The Efficacy and Safety of Yokukansankachimpihange for Treating behavioral and Psychological Symptoms of Dementia in Patients with Alzheimer’s Disease: An Open-Label Pilot Study

Previous clinical trials have demonstrated the efficacy of yokukansan, a traditional Japanese medicine, for the treatment of behavioral and psychological symptoms of dementia (BPSD). However, less evidence is available for the treatment of BPSD with yokukansankachimpihange (YKSCH), which consists of yokukansan and two additional herbal ingredients. The present study was conducted to investigate the efficacy and safety of YKSCH for treating BPSD in patients with Alzheimer’s disease (AD). We enrolled outpatients with mild-to-moderate AD who exhibited BPSD and obtained a Neuropsychiatric Inventory (NPI) score of >3 including subscale scores for “agitation”, “anxiety”, “irritability”, and “sleep and night-time behavior change”. A daily YKSCH dose of 7.5 g was administered for 12 weeks with concomitant administration of anti-dementia medication. BPSD was evaluated using the NPI at baseline and every 4 weeks during the intervention. We also examined apathy using the Japanese translation of the Apathy Scale, the short version of the Japanese version of the Zarit Caregiver Burden Interview, and the Modified Crichton Rating Scale for Predicting Activities of Daily Living. Cognitive dysfunction was evaluated using the Mini Mental State Examination and the AD Assessment Scale-Cognitive (Japanese version). Five participants were enrolled. The NPI total score tended to decrease between the baseline and 8-week evaluations during the YKSCH intervention (Wilcoxon signed rank test, P = 0.063). In terms of the NPI subscale scores, “apathy”, “agitation”, “delusions”, and “sleep and night-time behavior change” decreased after the intervention in those who exhibited each symptom at baseline. There were no significant differences in the other scores examined. No serious adverse events were observed. YKSCH could ameliorate BPSD in patients with mild-to-moderate AD with agitation, anxiety, irritability, and sleep and night-time behavior change, and it was well-tolerated.

Context: With the view to reorient both STI/HIV/AIDS prevention and adolescents pregnancies, this research study aims at evaluating cognitive and behavioral acquisitions, as well as the process of interactive sex education participatory approach among adolescents in Kinshasa high schools. Methods: Based on a “pre and post” virtually experimental design, two crosswise surveys were conducted in Kinshasa, for six months in 2011-2012 on 484 high school students (pre-survey) and on 441 high school students (post-survey), whose age range from 14 - 19 years including both sexes. Two participatory educational talks (PET) “A” and “B”, covered weekly in two different schools, were compared to a control group school. The PET “A” consisted of interactive interpersonal communication sessions given by an external expert as a substitute for the life education course in one school. The PET “B” carried out in another school, included more educational talk sessions, led by the external expert and supplemented by a close follow-up of teenagers divided into small groups of 10 participants. The subjects’ assessment was based on their knowledge, attitudes and practices relating to STI/HIV/AIDS dual prevention as well as to unwanted pregnancies. Results: The subjects involved in the PET “B” displayed a better/higher performance based on their knowledge, attitudes and practices related to STI/HIV/AIDS dual prevention and unwanted pregnancies. Broadly speaking, knowledge has been improved 6 times with the PET “B” (OR = 6, 10, IC 95%) (3.24 - 11.9), and 3 times with the PET “A” (OR = 3, 45, IC 95%) (1.79 - 6.81), compared to control school. Similarly, findings on subjects’ attitudes show an improvement rated 12 times with the PET”B” (OR = 11, 99, IC 95%) (5.67 - 27.38) and 5 times for the PET “A” (OR = 5.51, IC 95%) (2.54 - 12.87). Background: behavioral feeding disorders are common among children, which sometimes become progressive, and consequently, children may refuse to eat anything. Parents have lots of difficulties to reset such a disturbed eating pattern. The aim of this study was to perform an analysis of clinical intervention in behavioral feeding disorders in young children. Methods: We conducted a retrospective analysis of data of 28 children aged 1 - 9 years with behavioral feeding disorders. A pediatrician and pediatric social worker conducted the training in two groups: outpatient or inpatient setting. Both groups were treated with parental education and guidance. The inpatient group also had a temporarily (2 weeks) resetting of the pedagogic climate in a pediatric ward of a general hospital under guidance of a pediatric social worker. Results: Almost all parents were inconsistent in applying appropriate behavioral contingencies during meals. Eleven patients followed 8 months of outpatient treatment and 25 patients followed 2 weeks of inpatient treatment. The overall success rate of outpatient treatment after 2 weeks was 18%, and that of inpatient treatment after 8 months was 88%. The corrected relapse rates are 18% and 56% respectively after 6 months. Conclusion: Short clinical intervention in a structured pedagogic environment is a successful treatment in behavioral feeding disorders. Herewith, pediatricians have a powerful tool for treating behavioral feeding disorders by temporarily resetting and changing the pedagogic climate.

