Using Laparoscope to remove an ectopic intrauterine device in the anterior wall of urinary bladder: A case report

BACKGROUND:An intrauterine device (IUD) is a contraceptive device placed in the uterine cavity and is a common contraceptive method for Chinese women. However, an IUD may cause complications due to placement time, intrauterine pressure and other factors. Ectopic IUDs are among the most serious complications. Ectopic IUDs are common in the myometrium and periuterine organs, and there are few reports of ectopic IUDs in the urinary bladder, especially in the anterior wall.CASE SUMMARY:A 52-year-old woman was hospitalized due to a urinary bladder foreign body found abdominal ultrasound and computed tomography (CT) examination. The patient had a 2-year history of recurrent abdominal distension and lower abdominal pain, accompanied by frequent urination, urgency, dysuria and other discomfort. Ultrasound examination revealed foreign bodies in the bladder cavity, with calculus on the surface of the foreign bodies. CT revealed a circular foreign body on the anterior wall of the urinary bladder, suggesting the possibility of an ectopic IUD. After laparoscopic exploration, an annular IUD was found in the anterior wall of urinary bladder, and an oval calculus with a diameter of approximately 2 cm was attached to the surface of the bladder cavity. The IUD and calculus were successfully and completely removed. The patient recovered well after surgery.CONCLUSION:Abdominal ultrasound and CT are effective methods for detecting ectopic IUDs. The IUD is located in the urinary bladder and requires early surgical treatment. The choice of surgical method is determined by comprehensively considering the depth of the IUD in the bladder muscle layer, the situation of complicated calculus, the situation of intravesical inflammation and medical technology and equipment.

Prospective study of the effect of ERAS on postoperative recovery and complications in patients with gastric cancer

Objective:To study the efficacy of the enhanced recovery after surgery(ERAS)program on postoperative recovery and complications in patients with gastric cancer.Methods:Eighty patients in the perioperative period with radical gastrectomy were enrolled and randomly divided into 2 groups,the ERAS group and the non-ERAS group.The differences between the 2 groups in terms of postoperative recoveries and complications rate were determined.According to the body mass index(BMI)level,the ERAS group was divided into 2 subgroups,namely group A(BMI<28 kg/m2,n=16)and group B(BMI≥28 kg/m2,n=24).The non-ERAS group was also divided into group C(BMI<28 kg/m2,n=18)and group D(BMI≥28 kg/m2,n=22).The recovery and complications of each group were then determined.Results:The postoperative length of stay and visual analogue scale pain score were less in the ERAS group than the non-ERAS group(P<0.05).Time to first postoperative exhaustion,first postoperative defecation,returning leukocyte count to normal,and stopping intravenous nutrition were significantly shorter in the ERAS group(n=40),compared to the non-ERAS group(n=40,all P<0.05).The incidence of postoperative lower extremity intramuscular venous thrombosis was significantly higher in group D than in group B(χ2=4.800,P=0.028).In addition,the incidence of lower extremity intermuscular venous thrombosis and lung infection in group D was higher than those in other groups.Conclusions:The perioperative ERAS program was associated with faster recovery in patients undergoing radical gastrectomy.For patients with higher BMI(BMI≥28 kg/m2),the use of the perioperative ERAS program was more advantageous.

＜正＞A new laparoscopic method was used for percutaneous placement of the peritoneal end of cerebrospinal fluid shunts in patients with obstructive or normal-pressure hydrocephalus. The peritoneal end of shunts were placed and fixed upward between liver and diaphragm by two-Trocar method under direct laparoscopic vision. Meanwhile, patients with post-operation obstruction at peritoneal end of shunts underwent laparoscopic exploring and adjusting. One obstruction case at the peritoneal pipe end occurred on the third day after operation, two obstructed by the first month and the other obstructed by the sixth month after operation. All operations have been completed successfully, as well as all obstructions were adjusted successful under the Laparoscope. All patients were followed up from 6 to 38 months and kept unobstructed. Compared with the traditional open-abdominal method, this method has fewer traumas, quicker recovery and lower ratio of the pipe end obstructions. Besides, it’ s convenient to adjust obstructed peritoneal end of shunts.

Short-term Outcomes of Laparoscopic vs.Open Hepatectomy for Primary Hepatocellular Carcinoma:A Prospective Comparative Study

Laparoscopic hepatectomy(LH)is a newly developed technique associated with advantages as open surgery,but the study on outcome of liver function recovery was scarce.This preliminary report was aimed to comparatively assess the short?term outcomes between LH and open hepatectomy(OH)for primary hepatocellular carcinoma(PHC).This study retrospectively analyzed the demographic data and short-term outcomes of 81 patients who underwent LH or OH for the primary treatment of PHC between Oct.2017 and May 2018 at Union Hospital,Tongji Medical College,Huazhong University of Science and Technology(China).A total of 81 PHC patients who received major liver resection were enrolled.There were 38(47%)patients in the LH group and 43(53%)patients in the OH group.The operative time was significantly longer(373.53±173.38 vs.225.43±55.08,P<0.01),and hospital stay(17.34±5.93 vs.21.70±6.89,P=0.003),exhaust time(2.32±0.62 vs.3.07±0.59,P<0.01)and defecation time(2.92±0.78 vs.3.63±0.58,P<0.01)were significantly shorter in LH group than in OH group.The recovery of liver function was significantly faster in LH group,including higher serum albumin(P=0.002),higher ratio of albumin/globulin(P=0.029)and lower direct bilirubin(P=0.001)than in OH group.It is suggested that LH can serve as a fast recovery and cheap surgical procedure in the treatment of PHC,which is safe and feasible.

