This study aimed to report the outcomes of active surveillance(AS)in the management of low-risk prostate cancer(PCa).It recruited87 men who were prospectively followed up according to the Prostate Cancer Research International Active Surveillance(PRIAS)protocol with local adaptation at SH Ho Urology Centre,Prince of Wales Hospital,Hong Kong,China.We investigated the predictorsof disease progression and found that baseline prostate-specific antigen density(PSAD)and the presence of the highest ProstateImaging-Reporting and Data System(PI-RADS)score 5 lesion on magnetic resonance imaging(MRI)are significantly correlatedwith disease progression.Moreover,men with PSAD>0.2 ng ml^(−2)or PI-RADS 4 or 5 lesions had significantly worse upgradingfree survival compared to those with PSAD≤0.2 ng ml−2 and PI-RADS 2 or 3 lesions.The study concludes that AS is a safe andeffective management strategy for selected patients to defer radical treatment and that most disease progression can be detectedafter the first repeated biopsy.The combination of PSAD>0.2 ng ml^(−2)and PI-RADS 4 or 5 lesions may serve as a useful predictorof early disease progression and provide a guide to optimize follow-up protocols for men in different risk groups.

Background: Localized pancreatic cancer, including resectable (R), borderline resectable (BR) and locally advanced unresectable disease (LAU), is considered in clinical guidelines for diverse treatment options based on clinical trials in selected populations. Hence, exploring with real world evidence (RWE) clinicians’ preferences for treatment options and their results seems pertinent. Methods: In a set of consecutive patients with Localized pancreatic cancer assisted in a third level hospital from January 2013 to December 2022, medical records, symptoms, diagnostic process, distribution between subtypes, and treatment plans, with safety and efficacy results, were assessed. Results: A total of 152 patients with Localized disease were included (43.4% R, 21.0% BR, 33.6% LAU). The population characteristics exemplified differences between daily practice and clinical trials. Tumor location and symptoms were as expected. Treatment plan was conditioned by PS or comorbidities in 23.0% of patients. In patients with R disease, surgery followed by different adjuvant chemotherapy (CT) regimes was the antineoplastic treatment of choice (64.8%) with efficacy results (OS 37.5 months;95% CI 18.4 - 56.7), in the range of contemporary standards. The common use of neoadjuvant CT for BR disease (94.4%), with surgery in 50% of them, and its results (OS 30.8 months;95% CI 10.5 - 51.2) reflected current controversies of treatment recommendations and evolution in this scenario. Paliative CT with or without radiotherapy was the standard specific treatment in LAU disease (95.1%) with survival results (PFS: 10.8 months;95% CI 8.8 - 12.7. OS: 20.3 months;95% CI 13.5 - 27.2) that justify the distinct character and the specific study of this entity. Conclusion: RWE for Localized pancreatic cancer aroused from the analysis of this population confirms the distinct nature of patients assisted in daily practice, as well as mirrors the complexity of decision making in clinical assumptions in which achieving stronger evidence should be paramount.

A fundamental understanding of the charge transport mechanism in two-dimensional semiconductors(e.g., MoS2) is crucial for fully exploring their potential in electronic and optoelectronic devices. By using monolayer graphene as the barrier-free contact to MoS2, we show that the field-modulated conductivity can be used to probe the electronic structure of the Localized states. A series of regularly distributed plateaus were observed in the gate-dependent transfer curves. Calculations based on the variable-range hopping theory indicate that such plateaus can be attributed to the discrete Localized states near mobility edge. This method provides an effective approach to directly profiling the Localized states in conduction channel with an ultrahigh resolution up to 1 meV.

To clarify the corrosion mechanism associated with the precipitate of T1(Al2CuLi)in Al-Li alloys,the simulated bulk precipitate of T1 was fabricated through melting and casting.Its electrochemical behavior and coupling behavior with α(Al)in 3.5% NaCl solution were investigated.Meanwhile,the simulated Al alloy containing T1 particle was prepared and its corrosion morphology was observed.The results show that there exists a dynamic conversion corrosion mechanism associated with the precipitate of T1.At the beginning,the precipitate of T1 is anodic to the alloy base and corrosion occurs on its surface.However,during its corrosion process,its potential moves to a positive direction with immersion time increasing,due to the preferential dissolution of Li and the enrichment of Cu.As a result,the corroded T1 becomes cathodic to the alloy base at a later stage,leading to the anodic dissolution and corrosion of the alloy base at its adjacent periphery.It is suggested that the Localized corrosion associated with the precipitate of T1 in Al-Li alloys is caused by the alternate anodic dissolution of the T1 precipitate and the alloy base at its adjacent periphery.