Children with attention deficit and hyper activity disorder have various problems such as low academic performance, social and communication problems, anxiety, depression, aggression, conduct disorder and delinquency, and also when they become adults, they have problems in social relationships that these problems can increase or decrease in dealing with family. Hence, this study was performed in order to examine the impact of family-based behavioral treatment compared with Barkley behavior therapy in reducing symptoms of children with attention deficit disorder—ADHD. To meet the aim, 20 of children with age range of 7 to 12 years old with attention deficit disorder—hyperactivity were selected in both experimental and control groups, and exposed to the treatment of behavioral family therapy groups and changes in behavior Barkley. Before and after the treatment, they were evaluated according to the scale of the Conner’s Parent Rating Scale-Revised Short Form (CPRS-R: S). Results of multivariate analysis of covariance indicated that there were significant differences between the experimental group who had received family-based behavioral treatments based on changes in behavior Barkley and a control group who had received drug treatment in variables, including conduct problems, learning problems, psychosomatic symptoms, and signs of impulsive hyperactivity, anxiety and hyperactivity (p < 0.01). These results could be guidance for counsel or sand therapists for children with attention deficit disorder—ADHD.

Objective: To evaluate the toxicological and psychotropic properties of Calotropis(C.) procera. Methods: C. procera leaves and root-bark aqueous extracts were evaluated for their toxic and behavioral effects using adult mice. Toxicity studies were carried out using Organisation for Economic Cooperation and Development guidelines 423 and 407 for acute and subacute evaluation. behavioral studies were performed using traction test, fireplace test, hole-board test and forced-swimming test to evaluate the sedative, anxiety and depressive-like activities of the extracts. Results: Very low acute toxicity was observed in mice that received both leaves and rootbark extracts. The subacute test showed some morphological, biochemical and hematological changes in the treated groups. behavioral assessment demonstrated anxiety effects on mice for C. procera leaf extract(400 mg/kg of body weight). Conclusions: The acute use of C. procera(leaves and root-barks) aqueous extracts could be considered as low toxic. However, their repeated uses could have harmful effect on some organs. Likewise, a single dose up to 400 mg/kg body weight of these extracts produce no sedative or depressive-like effect, but they possess possible dose dependent anxiety effect. Yet, more studies are necessary to relate these results to the chemical profile of the plant extracts.

Effect of Cognitive behavioral Intervention Therapy on Immune Index in Patients with Breast Cancer

Objective: To explore the effect of cognitive behavioral intervention therapy on the cellular immune function of patients undergoing radical mastectomy for the first time, and to provide a scientific basis for improving the treatment of breast cancer patients. Methods: 100 patients with breast cancer who were eligible for admission to the Department of Breast Surgery, Hainan Provincial People’s Hospital from January 2016 to December 2017 were selected as subjects. It is the first time to improve the radical mastectomy. They were randomly divided into control group and intervention group, 50 cases each. The control group used conventional treatment and routine care. On the basis of this, the intervention group conducted cognitive behavioral interventions including cognitive reconstruction and behavioral therapy. The patients were measured for serum IL-2, IL-4, IL-6, IL-8 and TNF-α levels on the second day after admission and three months after discharge. The changes of serum IL-2, IL-4, IL-6, IL-8 and TNF-α levels in the two groups were compared before and after cognitive intervention. Results: The concentrations of IL-2, IL-4, IL-6, IL-8 and TNF-α in the serum of the intervention group and the control group before intervention were compared. The difference was not statistically significant (P > 0.05). After the intervention, the serum levels of IL-2, IL-4 and IL-8 in the intervention group were lower than before the intervention. The concentration of TNF-α was higher than before the intervention. The difference was statistically significant (P Conclusion: Cognitive behavioral intervention therapy can change the levels of IL-2, IL-4, IL-6, IL-8 and TNF-α in serum of breast cancer patients. It has a positive effect on the regulation of immune function in breast cancer patients.

