This study aimed to report the outcomes of active surveillance(AS)in the management of low-risk prostate cancer(PCa).It recruited87 men who were prospectively followed up according to the Prostate Cancer Research International Active Surveillance(PRIAS)protocol with local adaptation at SH Ho Urology Centre,Prince of Wales Hospital,Hong Kong,China.We investigated the predictorsof disease progression and found that baseline prostate-specific antigen density(PSAD)and the presence of the highest ProstateImaging-Reporting and Data System(PI-RADS)score 5 lesion on magnetic resonance imaging(MRI)are significantly correlatedwith disease progression.Moreover,men with PSAD>0.2 ng ml^(−2)or PI-RADS 4 or 5 lesions had significantly worse upgradingfree survival compared to those with PSAD≤0.2 ng ml−2 and PI-RADS 2 or 3 lesions.The study concludes that AS is a safe andeffective management strategy for selected patients to defer radical treatment and that most disease progression can be detectedafter the first repeated biopsy.The combination of PSAD>0.2 ng ml^(−2)and PI-RADS 4 or 5 lesions may serve as a useful predictorof early disease progression and provide a guide to optimize follow-up protocols for men in different risk groups.

Background: localized pancreatic cancer, including resectable (R), borderline resectable (BR) and locally advanced unresectable disease (LAU), is considered in clinical guidelines for diverse treatment options based on clinical trials in selected populations. Hence, exploring with real world evidence (RWE) clinicians’ preferences for treatment options and their results seems pertinent. Methods: In a set of consecutive patients with localized pancreatic cancer assisted in a third level hospital from January 2013 to December 2022, medical records, symptoms, diagnostic process, distribution between subtypes, and treatment plans, with safety and efficacy results, were assessed. Results: A total of 152 patients with localized disease were included (43.4% R, 21.0% BR, 33.6% LAU). The population characteristics exemplified differences between daily practice and clinical trials. Tumor location and symptoms were as expected. Treatment plan was conditioned by PS or comorbidities in 23.0% of patients. In patients with R disease, surgery followed by different adjuvant chemotherapy (CT) regimes was the antineoplastic treatment of choice (64.8%) with efficacy results (OS 37.5 months;95% CI 18.4 - 56.7), in the range of contemporary standards. The common use of neoadjuvant CT for BR disease (94.4%), with surgery in 50% of them, and its results (OS 30.8 months;95% CI 10.5 - 51.2) reflected current controversies of treatment recommendations and evolution in this scenario. Paliative CT with or without radiotherapy was the standard specific treatment in LAU disease (95.1%) with survival results (PFS: 10.8 months;95% CI 8.8 - 12.7. OS: 20.3 months;95% CI 13.5 - 27.2) that justify the distinct character and the specific study of this entity. Conclusion: RWE for localized pancreatic cancer aroused from the analysis of this population confirms the distinct nature of patients assisted in daily practice, as well as mirrors the complexity of decision making in clinical assumptions in which achieving stronger evidence should be paramount.

Background Although localized neuroblastoma has a good prognosis,some cases have undergone treatment failure or recur-rence.Apart from biologic features such as MYCN status,we wondered whether some characteristics of growing tumors are prognostic,such as a well-encapsulated mass without infiltration of vital organs.We analyzed the diagnostic utility of image-defined risk factors(IDRFs)to predict successful treatment and prognosis.The overall goal was to achieve maximum cure rates for patients with localized neuroblastoma through a better understanding of clinical characteristics.Methods We retrospectively reviewed the images of patients with localized neuroblastoma who were enrolled between June 1998 and December 2012 at a single institution in Shanghai,China.Unequivocal categorization regarding IDRFs was available in 67 patients.IDRF was assessed at diagnosis and after four cycles of neoadjuvant chemotherapy,on average.The median follow-up period was 84 months(range: 48-132 months)after diagnosis.Results MRI and CT indicated a total of 177 IDRFs in these 67 patients.Logistic regression analysis revealed a highly significant negative correlation between the numbers of IDRFs and the possibility of complete removal of neuroblastoma.Intraspinal extension of the tumor,compression of the trachea,and encasement of the main artery in localized neuroblastoma were predictors for incomplete tumor resection.According to univariate analysis,≥4 IDRFs and intraspinal extension of the tumor were significant indicators of poor prognosis.Conclusions The number of IDRFs was useful in predicting surgical outcome and event-free survival.The number of IDRFs should be considered in protocol planning,instead of IDRF presence or absence.

