Optimal control analysis of hepatitis c virus with acute and chronic stages in the presence of treatment and infected immigrants

In this paper, we consider a deterministic hepatitis c virus （HcV） model and study the impact of optimal control on the screening of immigrants and treatment of HcV on the transmission dynamics of the disease in a homogeneous population with constant immigration of susceptibles. First, we derived the condition in which disease-free equilibrium is locally asymptotically stable and established that a stable disease-free equilibrium can only be achieved in the absence of infective immigrants. Second we investigated the impact of each control mechanism individually and the combinations of these strategies in the control of HcV. The costs associated with each of these strategies are also investigated by formulating the costs function problem as an optimal control problem, and we then use the Pontryagin＇s Maximum Principle to solve the optimal control problems. From the numerical simulations we found that the optimal combination of treatment of acute-infected and chronic-infected individuals control strategy produced the same results as the combination of the three strategies （combination of screening of immigrants, treatment of acute-infected and chronic-infected individuals）. By our model and these results, we suggest the treatment of acute-infected and chronic-infected individuals control strategy should be optimized where resources are scarce, because the implementation of the three strategies （combination of screening of immigrants, treatment of acute-infected and chronic-infected individuals） would imply additional cost.

Retreatment of patients with treatment failure of directacting antivirals: Focus on hepatitis c virus genotype 1b

The recent development of direct-acting antiviral agents(DAAs) against hepatitis c virus(HcV) infection could lead to higher sustained virological response(SVR) rates, with shorter treatment durations and fewer adverse events compared with regimens that include interferon. However, a relatively small proportion of patients cannot achieve SVR in the first treatment, including DAAs with or without peginterferon and/or ribavirin. Although retreatment with a combination of DAAs should be conducted for these patients, it is more difficult to achieve SVR when retreating these patients because of resistance-associated substitutions(RASs) or treatment-emergent substitutions. In Japan, HcV genotype 1 b(GT1 b) is founded in 70% of HcVinfected individuals. In this minireview, we summarize the retreatment regimens and their SVR rates for HcV GT1 b. It is important to avoid drugs that target the regions targeted by initial drugs, but next-generation combinations of DAAs, such as sofosbuvir/velpatasvir/voxilaprevir for 12 wk or glecaprevir/pibrentasvir for 12 wk, are proposed to be potential solution for the HcV GT1 b-infected patients with treatment failure, mainly on a basis of targeting distinctive regions. clinicians should follow the new information and resources for DAAs and select the proper combination of DAAs for the retreatment of HcV GT1 b-infected patients with treatment failure.

Post-liver transplant hepatitis c virus recurrence: An unresolved thorny problem

hepatitis c virus (HcV)-related cirrhosis represents the leading cause of liver transplantation in developed, Western and Eastern countries. Unfortunately, liver transplantation does not cure recipient HcV infection: reinfection universally occurs and disease progression is faster after liver transplant. In this review we focus on what happens throughout the peri-transplant phase and in the first 6-12 mo after transplantation: during this crucial period a completely new balance between HcV, liver graft, the recipient’s immune response and anti-rejection therapy is achieved that will deeply affect subsequent outcomes. Nearly all patients show an early graft reinfection, with HcV viremia reaching and exceeding pre-transplant levels; in this setting, histological assessment is essential to differentiate recurrent hepatitis c from acute or chronic rejection; however, differentiating the two patterns remains difficult. The host immune response (mainly cellular mediated) appears to be crucial both in the control of HcV infection and in the genesis of rejection, and it is also strongly influenced by immunosuppressive treatment. At present no clear immunosuppressive strategy could be strongly recommended in HcV-positive recipients to prevent HcV recurrence, even immunotherapy appears to be ineffective. Nonetheless it seems reasonable that episodes of rejection and over-immunosuppression are more likely to enhance the risk of HcV recurrence through immunological mechanisms. Both complete prevention of rejection and optimization of immunosuppression should represent the main goals towards reducing the rate of graft HcV reinfection. In conclusion, post-transplant HcV recurrence remains an unresolved, thorny problem because many factors remain obscure and need to be better determined.

AIM: To elucidate the mechanism(s) by which S-adenosyl-L-methionine(SAM) decreases hepatitis c virus(HcV) ***: We examined the effects of SAM on viral expression using an HcV subgenomic replicon cell culture system. Huh7 HcV-replicon cells were treated with 1 mmol/L SAM for different times(24-72 h), then total RNA and proteins were isolated. c DNA was synthesized and real time-PcR was achieved to quantify HcV-RNA, superoxide dismutase 1 and 2(SOD-1, SOD-2) catalase, thioredoxin 1, methionine adenosyltransferase 1A and 2A(MAT1A, MAT2A) expression, and GAPDH and RPS18 as endogenous genes. Expression of cellular and viral protein was evaluated by western-blot analysis using antibodies vs HcV-NS5 A, SOD-1, SOD-2, catalase, thioredoxin-1, MAT1 A, MAT2 A, GAPDH and actin. Total glutathione levels were measured at different times by Ellman's recycling method(0-24 h). Reactive oxidative species(ROS) levels were quantified by the dichlorofluorescein assay(0-48 h); Pyrrolidin dithiocarbamate(PDTc) was tested as an antioxidant control and H2O2 as a positive oxidant ***: SAM exposition decreased HcV-RNA levels 50%-70% compared to non-treated controls(24-72 h). SAM induced a synergic antiviral effect with standard IFN treatment but it was independent of IFN signaling. In addition, 1 mmol/L SAM exposition did not modify viral RNA stability, but it needs cellular translation machinery in order to decrease HcV expression. Total glutathione levels increased upon SAM treatment in HcV-replicon cells. Transcriptional antioxidant enzyme expression(SOD-1, SOD-2 and thioredoxin-1) was increased at different times but interestingly, there was no significant change in ROS levels upon SAM treatment, contrary to what was detected with PDTc treatment, where an average 40% reduction was observed in exposed cells. Production and pathogenicity of hepatitis c virus core gene products
