To the Editor:Systemic lupus erythematosus(SLE)is an autoimmune disease characterized by the production of various autoantibodies.Disease prevalence is 20 to 70 per 100,000 among adults,and 1.89 to 25.7 per 100,000 among children.[1,2]The pathogenesis of SLE is associated with immune disorders,especially involving the activation of B and T cells,which can lead to abnormal antibodies with nuclear and cytosolic antigens from apoptotic cells of the immune complexes(ICs)removing the damaged cells,and excessive activation of the complement system,thus causing damage to target organs,such as the kidneys and lungs.

系统性红斑狼疮(systemic lupus erythematosus,SLE)是一种典型的系统性自身免疫疾病,可导致全身多器官炎性反应。近年来,越来越多的研究关注到SLE与听力损失的发生存在关联,SLE患者中听力损失的发生率高于正常人群。SLE听力损失的发生发展与多种因素相关,且存在多种可能的发病机制。对SLE患者听力损失现有的评估方法较为局限,治疗方案缺乏高质量循证医学证据的支持,预后较难判断。对SLE患者听力损失的临床特征及发病机制加以阐释具有重要意义;探究其筛查方法、管理措施与预后判断有重要临床价值。本文对SLE中听力损失的临床特征、病理机制、评估和预后进行综述,以期为临床工作提供参考。

To the Editor:Systemic lupus erythematosus(SLE)is an autoimmune disease involving multiple systems.In recent years,the therapeutic treat-to-target(T2T)strategy was recommended for SLE.^([1])The immunosuppressive drugs are the standard of care in SLE treatment.Tacrolimus was one of the calcineurin inhibitors which was recommended in the 2019 update of the Joint European League Against Rheumatism and European Renal Association-European Dialysis and Transplant Association recommendations for the management of lupus nephritis(LN)^([2]).

Systemic lupus erythematosus(SLE),the most common form of lupus,is a chronic autoimmune disorder characterized by a global loss of self-tolerance and hyper-activation of both innate and adaptive immune systems(Kaul et al.,2016).Neutrophils,the most abundant leukocytes in human blood,have a critical role in maintaining immune surveillance and tissue homeostasis,and its dysregulation is of high relevance to SLE(Ricklin et al.,2010).In patients with SLE,accelerated neutrophil death and the deficiency in clearing dying neutrophils cause nuclear and cytoplasmic antigen exposure,excessive production of type I interferon(IFN),and neutrophil extracellular trap(NET)release,subsequently inducing autoimmune responses(Garcia-Romo et al.,2011).Dysregulated neutrophil death is believed to be a major cause of SLE;however,the underlying mechanism of neutrophil death in SLE is not well-defined.

As a chronic systemic autoimmune disease,systemic lupus erythematosus(SLE)can influence multiple organs and systems.Although the pathological basis and risk factors for SLE have been extensively investigated,the exact pathogenesis of SLE remains to be explored.However,there is increasing evidence that T-cell−Bcell interactions promote the development of SLE,resulting in the generation of high-affinity antibodies[1].Tfh cells,a subgroup of T cells,can promote the formation of germinal centers,the maturation of B cells,and the differentiation of plasma cells[2].It has been found that multiple transcription factors(TFs)can regulate the generation of Tfh cells.For instance,B-cell lymphoma-6(BCL-6),an evolutionarily conserved zinc-finger protein,is an important positive transcription factor responsible for the differentiation and maturation of Tfh cells.In addition to maintaining the phenotype and GC response of Tfh cells,BCL-6 can also regulate the dynamics of T-cell−B-cell interactions and improve the efficiency of delivering help to B cells[3].

Systemic lupus erythematosus(SLE)is an autoimmune disease with multisystemic features and a variety of clinical characteristics.Typical clinical manifestations of SLE include alopecia,oral ulcers,cutaneous lesions,arthritis,renal injury,and cardiac damage.[1]Cardiac involvement is one of the common complications of SLE,which is mostly asymptomatic with only approximately 10%of cardiac injury patients experiencing symptoms such as shortness of breath and arrhythmia.However,cardiogenic shock is rare,especially as the initial manifestation of SLE.[2]Herein,we report a case of a 15-year-old male who presented with cardiogenic shock and was eventually diagnosed with SLE.

Background:lupus nephritis(LN)results in high morbidity and mortality in patients with systemic lupus erythematosus.Refractory cases of standard immunosuppressive therapy have been recorded.Mesenchymal stem cell transplantation(MSCT)shows potential use in autoimmune diseases.We aim to review MSCT in the treatment of refractory LN.Methods:Published articles were systematically extracted from the databases in May 2022.The quality of the studies was assessed using the Oxford Centre for Evidence-Based Medicine critical appraisal checklist.Results:Four studies were included.All studies recruit severe or refractory LN patients and show significant improvement in post-MSCT 24-h proteinuria.However,the improvement of renal function after MSCT is inconsistent between studies after a 1-month follow-up,with one study showing deterioration of the estimated glomerular filtration rate after 3 months and one study showing an increase in serum creatinine at 6 and 9 months.Moreover,all studies show improvement in Systemic lupus Erythematosus Disease Activity Index scores after MSCT,while two studies show that a decrease in anti-double-stranded DNA(anti-dsDNA)levels after MSCT is only evident for up to 1-month period.Conclusions:MSCT can be effective,especially in severe or refractory LN.More randomized-controlled trials are needed to support this evidence.

Background:Systemic lupus erythematosus(SLE)is a complex prototypic autoimmune disease,with a prevalence of 20–150 per 100,000,most commonly affecting women at child-bearing age.Genome-wide association studies and fine mapping of candidate regions have paved the way for greater understanding of SLE as a disease of genetic–environmental susceptibility and the functions of the genes involved.Method:This study was conducted to determine the association of selected single nucleotide polymorphisms(SNPs)from established genomic databases based on theoretical risk and protective odds ratios>2.5 orlupus patients,majority of the SNPs were found in the major histocompatibilty complex genes in the HLA region.Our study identified two unique SNPs that will be validated as potential diagnostic markers for SLE.The findings of this study may contribute to the development of a polygenic risk score in determining susceptibility to SLE among Filipinos.