Comparing to orthogonal localized molecular orbitals (OLMO), the nonorthogonal localized molecular orbitals (NOLMO) exhibit bonding pictures more accordant with those in the traditional chemistry. They are more contracted, so that they have a better transferability and better performances for the calculation of election correlation energies and for the linear scaling algorithms of large systems. The satisfactory NOLMOs should be as contracted as possible while their shapes and spatial distribution keep in accordance with the traditional chemical bonding picture. It is found that the spread of NOLMOs is a monotonic decreasing function of their orthogonality, and it may reduce to any extent as the orthogonality descends. However, when the orthogonality descends to some point, the shapes and spatial distribution of the NOLMOs deviate drastically from the traditional chemical bonding picture, and finally the NOLMOs tend to linear dependence. Without the requirement of orthogonalization, some other constraints have to be imposed for constructing satisfactory NOLMOs by minimizing their spread functional. It is shown that satisfactory results can be generated by coupling the minimization of orbital spread functionals with the maximization of the distances between orbital centroids.

To clarify the corrosion mechanism associated with the precipitate of T1(Al2CuLi)in Al-Li alloys,the simulated bulk precipitate of T1 was fabricated through melting and casting.Its electrochemical behavior and coupling behavior with α(Al)in 3.5% NaCl solution were investigated.Meanwhile,the simulated Al alloy containing T1 particle was prepared and its corrosion morphology was observed.The results show that there exists a dynamic conversion corrosion mechanism associated with the precipitate of T1.At the beginning,the precipitate of T1 is anodic to the alloy base and corrosion occurs on its surface.However,during its corrosion process,its potential moves to a positive direction with immersion time increasing,due to the preferential dissolution of Li and the enrichment of Cu.As a result,the corroded T1 becomes cathodic to the alloy base at a later stage,leading to the anodic dissolution and corrosion of the alloy base at its adjacent periphery.It is suggested that the localized corrosion associated with the precipitate of T1 in Al-Li alloys is caused by the alternate anodic dissolution of the T1 precipitate and the alloy base at its adjacent periphery.

A fundamental understanding of the charge transport mechanism in two-dimensional semiconductors(e.g., MoS2) is crucial for fully exploring their potential in electronic and optoelectronic devices. By using monolayer graphene as the barrier-free contact to MoS2, we show that the field-modulated conductivity can be used to probe the electronic structure of the localized states. A series of regularly distributed plateaus were observed in the gate-dependent transfer curves. Calculations based on the variable-range hopping theory indicate that such plateaus can be attributed to the discrete localized states near mobility edge. This method provides an effective approach to directly profiling the localized states in conduction channel with an ultrahigh resolution up to 1 meV.

Inexpe nsive copper nano particles are generally thought to possess weak and broad localized surface plasm on resonance(LSPR).The,present experimental and theoretical studies show that tailoring the Cu nanoparticle to a cubic shape results in a single intense,narrow,and asymmetric LSPR line shape,which is even superior to round-shaped gold nanoparticles.In this study,the dielectric function of copper is decomposed into an interband transition component and a free-electron component.This allows interband transition-induced plasmon damping to be visualized both spectrally and by surface polarization charges.The results reveal that the LSPR of Cu nanocubes originates from the comer mode as it is spectrally separated from the interb and transitions.In additi on,the interband tran sitions lead to severe damping of the local electromagnetic field but the cubic corner LSPR mode survives.Cu nanocubes display an extinction coefficient comparable to the dipole mode of a gold nanosphere with the same volume and show a larger local electromagnetic field enhancement These results will guider-development of in expensive plasmonic copper-based nano materials.

Network layer multicast is a highly efficient oneto ?many transmission mode. Data rates supported by different group members may differ if these members are located in different network environments. Currently there are roughly two types of methods solving the problem, one is limiting the data rate so that every group member can sustain transmissions, and the other is building multiple trees to increase the provision of network bandwidth. The former is inefficient in bandwidth usage, and the latter adds too many states in the network, which is a serious problem in Software-Defined Networks. In this paper, we propose to build localized extra path(s) for each bottleneck link in the tree. By providing extra bandwidth to reinforce the bottleneck links, the overall data rate is increased. As extra paths are only built in small areas around the bottleneck links, the number of states added in the network is restrained to be as small as possible. Experiments on Mimnet verify the effectiveness of our solution.