We obtain exact spatial Localized mode solutions of a(2+1)-dimensional nonlinear Schr¨odinger equation with constant diffraction and cubic-quintic nonlinearity in PT-symmetric potential, and study the linear stability of these solutions. Based on these results, we further derive exact spatial Localized mode solutions in a cubic-quintic medium with harmonic and PT-symmetric potentials. Moreover, the dynamical behaviors of spatial Localized modes in the exponential diffraction decreasing waveguide and the periodic distributed amplification system are investigated.

The existence and properties of intrinsic Localized spin-wave modes in a ferromagnetic XXZ spin chain with Dzyaloshinsky–Moriya interaction are investigated analytically in the semiclassical limit. The model Hamiltonian is quantized by introducing the Dyson–Maleev transformation and the coherent state representation is chosen as the basic representation of the system. By making use of the method of multiple scales combined with a quasidiscreteness approximation, the equation of motion for the coherent-state amplitude is reduced to the nonlinear Schr¨odinger equation.It is shown that a bright intrinsic Localized spin-wave mode whose eigenfrequency lies below the bottom of the magnon frequency band can exist in the ferromagnetic system. We also show that the system can produce a dark intrinsic Localized spin-wave mode, i.e., nonpropagating kink, whose eigenfrequency is below the upper of the magnon frequency band. In addition, we find that the introduction of the Dzyaloshinsky–Moriya interaction changes wave numbers in the Brillouin-zone corresponding to the appearance of intrinsic Localized spin-wave modes.

Background Although Localized neuroblastoma has a good prognosis,some cases have undergone treatment failure or recur-rence.Apart from biologic features such as MYCN status,we wondered whether some characteristics of growing tumors are prognostic,such as a well-encapsulated mass without infiltration of vital organs.We analyzed the diagnostic utility of image-defined risk factors(IDRFs)to predict successful treatment and prognosis.The overall goal was to achieve maximum cure rates for patients with Localized neuroblastoma through a better understanding of clinical characteristics.Methods We retrospectively reviewed the images of patients with Localized neuroblastoma who were enrolled between June 1998 and December 2012 at a single institution in Shanghai,China.Unequivocal categorization regarding IDRFs was available in 67 patients.IDRF was assessed at diagnosis and after four cycles of neoadjuvant chemotherapy,on average.The median follow-up period was 84 months(range: 48-132 months)after diagnosis.Results MRI and CT indicated a total of 177 IDRFs in these 67 patients.Logistic regression analysis revealed a highly significant negative correlation between the numbers of IDRFs and the possibility of complete removal of neuroblastoma.Intraspinal extension of the tumor,compression of the trachea,and encasement of the main artery in Localized neuroblastoma were predictors for incomplete tumor resection.According to univariate analysis,≥4 IDRFs and intraspinal extension of the tumor were significant indicators of poor prognosis.Conclusions The number of IDRFs was useful in predicting surgical outcome and event-free survival.The number of IDRFs should be considered in protocol planning,instead of IDRF presence or absence.

Comparing to orthogonal Localized molecular orbitals (OLMO), the nonorthogonal Localized molecular orbitals (NOLMO) exhibit bonding pictures more accordant with those in the traditional chemistry. They are more contracted, so that they have a better transferability and better performances for the calculation of election correlation energies and for the linear scaling algorithms of large systems. The satisfactory NOLMOs should be as contracted as possible while their shapes and spatial distribution keep in accordance with the traditional chemical bonding picture. It is found that the spread of NOLMOs is a monotonic decreasing function of their orthogonality, and it may reduce to any extent as the orthogonality descends. However, when the orthogonality descends to some point, the shapes and spatial distribution of the NOLMOs deviate drastically from the traditional chemical bonding picture, and finally the NOLMOs tend to linear dependence. Without the requirement of orthogonalization, some other constraints have to be imposed for constructing satisfactory NOLMOs by minimizing their spread functional. It is shown that satisfactory results can be generated by coupling the minimization of orbital spread functionals with the maximization of the distances between orbital centroids.