Compatibility is the precondition to ensure the correct interaction among components in composition process, how to make the mismatch components coordinate correctly is a vital problem in component composition. This paper first modeled component behavior by LTS and expressed action mapping as synchronous vector then defined the sequential relationship among synchronous vectors as adaptation contract. Thirdly we analyzed the different mismatch situations and corresponding adaptation strategies. At last designed adaptation algorithm to produce adaptor specification automatically and ensured the mismatch components can correct interaction under the mediation of adaptor and verified the validity of proposed method through an application system.

Ginsenoside Rd improves behavioral impairment of rats with acute plateau status by modulating synaptic plasticity

Objective:To investigate the protective effect of ginsenoside Rd on the improvement of the behavior and synaptic plasticity in rats with acute plateau status.Methods:A total of 60 Wistar rats were randomly divided into the control group,the model group,and the intervention group,with 20 rats in each group.The model was established in low-pressure oxygen chamber simulating the plateau,and the intervention group was administered with ginsenoside.Electron microscope was used to observe synaptic ultrastructure of hippocampal CA1 area,and analyze the structural parameters on the Gray I synaptic interface.Morris water maze and Y electric maze experiment were used for behavioral detection.Results:Compared with the control group,the number of electrical stimulation required for rat to avoid was increased in the model group,the latency in the Morris water maze was prolonged,the swimming distance was increased,and the frequency of crossing the platform was decreased.Under the electron microscope,the synaptic cleft was increased,the length of the synaptic active area was shorter,the postsynaptic density(PSD)was thinner,the flat synapse was increased,and the concave and perforated types were significantly reduced.Compared with the model group,the number of electrical stimulation required for rat to avoid was decreased in the intervention group,the latency in the Morris water maze was shortened,the swimming distance was decreased,and the frequency of crossing the platform was increased.Under the electron microscope,the synaptic cleft was decreased,PSD was thicker,the flat synapse was decreased,and the concave and perforated types were increased.Conclusion:Low pressure and low oxygen environment of plateau damages the plasticity changes of the synaptic structure and function.And to a certain extent,ginsenoside Rd reverses Gray I synaptic interface structure parameters,so as to improve the behavior performance of model rats at high altitude condition.

In nursing homes, antipsychotic prescribing decisions (APDs) for managing behavioral and psychological symptoms of dementia (BPSD) depend on the nursing staff’s feedback. Inappropriate APDs can result in the lack of timeliness, objectivity and important clinical information when nursing staff’s feedback on residents’ behavior and pharmacotherapy outcomes. Currently, there are no reported interventions for improving psychiatrists’ APDs through nursing staff’s monitoring and feedback processes. This one-group pre-and-post pilot study aimed to evaluate the feasibility and impact of implementing a newly-developed Psychotropic Use Monitoring (PUM) program for improving the appropriateness of APDs in a 50-bed dementia ward of a nursing home. The PUM intervention involved 16 pharmacist-trained nursing staff, who monitored and reported residents’ BPSD changes and psychotropic side effects for 24 weeks, while carrying out their routine care duties. A face-to-face interview was then administered to determine the nursing staff’s perceptions of PUM. Data of 51 residents were collected from hardcopy individual patient records to evaluate the changes in APDs and the number of resident falls before and after implementing PUM. The nursing staff reported increases in their knowledge, awareness, confidence, and actual frequency of monitoring for side effects, as well as their ability in differentiating and managing BPSD (p < 0.05). After PUM, there was a significant increase in the number of APDs due to side effect-related reasons (4 versus 16) (p < 0.031). Although not significant, the number of APDs with no documented reasons (5 versus 9) and the number of resident falls (7 versus 15) appeared to be lesser after PUM. This study demonstrated the nursing staff’s positive participation in PUM intervention, specifically in monitoring and feedback of side effects.
