increasing workload in consultant-led clinics often means patients to wait a long time for clinic appointments. To address this, there is an increasing trend in developing nurse-led clinics across many specialities in the National Health Service. This study aims to assess whether the implementation of a nurse-led clinic in thoracic aortic surgery will optimise the utilisation of health care services and improve overall patient satisfaction. Methods: 80 follow-up patients were asked to complete a questionnaire following their appointment in an aortic clinic, which was led either by a consultant (n = 40) or an aortic specialist nurse (n = 40). All patients seen by a nurse in the clinic were assessed by a consultant surgeon prior to the clinic for suitability. No new patients were seen by a nurse. Any patient with an aortic dimension of 5 cm or greater was seen by the consultant. If there were any complicated clinical features, the patient was seen in the consultant-led clinic. Patients were asked questions about their time spent with the respective health care professionals across 12 categories (punctuality, preparedness, understanding of concerns, clarity of speech, listening, respect, explaining, letting you talk, putting you at ease, emotional support, advice and advice for next follow-up). Patients rated each category using an ordinal scale from 0 - 10. Results: Patient scores were greater in nurse-led clinics compared to consultant-led clinics across a number of categories although only punctuality reached significance (mean 9.2 vs. 6.8, p 0.05). Conclusion: Patients were highly satisfied with the nurse-led clinic across all categories, with greater satisfaction for punctuality. These findings suggest that a nurse-led clinic can be implemented for the management of carefully selected thoracic aortic surgery patient without reduction in patient satisfaction.
