Objective:To evaluate whether low-frequency ultrasound-facilitated transdermal delivery of a Chinese medicine(CM)formula could improve the efficacy of intrapleural administration of interleukin?2(IL-2)in treatment of malignant pleural effusion(MPE).Methods:A total of 110 eligible participants were randomized into the low-frequency sonophoresis(LFS)of CM(LSF/CM)group(55 cases)and the control group(55 cases)by simple randomization using a random number *** control group was treated with an intrapleural administration of IL-2;and the LFS/CM group was treated with LFS of a CM gel formulation,combined with the same IL-2 injection as in the control *** CM formula consisted of Semen Lepidii,Semen Sinapis,Flamulus Cinnamomi,Poriacocos,Herba Lycopi,and Radix Paeoniae *** 2-week treatment,the therapeutic outcome was determined by the change of the amount of MPE,which was evaluated by B-scan ultrasound and/or chest X-ray,and the change of quality of life(QOL)scores,which were evaluated by the Eastern Cooperative Oncology Group(ECOG)performance ***:A significantly higher objective remission rate(ORR)was obtained with intrapleural IL-2 plus LFS/CM than IL-2 treatment alone(P=0.049).In addition,more patients in the LFS/CM group than in the control group had an improved QOL score(P=0.048),and no patients in the LFS/CM group had a reduced ***:LFS of CM formulation could effectively alleviate MPE and improve the QOL of cancer patients.

Association of asymptomatic otitis media with effusion in patients with adenoid hypertrophy

Objective: Clinical symptoms of otitis media with effusion are rarely brought forward to the guardians of young children who the disease is most prevalent in. This often leads to poor scholastic performances and difficult social interactions. The objective of this study was to identify asymptomatic cases of otitis media with effusion present in individuals with adenoid hypertrophy. Material and Methods: In a cross sectional study advocated in Justice *** Hospital, Karnataka India we evaluated one hundred patients above the age of three from August 2016 to December 2017. Candidates who presented with an adenoid nasopharyngeal ratio of more than 0.5 were selected for the study. Individuals who complained of otological symptoms were not considered for the study. Patients cleared of other pathological otological conditions were underwent audiological evaluation with pure tone audiometry and tympanometry for evaluating the middle ear status and hearing loss. Results: The study showed a total of 36% of patients evaluated presented with asymptomatic otitis media with effusion. In candidates who presented with a bilateral B tympanogram, 40% had significant conductive hearing loss of more than 25dB. Conclusion: An objective test such as impedance audiometry in all patients with adenoid hypertrophy would aid in the diagnosis of fluid in the middle ear, so that timely intervention can be done and possible complications be averted.

Otitis media with effusion(OME)is a frequent paediatric *** condition is often asymptomatic,and so can easily be ***,OME can lead to hearing loss that impairs the child's language and behavioural *** diagnosis is essentially clinical,and is based on otoscopy and(in some cases)*** endoscopy is only indicated in cases of unilateral OME or when obstructive adenoid hypertrophy is *** media with effusion is defined as the observation of middle-ear effusion at consultations three months *** must be evaluated(using an age-appropriate audiometry technique)before and after treatment,so as not to miss another underlying cause of deafness(*** deafness).Craniofacial dysmorphism,respiratory allergy and gastro-oesophageal reflux all favour the development of *** a certain number of medications(antibiotics,corticoids,antihistamines,mucokinetic agents,and nasal decongestants)can be used to treat OME,they are not reliably effective and rarely provide long-term *** benchmark treatment for OME is placement of tympanostomy tubes(TTs)and(in some cases)adjunct *** TTs rapidly normalize hearing and effectively prevent the development of cholesteatoma in the middle *** contrast,TTs do not prevent progression towards tympanic atrophy or a retraction *** enhances the effectiveness of *** children with adenoid hypertrophy,adenoidectomy is indicated before the age of 4 but can be performed later when OME is identified by nasal *** must be followed up until OME has disappeared completely,so that any complications are not missed.

