Silver dressing in the management of an infant's urachal anomaly infected with methicillin-resistant Staphylococcus aureus:A case report

BACKGROUND Symptomatic urachal anomalies are rare disorders.The management of urachal remnants has historically been surgical excision because of the connection between urachal remnants and risk of malignancy development later in life.However,recent literature suggests that urachal anomalies that do not extend to the bladder can be treated with conservative management.In this case,we report a newborn with an infected urachal remnant who was treated with a combination of antibiotics and a silver-based dressing and finally recovered well.CASE SUMMARY Female baby A,weighing 2.88 kg at 38^(+5) wk of gestational age,was referred to the hospital because of a red,swollen umbilicus approximately 2 cm×2 cm in size with yellow purulent exudate.Through physical and ultrasound examination,the baby was finally diagnosed with a urachal anomaly.We first used oxacillin to prevent infection for 3 d.On the 4^(th) day,microbiology testing of the umbilical exudate revealed the presence of methicillin-resistant Staphylococcus aureus(MRSA).We changed the treatment with oxacillin to vancomycin for systemic infection and treated the umbilical inflammation with a silver sulfate dressing.After 5 d,the symptoms of the umbilicus disappeared,and we discontinued silver dressing application.On the 12^(th) day,umbilical exudate testing was negative for MRSA.On the 14^(th) day,the baby's blood testing showed a white blood cell count of 14.7×10^(9)/L,neutrophil percentage of 27.8%,and C-reactive protein level of 1.0 mg/L,suggesting that the infection had been controlled.We stopped treatment,and the baby was discharged with no complications.In this case,the infected urachal anomaly was cured with silver dressing and antibiotic application instead of surgical methods,which was a different course from that of some other urachal remnant cases.CONCLUSION Anomalies that do not connect with the bladder can be treated with nonoperative management,including application of conservative antibiotics and local intervention with silver-based dressings.Silver s

infant Cry Language Analysis and Recognition:An Experimental Approach

Recently, lots of research has been directed towards natural language processing. However, the baby's cry, which serves as the primary means of communication for infants, has not yet been extensively explored, because it is not a language that can be easily understood. Since cry signals carry information about a babies' wellbeing and can be understood by experienced parents and experts to an extent, recognition and analysis of an infant's cry is not only possible, but also has profound medical and societal applications. In this paper, we obtain and analyze audio features of infant cry signals in time and frequency domains.Based on the related features, we can classify given cry signals to specific cry meanings for cry language recognition. Features extracted from audio feature space include linear predictive coding(LPC), linear predictive cepstral coefficients(LPCC),Bark frequency cepstral coefficients(BFCC), and Mel frequency cepstral coefficients(MFCC). Compressed sensing technique was used for classification and practical data were used to design and verify the proposed approaches. Experiments show that the proposed infant cry recognition approaches offer accurate and promising results.

To the Editor,Developmental delay or intellectual disability(DD/ID)is one of the most common neurodevelopmental disabilities worldwide with high clinical and genetic heterogeneity.Its etiology remains unexplained in nearly 70%of these patients,and an accurate diagnosis still poses a challenge in clinical practice.Previous DD/ID cohort studies mostly used panel sequencing or chromosome microarray analysis(CMA),but targeted capture probes could not be updated in time,resulting in an increased risk of missed detection of genetic abnormalities.

Hemangiomas are the most common benign vascular tumours of the pediatric age group.They originate from embryonic remnants of unipotent angioblastic cells.The liver is the most common site for hemangioma in the abdomen.1 The prevalence of hepatic hemangioma(HH)varies from 1%to 20%in the general popula-tion,and it is two to five times more common in women.HH is usually detected incidentally and shows no symptoms;therefore,most of the time it does not require any treatment.2 However,hemangioma may sometimes occur at the atypical location and present with unusual clinical manifestations within the abdominal cavity.Due to these reasons,it leads to diag-nostic confusion,resulting in their inadvertent surgical excision.12 We present here a case of intra-abdominal hemangioma in an infant who presented with the signs and symptoms of intestinal obstruction.

