Successful endoscopic surgery for emphysematous pyelonephritis in a non-diabetic patient with autosomal dominant polycystic kidney disease: A case report

BACKGROUND Emphysema pyelonephritis(EPN)is a very dangerous type of urinary tract *** is a lethal disease that develops rapidly and causes the patient to deteriorate rapidly,and it can easily lead to systemic infections and even *** incidence is extremely low,and it is prevalent in patients with *** here report a case of EPN in a non-diabetic patient with autosomal dominant polycystic kidney disease(ADPKD).We share the diagnosis and treatment procedure for this extremely rare condition to make this disease easier to identify and address *** SUMMARY A 47-year-old woman presented to the emergency department of our hospital with a high fever and left back pain lasting 4 *** had a history of autosomal dominant polycystic kidney and polycystic *** was diagnosed with left type I EPN and her vital signs deteriorated so quickly that she underwent an emergency operation in which a D-J tube was inserted into her left ureter on the second day after *** months later,she underwent a second-stage flexible ureteroscopy and *** postoperative sepsis,she finally recovered after active symptomatic support treatment and effective anti-infective *** Although EPN is more likely to occur in diabetic patients,for non-diabetic patients with ADPKD and upper urinary tract obstruction,the disease also causes rapid *** and accurate diagnosis and timely removal of the obstruction by invasive means may be able to save the damaged kidney and the patient’s life.

Submucosal injection material has shown protective effect against gastrointestinal injury during endoscopic surgery in ***,the protective ability of existing submucosal injection material is strictly limited by their difficult injectability and short barrier ***,we report a shear-thinning gellan gum hydrogel that simultaneously has easy injectability and long-lasting barrier function,together with good hemostatic property and ***-thinning property endows our gellan gum hydrogel with excellent endoscopic injection performance,and the injection pressure of our gellan gum hydrogel is much lower than that of the small molecule solution(50 wt%dextrose)when injected through the endoscopic *** importantly,our gellan gum hydrogel shows much stronger barrier retention ability than normal saline and sodium hyaluronate solution in the ex vivo and in vivo ***,our epinephrine-containing gellan gum hydrogel has a satisfactory hemostatic effect in the mucosal lesion resection model of *** results indicate an appealing application prospect for gellan gum hydrogel utilizing as a submucosal injection material in endoscopic surgery.

Current developments in natural orifices transluminal endoscopic surgery:An evidence-based review

Tremendous advances have been made in recent years addressing the key obstacles to safe performance and introduction of human natural orifice transluminal endoscopic surgery (NOTES). Animal studies have focused on identifying optimal solutions to these obstacles, in particular methods of creating transluminal access,safe closure of the point of access, and development of a multitasking platform with dedicated instruments. Whether the performance data generated from these animal studies can be reproduced in humans has yet to be determined. Reports of human NOTES procedures are emerging, and the possibility of accomplishing human NOTES based on existing technology has been demonstrated. However, dedicated platforms and devices are still lacking to allow for pure NOTES procedures, and whether NOTES can deliver the postulated benef its of earlier recovery and improved cosmesis remains uncertain.

Natural orifice transluminal endoscopic surgery:Where are we going?

The foundation for natural orifice transluminal endoscopic surgery(NOTES) is to access the peritoneal and other body cavities through the wall of the alimentary tract via natural orifices,with the goal of performing procedures within the peritoneum and other cavities,without the need to make incisions in the abdominal *** have made great progress in the field of NOTES since the publication of the White Paper in *** are still major fundamental goals as outlined by the Society of American Gastrointestinal and endoscopic Surgeons/American Society for Gastrointestinal Endoscopy joint committee that need to be evaluated and answered before NOTES is ready for widespread clinical *** include prevention of infection,instrument development,creation of a multitasking platform,and the ability to recognize and treat intraperitoneal complications such as hemorrhage and other physiological adverse *** response to this need,recent abstracts and papers have focused on the management of intraoperative *** next phase is to focus on controlled prospective multicenter clinical trials that compare defined NOTES procedure to standard *** goal is to produce reliable and convincing data for the United States Food and Drug Administration,insurance companies,the physician community and the general *** the present time,we still have many important milestones that still need to be *** investigators agree that a hybrid technique and not a pure NOTES practice should be advocated until devices can meet the current and new challenges in this f ield.

The evolution of sacrocolpopexy and development of novel techniques via natural orifice transluminal endoscopic surgery

Prolapse of one or more pelvic organs through the vagina,such as the uterus,bladder,or rectum,is an increasingly common occurrence in the aging female population,with rates approaching 60%of parous women.1 Its associated symptoms of vaginal bulge or bladder,bowel,or sexual dysfunction can drastically affect a woman's quality of *** surgical options for the treatment pelvic organ prolapse exist and may involve autologous tissue repair or the use of biological or synthetic *** sacrocolpopexy,first described in 1957 by Arthure and Savage,involves using the sacrum as a point of support for the vaginal apex.2 The technique was further refined by Huguier and Scali by the incorporation of a graft to improve tissue strength and to improve postoperative anatomical cure rates.3 For many surgeons,abdominal sacrocolpopexy is the preferred surgical technique as it has demonstrated superior outcomes with higher postoperative success rates through restoration of normal anatomy and lower rates of prolapse recurrence and postoperative dyspareunia.

endoscopic Transsphenoidal surgery for Pituitary Adenoma. Early Experience in Sohag University Hospitals

Objective: To report the results of our first series of patients undergoing a purely endoscopic endonasal approach for pituitary surgery in Sohag University Hospitals. Patients and Methods: We reviewed 20 consecutive patients during the period from January 2017 to January 2020 with pituitary adenoma who underwent purely endoscopic transsphenoidal resection of their lesions. The patients’ clinical outcomes, degrees of tumor removal, and complications were reported. Results: There were 18 primary and 2 recurrent adenomas (19 nonfunctioning and 1 functioning GH secreting adenoma). The average degree of gross total removal for tumors was 70%, C.S.F leak occurred in 20%, diabetes insipidus in 10% and sellar hemorrhage in 5%. There was no post-operative mortality, vascular injury, sphenoiditis, septal perforation, epistaxis or general complication. Conclusion: A purely endoscopic approach for pituitary adenoma treatment is a safe and effective alternative to the traditional microscopic procedure.

Exposed endoscopic full-thickness resection(EFTR)without laparoscopic assistance is a minimally invasive natural orifice transluminal endoscopic surgery technique that is emerging as a promising effective and safe alternative to surgery for the treatment of muscularis propria-originating gastric submucosal *** date,various techniques have been used for the closure of the transmural postEFTR defect,mainly consisting in clip-and endoloop-assisted closure ***,the recent advent of dedicated tools capable of providing full-thickness defect suture could further improve the efficacy and safety of the exposed EFTR *** aim of our review was to evaluate the efficacy and safety of the different closure methods adopted in gastric-exposed EFTR without laparoscopic assistance,also considering the recent advent of flexible endoscopic suturing.
