nasopharyngeal carcinoma with synchronous breast metastasis:A case report

BACKGROUND Recent reports have described cases of metachronous breast metastasis in patients with nasopharyngeal ***,no similar cases of synchronous breast metastasis have been reported,and evidence that can be used to support the clinical diagnosis of stage IV nasopharyngeal carcinoma in patients with concurrent breast metastasis remains ***,additional evidence is required to elucidate the clinical characteristics of this condition and aid in the development of optimal management *** SUMMARY We report the case of a 46-year-old woman who visited our hospital with a right breast mass as the first *** first pathological biopsy report suggested triple-negative breast invasive *** imaging revealed a nasopharyngeal *** puncture biopsy of the nasopharyngeal mass,molecular pathological Epstein–Barr virus in situ hybridization,and immunohistochemistry confirmed the diagnosis of nasopharyngeal carcinoma with breast *** patient did not undergo a mastectomy and achieved complete remission after chemotherapy and *** continued to receive oral chemotherapy as maintenance therapy and experienced no recurrence or metastasis during the 6-month follow-up *** This case report suggests that breast specialists should carefully rule out secondary breast cancers when diagnosing and treating breast ***,clinicians should aim to identify the pathological type of the tumor to obtain the most accurate diagnosis and prevent excessive diagnosis and treatment.

BACKGROUND Bone is a major site of metastasis in nasopharyngeal carcinoma(NPC).Recently,nuclear factor kappa-beta ligand(RANKL)inhibitors have garnered attention for their ability to inhibit osteoclast formation and bone resorption,as well as their potential to modulate immune functions and thereby enhance the efficacy of programmed cell death protein 1(PD-1)inhibitor *** SUMMARY We present a case of a patient with NPC who developed sternal stalk metastasis and multiple bone metastases with soft tissue invasion following radical chemoradiotherapy and targeted *** to chemotherapy,the patient experienced severe bone marrow suppression and opted out of further chemotherapy ***,the patient received combination therapy,including RANKL inhibitors(denosumab)alongside PD-1,radiotherapy,and granulocyte-macrophage colonystimulating factor(PRaG)therapy(NCT05435768),and achieved 16 months of progression-free survival and more than 35 months of overall survival,without encountering any grade 2 or higher treatment-related adverse *** Denosumab combined with PRaG therapy could be a new therapeutic approach for the second-line treatment in patients with bone metastases.

Objective To evaluate the use of enhanced dynamic MR images in nasopharyngeal carcinoma (NPC). Methods Dynamic enhanced turbo spin-echo transverse images were obtained in 35 patients with newly diagnosed NPC. Comparison was made between the pre-contrast image, last enhanced scan and dynamic series in assessment of the extent of primary tumor and lymph node *** Enhanced MR images improved delineation of tumor in the following regions, nasopharyngeal wall (6/35), levator palatini (44/70), tensor palatini (12/70), prevertebral muscles (3/70), medial pterygoid muscle (7/70), pharyngobasilar fascia (30/35), parapharyngeal fat space (23/70), nasal cavity (33/70), oropharynx (6/35), sphenoid sinus (6/35), maxillary sinus (6/70), ethmoid air cells (14/35), cavernous sinus (5/35), cranial fossa (4/35), clivus (13/35) and pterygoid genu (2/70). Eight of the 9 normal and 79/82 abnormal nodal groups, on conventional imaging criteria, enhanced with the same pattern and intensity as the primary tumor. The dynamic series did not improve delineation of tumor extent in comparison with that obtained on last enhanced image. Conclusion Enhanced MR images are valuable in evaluating the extent of primary tumor. Dynamic imaging was of no additional benefit in the evaluation tumor extent.