Relevance of adenosine deaminase as a marker for tuberculous pleural effusion in developing countries

Objective:Relevance of estimation of pleural adenosine deaminase(PADA) and serum adenosine deaminase (SADA) levels in-pleural effusion especially in cases of lymphocytic predominant exudative tubercular effusions. Methods:Fifty patients(33 male and 17 female;age:44.12±11.51 years) with pleural effusions were selected to assay adenosine deaminase(ADA) activity in pleural fluid and serum in adjunct to pleural fluid *** were individually classified as transudates or exudates after careful evaluation of all the biochemical parameters of pleural fluid and serum of patients and on the basis of Lights *** value for PADA was taken as 60U/L and that for pleural/serum ADA ratio(P/S ADA) was ***:Fourty -three patients had exudative effusions among which 38 patients had tuberculous pleural effusions and 5 had nontubercular effusions.7 cases were *** PADA levels in tubercular group(78.95±25.32 U/ L) were found to be much higher P=0.0000) than nontubercular(23.00±5.22 U/L) *** levels in tubercular group(31.05±6.42 U/L) were significantly higher(P=0.0000)as compared to nontubercular group(15.58±8.35 U/L).PADA cutoff at 60 U/L yielded sensitivity *** of 81.5%and 100%respectively,whereas P/S ADA ratio at 1.8 gave sensitivity and specificity of 84.2% and 75%respectively. A positive correlation(r=0.507,P= 0.001 1)between PADA and SADA was found in tubercular group but no such correlation(r=0.302,P=0.3407)was observed in nontubercular ***: The measurement of ADA in tubercular pleural effusions has not only relevance but also a high diagnostic utility when other clinical and laboratory tests are either negative or confusing.

TRANSFORMATION OF effusion AND FORMATION OF GRANULATION TISSUE IN PATHOLOGIC PROCESS OF OTITIS MEDIA WITH effusion

Objective To explore the diversity of stagnant effusions and the pathologic processes leading to granulation tissue formation in otitis media with effusion(OME). Methods Temporal bone slides from 306 ears with OME were studied histopathologically under the light microscope. Results Results of this study revealed a pathologic process in witch the type and the condition of stagnant effusion in the middle ear cleft was a variable in the dynamics of OME progression from an early stage to an advanced stage. The location of granulation tissue and retentive effusion were found to be closely related. Conclusion Early stage granulation tissue formation exhibited a pathologic process in which granulation tissue formation occurred only in areas where effusion had stagnated or was absorbed. The incidence of the retentive effusion and formation of granulation tissue was much higher and the pathologic changes most extensive in the area around the ossicular chain.

To the Editor:Otitis media with effusion(OME),also known as secretory otitis,is a disease in which secreted fluid accumulates in the middle ear cavity and is a major cause of hearing loss,both in children and adults.[1]Causes of OME include recurrent infections,inflammatory conditions,malformations,and eustachian tube dysfunctions.[2]Although most patients could recover spontaneously,by medication,or by surgery,some patients show frequent recurrence of otitis media for more than 3 months,called chronic otitis media with effusion(COME).[1]The pathogenesis of COME is not completely *** evidence suggests a central role of immunologic cells and th&r associated cytokines in the chronic inflammation that characterizes COME.

Sonographic Evaluation of Pleural effusion

Background: Although at times small amounts of pleural fluid is detected on the lateral decubitus chest radiograph, this may be impossible to obtain in severely ill patients. Because of its ready availability and ability for bedside imaging, ultrasonography has become a crucial imaging modality not only in detecting the presence of pleural fluid but also as a guide to aspiration. Aims: To sonographically determine the nature of pleural effusions. To analyze predictability of both benign and malignant pleural effusions and to study statistical value of various ultrasound characteristics in differentiating exudative and transudative as well as benign and malignant effusions in correlation with thoracocentesis. Material and Methods: Sonographic feature as well as fluid cytology was evaluated. Following categorization into exudates or transudate as well as benign or malignant the diagnosis was then correlated biochemically. Results: Transudates were anechoic, while an anechoic effusion may be either a transudate or an exudate. Complex septation, internal echoes, thickened pleura or homogeneously echogenic patterns were always exudates. Sonographic findings of pleural nodules and associated parenchymal lesions in the lung and liver were indicative of malignancy. Conclusions: Ultrasound in addition to being highly sensitive for pleural effusion also aided in characterizing the nature and minimizing the complications during thoracocentesis. Pleural effusions were categorized into exudates and transudate as well as benign and malignant with a certain degree of confidence based on sonographic findings.
