OBJECTIVE： To systematically assess the clinical ef- fectiveness of traditional Chinese medicine （TCM） in the treatment of functional dyspepsia （FD） of liv- er-stomach disharmony syndrome by meta-analysis. METHODS： Random controlled trials （RCTs） were retrieved from databases, including Pubmed, China national knowledge infrastructure, Wanfang Data, VlP Information, and the Cochrane Library. Trials were selected according to inclusion criteria. The ef- fects of traditional Chinese medicine （TCM） versus prokinetic agents in the treatment of functional dyspepsia （FD） of liver-stomach disharmony syn- drome were compared by meta-analysis. RevMan 5.0.24 was used for data analysis. The effective rate was assessed by odds ratios （ORs）, and 95% confi- dence intervals （C＇/s） were calculated. The cure rate was analyzed by the Peto OR. Simple statistica~ anal- ysis was chosen to assess the frequency of pre- scribed Chinese herbs in treating this ***： Thirteen trials were included, involving 1153 patients, and these were of poor methodolog- ical quality. Twelve studies mentioned the effective rate and cure rate. TCM therapy showed a better clinical effect rate compared with that with proki- netic agents [OR： 3.2, 95% CI （2.27, 4.51）]. The TCM group also had a better cure rate than that in the group of prokinetic agents [Peto OR： 2.26, 95% CI （1.61, 3.18）]. With regard to the frequency of Chi- nese herbs used in these 13 trials, Baishao （Radix Paeoniae）, Chaihu （Radix Bupleuri）, and Gancao （Ra- dix Glycyrrhizae） were mostly prescribed, followed by Xiangfu （Rhizoma Cyperi）, Zhishi （Fructus Auran- tii Immaturus）, Zhiqiao （Fructus Aurantii）, Foshou （Citrus medica ***）, and Chenpi （Peri- carpium Citri Reticulatae）. No serious adverse ef- fects were reported. AIM: To investigate the prevalence of celiac disease (CD) in adult patients referred to an open access gas-troenterology clinic in the south of Italy and submitted to esophago-gastro-duodenoscopy (EGD) for evaluation of refractory functional ***: Seven hundred and twenty six consecutive dyspeptic patients (282 male, 444 female; mean age 39.6 years, range 18-75 years) with unexplained prolonged dyspepsia were prospectively enrolled. Duo-denal biopsies were taken and processed by standard staining. Histological evaluation was carried out according to the Marsh-Oberhuber ***: The endoscopic findings were: normal in 61.2%, peptic lesions in 20.5%, malignancies in 0.5%, miscellaneous in 16.7%. CD was endoscopically diagnosed in 8 patients (1.1%), histologically in 15 patients (2%). The endoscopic features alone showed a sensitivity of 34.8% and specificity of 100%, with a positive predictive value (PPV) of 100% and a negative predictive value (NPP) of 97.9%.CONCLUSION: This prospective study showed that CD has a high prevalence (1:48) in adult dyspeptic patients and suggests the routine use of duodenal biopsy in this type of patient undergoing EGD.

Acupuncture is widely used to treat functional dyspepsia with satisfactory outcomes. Combination of the He and Mu acupoints is commonly used and has a synergistic effect on functional dyspepsia; however, its underlying mechanisms remain unclear. Therefore, a randomized controlled parallel clinical trial is currently underway at Chengdu University of Traditional Chinese Medicine, China. This trial is designed to explore the efficacy of and central responses to the He-Mu point combination in patients with functional dyspepsia using functional magnetic resonance imaging. A total of 105 patients with functional dyspepsia will be allocated into 3 groups： the low-He point group（puncturing at Zusanli（ST36））, Mu point group（puncturing at Zhongwan（CV12））, and He-Mu point combination group（puncturing at ST36 and CV12）. Every participant will receive 20 sessions of manual acupuncture for 4 weeks. The needles will be inserted perpendicularly to a depth of 1 to 2 cun. The angle of rotation and twisting will range from 90 to 180 degrees, while lifting and thrusting will range from 0.3 to 0.5 cm. The various manipulations will be performed 60 to 90 times per minute. The needles will remain in place for 30 minutes, during which manipulation will be applied every 10 minutes. Magnetic resonance imaging will be performed before and after 20 sessions of acupuncture. The primary outcome is symptom improvement according to the Chinese version of the Nepean dyspepsia Index. Secondary outcomes include the Leeds dyspepsia questionnaire, Self-Rating Anxiety Scale, Self-Rating Depression Scale, Beck Anxiety Inventory, Beck Depression Inventory, and visual analogue scale scores before and after 10 and 20 sessions of acupuncture. Needle sensation and adverse events will be used to assess the therapeutic effects. AIM:To evaluate the relationship between gallbladder (GB) motor function and H pylori infection in the stomach. METHODS: All cases (86) underwent the 14C urea breath test (UBT). 14C-UBT was found as positive in 58 and negative in 28 dyspeptic patients. 14C- UBT was accepted as a gold standard test. Clo test and histopathologic examination were compared with the results of 14C-UBT in cases who tolerated upper gastrointestinal endoscopy procedure. Cholescintigraphy with 99mTc-mebrofenin was used to determine the parameters of GB motor function (GB filling and emptying time, half of the emptying time, ejection fraction at 30th and 60th min) in all patients. RESULTS: We found the sensitivity and specificity as 88% and 86% for Clo test and as 89% and 80% for histologic evaluation, respectively. The parameters of GB function were not significantly different in H pylori positive and negative patients. The GB emptying was normal in both groups. Minimum GB filling time was 30 min in 34 of 86 cases (39.5%), filling was not observed in 2 cases. The GB ultrasonography (USG) results were normal for all cases and bile composition abnormality was not ***: Our study showed that 14C-UBT is highly reliable method to detect the presence of H pylori. The presence of H pylori infection does not directly affect the GB function.
