It is believed that acne is a multifactorial dis-ease, and its clinical expression depends on the in-teraction of several factors: (1) increased produc-tion of sebum and enlarged sebaceous glands as theresult of androgenic stimulation; (2) obstructionof the follicular duct, due to keratinization of thesebaceous follicle, retention of sebum and blockageof hair follicles by keratinized epithelium, leadingto the formation of a microcomedo; (3) propaga-tion of Propionibacterium acnes(P. Aches); and (4)release of lipases by P. Acnes, resulting in hydrol-ysis of sebum triglycerides into free fatty acids,which irritate the wall of follicle, leading to rup-ture of follicle, irritate the surrounding dermis andinduce inflammatory changes.

Nicotinic acid (NA) comprises the oldest hypolipidemic drug, in use since 1955. Despite its established broad spectrum effect on lipid profle and the associated re-duction in cardiovascular morbidity and mortality, the mechanisms by which NA achieves its benefcial effects remain elusive. Regarding the NA-associated reduction in triglyceride and low density lipoprotein cholesterol levels, data are controversial. The prevailing view which suggested that NA inhibits lipolysis and decreases free fatty acid (FFA) release both via activation of adipose tissue G-protein receptor-109A (GPR109A) and via inhibition of hepatic triglyceride synthesis is currently debated by the observation that the initially decreased FFA levels rebound during long-term NA treatment even though the benefcial NA effects on lipid metabo-lism are preserved, while other mechanisms involving modulation of transcription and translation pathways are emerging. In addition, NA has been demonstrated to affect high density lipoprotein (HDL) particles remod-eling in a number of ways, including reducing choles-terol ester transfer protein levels and activity, increas-ing apolipoprotein A-I levels, eliminating HDL hepatic uptake, increasing cholesterol efflux via ATP-binding cassette A1, inhibiting hepatic lipase, thereby overall in-creasing the plasma residence time of HDL and apoA-I with retention of cholesterol esters in HDL. Focus of this article is to present the mechanisms by which NA exerts its broad spectrum hypolipidemic actions.

AIM:Essential hypertension is inherently associated with metabolic disorders,and its pathogenesis remains poorly *** of the metabolic perturbation of hypertension and the regulation of this perturbed metabolism using therapeutic agents can extend our insight into the pathogenesis of this ***:We used gas chromatography mass spectrometry and principal component analysis to profile the metabolic phenotype of hypertension in spontaneously hypertensive rats(SHR) and studied the regulatory effect of total ginsenosides(TG) and four typical antihypertensive agents on in vivo ***:The five agents yielded different hypotensive effects and distinct regulation patterns on the perturbed metabolism that occurs in ***,amlodipine,and terazosin efficiently depressed systolic blood pressure(SP) but had a negligible regulatory effect on the perturbed metabolism of SHR,even after the remission of hypertension for 8 *** finding suggests that hypertension may not be the cause of the metabolism perturbation observed in these *** contrast,TG gradually and markedly regulated the perturbed metabolism, and led to a mild but sustained depression of SP up to 2 weeks after the cessation of ***, TG downregulated free fatty acids(FFA) toward normal levels;the elevation of FFA correlates with blood pressure,in both animals and ***:These results suggest that TG may act as a special antihypertensive agent that regulates the perturbed metabolism of hypertension.