Superiority of Laparoscopy in the Peritoneal Dialysis Catheter Reset Surgery

Peritoneal dialysis catheter surgery has been used in clinical treatment for nearly 40 years, and open surgery under local anesthesia is the conventional method. However, catheter displacement after open surgery is still the thorny issue during our clinical practice. Then the reset surgery is often required to be taken again. Nowadays, laparoscopic peritoneal dialysis catheter draws our attention due to its advantages of accurate positioning, smaller incision, and less pain, and its clinical application has been limited. While laparoscopic surgery is recognized, there are few relevant studies on whether there is difference during the catheter reset process between the two surgical approaches. In this study, we mainly discussed the rate of secondary catheter migration, the incidence of complications after catheter reset for two surgical approaches and the hospital stay as well as the total clinical cost for the two surgical approaches. In this study, we retrospectively analyzed 25 cases of end-stage renal disease, who received catheterization for peritoneal dialysis and regular peritoneal dialysis in our hospital from March 2010 to December 2013, and had a medical history of catheter migration. We collected the relevant clinical data for all patients. Fifteen patients selected laparoscopic catheter reset, and 10 patients selected the traditional surgical method for catheter reset by themselves. For all patients enrolled, we analyzed the incidence of secondary catheter migration and postoperative complications, hospitalization time, and total cost for different methods of reset. Through the studies above, we found that laparoscopic peritoneal dialysis catheter surgery offered accurate catheter location and a small incision that was easy to heal. Besides, the incidence of postoperative complications for the laparoscopic surgery was lower than that for traditional surgical approach for catheter reset. Early Clinical Efficacy of Laparoscopic Sleeve Gastrectomy as a Bariatric Surgery for Obese Patients: A Uni-center Report in China

Objective To report the experience with laparoscopic sleeve gastrectomy as a bariatric surgery in our center. Methods Twenty obese patients were followed up for i year after receiving laparoscopic sleeve gastrectomy as a bariatric surgery in our center from January 2009 to October 2010, during which their general conditions, complications, and improvement of obesity-related diseases were assessed. Results Of the 20 patients, 19 underwent laparoscopic sleeve gastrectomy and 1 underwent open sleeve gastrectomy with no death occurred. The average weight loss was 36.4＋10.0 kg, the average BMI decreased from 46.1＋11.5 to 33.6＋5.6 kg/m2, and the excess weight loss was 55.9%＋14.2% one year after the operation. The majority of obesity-related diseases were improved. In one year after the operation, excellent, good, fairly good and poor scores were achieved in 2 （10.0%）, 8 （40.0%）, 7 （35.0%）, and 3 patients （15.0%）, respectively. Conclusion Laparoscopic sleeve gastrectomy as a bariatric surgery is a safe and feasible procedure for obesity with excellent short-term effects. More studies with a long-term follow-up are needed to validate its benefits.

Evaluation of short-and medium-term efficacy and complications of ultrasound-guided ablation for small liver cancer

BACKGROUND To ensure clinical efficacy and prolong patient survival,treatments such as surgery and microwave ablation(MWA)are used for early liver cancer.MWA is preferred because it effectively preserves the normal liver tissue and causes transient coagulation necrosis of local liver tumor cells.However,due to technical limitations,the cancerous liver tissue cannot be completely ablated;therefore,the probability of local tumor recurrence is high.AIM To investigate the clinical efficacy and safety of ultrasound-guided percutaneous MWA in the treatment of small liver cancer.METHODS A total of 118 patients treated for small liver cancer in The Central Hospital of Yongzhou from January 2018 to April 2019 were selected.Sixty-six patients received ultrasound-guided percutaneous MWA(MWA group)and 52 received laparoscopic surgery(Laparoscope group).The operation time,blood loss,hospital stay,and medical expenses of both groups were statistically analyzed.Serum alanine aminotransferase(ALT),aspartate aminotransferase(AST),total bilirubin(TBIL),albumin(ALB),alpha fetal protein(AFP),carcinoembryonic antigen(CEA),and peripheral blood regulatory T lymphocytes(Treg)levels were evaluated pre-and post-operatively.The cross-sectional area of tumors measured before and after ablation was analyzed statistically;the therapeutic effect was compared between both groups in terms of surgical complications,2-year progression-free survival rate,and overall survival rate.RESULTS The operation time,blood loss,hospital stay,and medical expenses in the MWA group were lower than those of the Laparoscope group,and the differences were significant(P0.05).The operation time,blood loss,hospital stay,and medical expenses for 2 d and 1 wk after surgery,the ALT and AST of the MWA group were lower than those of the Laparoscope group,and the difference was significant(P<0.05).The operation time,blood loss,hospital sta