Maternal exposure to antibiotics increases the risk of infant eczema before one year of life:a meta-analysis of observational studies

Background There are some conflicting evidences showing that maternal exposure to antibiotics may increase the risk of infant eczema.The present study aims to estimate the effect of prenatal antibiotics administration on infant eczema.Methods According to the established inclusion criteria,eligible observational studies were collected by comprehensive database search.The qualities of the included studies were assessed using the Newcastle-Ottawa Scale.Effect sizes that were adjusted by multivariable models were extracted and combined.Publication bias was evaluated by visual inspection of funnel plot.Results A total of seven observational studies were included.The qualities of the included studies were at moderate or high level.Prenatal antibiotics use was positively associated with eczema before one year of age[odds ratio(OR)=1.93,95%confidence interval(CI)1.35-2.76].In addition,antibiotics exposure in utero was likely to be related to eczema after one year of age(OR 1.21,95%CI 0.98-1.49).The exposure to antibiotics during third trimester was not associated with infant eczema(OR0.97,95%CI 0.86-1.09).Conclusions Maternal exposure to antibiotics is associated with eczema by one-year age and may have a prolonged effect on eczema after 1-year age.The influence of timing of antibiotics exposure during pregnancy needs more studies to clarify.

Interchange between grooming and infant handling in female Tibetan macaques(Macaca thibetana)

In some non-human primates, infants function as a social tool that can bridge relationships among group members. infants are a desired commodity for group members, and mothers control access to them. The biological market theory suggests that grooming is widespread and represents a commodity that can be exchanged for infant handling. As a limited resource, however, the extent to which infants are interchanged between mothers(females with an infant) and non-mothers(potential handlers,females without an infant) remains unclear. In this study, we collected behavioral data to investigate the relationship between grooming and infant handling in free-ranging Tibetan macaques(Macaca thibetana)at Mt. Huangshan, China. Our results showed that females with infants received more grooming than females without infants. After her infant was handled,mother females received more grooming than they did during daily grooming interactions. However, with the increasing number of infants within the social group, both the grooming that mothers received and the grooming that non-mothers invested for handling infants decreased. We also found that non-mothers invested more time in grooming to gain access to younger infants than older infants. Our results provide evidence that infants are social commodities for both mother and non-mother females. Mothers use infants for obtain grooming and non-mothers use grooming to gain access to infants. The current study implies a bidirectional and complex interchange pattern between grooming and infant handling to compensate for the dyadic grooming disparity in non-human primates.

Effects of Antiretroviral Therapy and HIV Exposure in Utero on Adverse Pregnancy and infant Outcomes:A Prospective Cohort Study in Guangzhou, China

Objective This study aimed to evaluate the effects of in-utero exposure to HIV and ART on pregnancy outcome and early growth of children.Methods This cohort study enrolled 802 HIV-infected pregnant women between October 2009 and May 2018 in Guangzhou, China. The women were assigned to receive combination ART(c ART) or mono/dual ART or no treatment. The primary outcomes were the combined endpoints of any adverse pregnancy outcome [including ectopic pregnancy, spontaneous abortion, stillbirth, preterm birth, small for gestational age(SGA)] and adverse early growth outcome(including infant death, HIV infection of mother-to-child transmission, and underweight, wasting and stunting of infants at 4 weeks of age).Results Adverse pregnancy outcomes occurred in 202(35.1%) of all enrolled HIV-infected women, and121(31.3%) of all infants exhibited adverse effects on early growth at 4 weeks of age. The rates of adverse pregnancy outcomes, spontaneous abortion, ectopic pregnancy, stillbirth, infant death and perinatal HIV infection were higher among women not receiving ART, compared to those treated with c ART or mono/dual ART(P infant growth were observed among the different treatment regimens.Conclusion Our findings underscore the essentiality of prioritizing HIV-positive pregnant women for ART, as even mono/dual ART available in resource-limited countries could improve pregnancy outcomes and infant survival.