nasopharyngeal carcinoma(NPC)is a serious and highly invasive epithelial malignancy that is closely associated with Epstein‒Barr virus(EBV).Due to the lack of therapeutic vaccines for NPC,we selected EBV latent membrane protein 2(LMP2)as a preferable targeting antigen to develop a lipid-based LMP2-mRNA(mLMP2)***-length mLMP2 expressing LMP2 was first synthesized using an in vitro transcription method and then encapsulated into(2,3-dioleacyl propyl)trimethylammonium chloride(DOTAP)-based cationic liposomes to obtain the mRNA vaccine(LPX-mLMP2).The cell assays showed that the antigenpresenting cells were capable of highly efficient uptake of LPX-mLMP2 and expression of ***2 could subsequently be presented to form the peptide-major histocompatibility complex(pMHC).Furthermore,LPX-mLMP2 could accumulate in the spleen,express antigens,promote the maturation of dendritic cells and stimulate antigen-specific T-cell responses in *** dramatically inhibited the tumor growth of the LMP2-expressing tumor model after three doses of ***,the proliferation of antigen-specific T cells in the tumor site made a good sign for the promise of mRNA vaccines in virus-induced ***,we provided a newly developed antigen-encoding mRNA vaccine with advantages against *** also demonstrated that mRNA vaccines are attractive candidates for cancer immunotherapy.

The DLC-1 gene,located at the human chromosome region 8p22,behaves like a tumorsuppressor gene and is frequently deleted in diverse *** deletion of 8p22 is not an uncommon eventin nasopharyngeal carcinoma(NPC),therefore we explored the expression levels of the DLC-1 gene in NPCsand NPC cell lines by reverse transcription-polymerase chain *** results showed the mRNA levelof DLC-1 was *** identify the mechanism of DLC-1 downregulation in NPC,we investigatedthe methylation status of the DLC-1 gene using methylation-specific PCR,and found that 79%(31 of 39)ofthe NPC tissues and two DLC-1 nonexpressing NPC cell lines,6-10B and 5-8F,were methylated in theDLC-1 CpG *** PCR was also carried out,and loss of heterozygosity was found at fourmicrosatellite sites(D8S552,D8S 1754,D8S 1790 and D8S549)covering the whole DLC-1 gene with ratiosof 33%(4 of 12 informative cases),18%(2 of 11),5%(1 of 18),and 25%(3 of 12),***,our results suggest that DLC-1 might be an NPC-related tumor suppressor gene affected by aberrantpromoter methylation and gene deletion.

cDNA microarray is a powerful tool to analyze simultaneously the expression levels of tens ofthousands of *** with normal nasopharynx (NP) tissues,2210 genes were highly differentiallyexpressed in nasopharyngeal carcinoma (NPC) tissues detected by cDNA *** signal pathwayis widely used to describe the complex relationship between genes,a pathway-based network was con-structed to visualize the connection between the genes obtained from microarray data in this *** the targeted genes that may have more important influence on this gene network with statisticalmethods and found that some genes might have significant influence on this network,especially Ras-relatednuclear protein (RAN),carboxyl ester lipase (CEL),v-rel reticuloendotheliosis viral oncogene homolog A(RELA) *** verify the results from pathway-based selection,reverse transcription-polymerase chainreaction (RT-PCR) and real-time RT-PCR were performed to detect the expression levels of RAN,CEL andRELA genes and it was found that the RAN and CEL genes were significantly up-regulated in more than 80%of NPC *** further elucidate the function of the RAN gene,RAN expression was specifically sup-pressed in a 5-8F NPC cell line by RNA interference (RNAi).As expected,the depletion of RAN couldeffectively block the proliferation of tumor ***,our study may open up a new way to analyze thevast microarray data.

Cytokinesis-block micronucleus method is used to evaluate the radiosensitivity of a nasopharyngeal carcinoma cell line (CNE-1) and biopsies obtained from 31 patients with nasopharyngeal carcinoma. The number of micronuclei increases with theradiation dose. A good correlation was found between the radiosensitivity determined by the micro-nucleus assay and that measured by the colony-forming assay in CNE-1 cell line (r=-0.998). Moreover, the results of micronucleus assay for tumor cells from biopsies of patients with primary carcinoma look promising for the prediction of tumor radiosensitivity. These results are encouraging but fleed to be confirmed with a larger number of patients with a longer follow-up.

To explore the effect of suppressing BCL-2 expression using RNA interference (RNAi) technique in nasopharyngeal carcinoma cell line CNE1. CNE1 cell lines stably expressing shRNAs targeted bcl-2 and GL3 gene were established and gene expression inhibition was assessed by Western blotting analysis.