Comparison of clinical efficacy and postoperative inflammatory response between laparoscopic and open radical resection of colorectal cancer

BACKGROUND In recent years,the incidence of colorectal cancer(CRC)has increased annually,which has seriously threatened the health and quality of life of patients.In the treatment of CRC,both laparoscopic and radical resection are widely used.AIM To explore and discuss clinical efficacy and postoperative inflammatory response of laparoscopic and open radical resection of CRC.METHODS A total of 96 patients with CRC diagnosed in our hospital from March 2016 to April 2021 were selected,and were divided into the study group(n=48)and control group(n=48)using a simple random method.The control group was treated with open radical resection of CRC,and the study group was treated with laparoscopic radical resection of CRC.The perioperative conditions(operation time,intraoperative blood loss,the recovery time of gastrointestinal function,number of lymph node dissections and length of hospital stay),inflammatory response index levels[interleukin(IL)-6,IL-8,IL-10,C-reactive protein(CRP)]before and after operation,pain stress response indices[levels of neuropeptide(NPY),prostaglandin E2(PGE2),5-hydroxytryptamine(5-HT)],and the incidence of the complications between the two groups were counted.RESULTS The operation time in the study group was(186.18±33.54 min),which was longer than that of the control group(129.38±26.83 min),but the intraoperative blood loss(111.34±21.45 mL),recovery time of gastrointestinal function(25.35±4.55 h),and hospital stay(10.09±2.38 d)were better than those in the control group(163.77±32.41 mL,36.06±7.13 h,13.51±3.66 d)(P0.05).There was no significant difference between the levels of serum IL-6(9.79±4.11 ng/mL),IL-8(3.79±1.71 ng/L),IL-10(48.96±12.51 ng/L)and CRP(7.98±2.33 mg/L)in the study group and the control group(10.56±3.78 ng/mL,4.08±1.45 ng/L,50.13±11.67 ng/L,8.29±2.60 mg/L)before the operation(P>0.05).After the operation,there was no signific

Laparoscopic colonic anastomosis using a degradable stent in a porcine model

AIM: To explore the feasibility and safety of laparoscopic colonic anastomosis using a degradable stent in a porcine model.METHODS: Twenty Bama mini-pigs were randomly assigned to a stent group(n = 10) and control group(hand-sewn anastomosis, n = 10). The anastomotic completion and operation times were recorded, along with histological examination, postoperative general condition, complications, mortality, bursting pressure, and the average anastomotic circumference(AC).RESULTS: All pigs survived postoperatively except for one in the stent group that died from ileus at 11 wk postoperatively. The operation and anastomotic completion times of the stent group were significantly shorter than those of the control group(P = 0.004 and P = 0.001, respectively). There were no significant differences in bursting pressure between the groups(P = 0.751). No obvious difference was found between the AC and normal circumference in the stent group, but AC was significantly less than normal circumferencein the control group(P = 0.047, P < 0.05). No intestinal leakage and luminal stenosis occurred in the stent group. Histological examination revealed that the stent group presented with lower general inflammation and better healing.CONCLUSION: Laparoscopic colonic anastomosis with a degradable stent is a simple, rapid, and safe procedure in this porcine model.

Comparative analysis of two digestive tract reconstruction methods in total laparoscopic radical total gastrectomy

BACKGROUND The incidence of gastric cancer has significantly increased in recent years.Surgical resection is the main treatment,but the method of digestive tract reconstruction after gastric cancer surgery remains controversial.In the current study,we sought to explore a reasonable method of digestive tract reconstruction and improve the quality of life and nutritional status of patients after surgery.To this end,we statistically analyzed the clinical results of patients with gastric cancer who underwent jejunal interposition double-tract reconstruction(DTR)and esophageal jejunum Roux-en-Y reconstruction(RY).AIM To explore the application effect of DTR in total laparoscopic radical total gastrectomy(TLTG)and evaluate its safety and efficacy.METHODS We collected the relevant data of 77 patients who underwent TLTG at the Fourth Hospital of Hebei Medical University from October 2021 to January 2023.Among them,35 cases were treated with DTR,and the remaining 42 cases were treated with traditional RY.After 1:1 propensity score matching,the cases were grouped into 31 cases per group,with evenly distributed data.The clinical characteristics and short-and long-term clinical outcomes of the two groups were statistically analyzed.RESULTS The two groups showed no significant differences in basic data,intraoperative blood loss,number of lymph node dissections,first defecation time after operation,postoperative hospital stay,postoperative complications,and laboratory examination results on the 1st,3rd,and 5th days after operation.The operation time of the DTR group was longer than that of the RY group[(307.58±65.14)min vs(272.45±62.09)min,P=0.016],but the first intake of liquid food in the DTR group was shorter than that in the RY group[(4.45±1.18)d vs(6.0±5.18)d,P=0.028].The incidence of reflux heartburn(Visick grade)and postoperative gallbladder disease in the DTR group was lower than that in the RY group(P=0.033 and P=0.038).Although there was no significant difference in body weight,hemoglobin,prealbumin,and albumin betwe
