High ce rvical spinal co rd injuries induce permanent neuromotor and autonomic deficits.These injuries impact both central respiratory and Cardiovascular functions through modulation of the sympathetic nervous system.So far,Cardiovascular studies have focused on models of complete contusion or transection at the lower cervical and thoracic levels and diaphragm activity evaluations using invasive methods.The present study aimed to evaluate the impact of C2 hemisection on different parameters representing vital functions(i.e.,respiratory function,Cardiovascular,and renal filtration parameters)at the moment of injury and 7 days post-injury in rats.No ventilatory parameters evaluated by plethys mography were impacted during quiet breathing after 7 days post-injury,whereas permanent diaphragm hemiplegia was observed by ultrasound and confirmed by diaphragmatic electromyography in anesthetized rats.Interestingly,the mean arterial pressure was reduced immediately after C2 hemisection,with complete compensation at 7 days post-injury.Renal filtration was unaffected at 7 days post-injury;however,remnant systolic dysfunction chara cterized by a reduced left ventricular ejection fraction persisted at 7 days post-injury.Taken together,these results demonstrated that following C2 hemisection,diaphragm activity and systolic function are impa cted up to 7 days post-injury,whereas the respiratory and Cardiovascular systems display vast ada ptation to maintain ventilatory parameters and blood pressure homeostasis,with the latter likely sustained by the remaining descending sympathetic inputs spared by the initial injury.A better broad characterization of the physiopathology of high cervical spinal cord injuries covering a longer time period post-injury could be beneficial for understanding evaluations of putative therapeutics to further increase cardiorespiratory recovery.

Background Previous prediction algorithms for Cardiovascular diseases(CVD)were established using risk factors retrieved largely based on empirical clinical knowledge.This study sought to identify predictors among a comprehensive variable space,and then employ machine learning(ML)algorithms to develop a novel CVD risk prediction model.Methods From a longitudinal population-based cohort of UK Biobank,this study included 473611 CVD-free participants aged between 37 and 73 years old.We implemented an ML-based data-driven pipeline to identify predictors from 645 candidate variables covering a comprehensive range of health-related factors and assessed multiple ML classifiers to establish a risk prediction model on 10-year incident CVD.The model was validated through a leave-one center-out cross-validation.Results During a median follow-up of 12.2 years,31466 participants developed CVD within 10 years after baseline visits.A novel UK Biobank CVD risk prediction(UKCRP)model was established that comprised 10 predictors including age,sex,medication of cholesterol and blood pressure,cholesterol ratio(total/high-density lipoprotein),systolic blood pressure,previous angina or heart disease,number of medications taken,cystatin C,chest pain and pack-years of smoking.Our model obtained satisfied discriminative performance with an area under the receiver operating characteristic curve(AUC)of 0.762±0.010 that outperformed multiple existing clinical models,and it was well-calibrated with a Brier Score of 0.057±0.006.Further,the UKCRP can obtain comparable performance for myocardial infarction(AUC 0.774±0.011)and ischaemic stroke(AUC 0.730±0.020),but inferior performance for haemorrhagic stroke(AUC 0.644±0.026).Conclusion ML-based classification models can learn expressive representations from potential high-risked CVD participants who may benefit from earlier clinical decisions.

Dear Editor,Cardiovascular diseases(CVDs)are the leading cause of death world-wide.Thus,diagnosing and treating CVD remains at the forefront for clinicians while identifying targetable disease mechanisms in preclinical models are focus areas for researchers and drug developers(Cai et al.,2022a).The polymorphic protein apolipoprotein E(APOE),central to lipid transport and metabolism,is well-recognized for the role of its isoforms as important predictors for human Cardiovascular disorders and neurodegenerative diseases(Tudorache et al.,2017).Plasma APOE is generated primarily from liver hepatocytes,accounting for around 75%of the APOE production from the whole body(Getz and Reardon,2009),and plays important functional roles in monocytes/macrophages,adipocytes,and the central nervous system(Kockx et al.,2018).However,despite the fact that APOE is widely expressed in different mammalian cells,studies on the functional roles of APOE mostly focus on its extracellular secreted form,and the specific effects of APOE,particularly intracellular form in cell types closely related to human Cardiovascular diseases are therefore still poorly understood.

In 2019,Cardiovascular disease(CVD)accounted for 46.74%and 44.26%of all deaths in rural and urban areas,respectively.Two out of every five deaths were attributed to CVD.It is estimated that approximately 330 million individuals in China are affected by CVD.Among them,there are 13 million cases of stroke,11.4 million cases of coronary heart disease,5 million cases of pulmonary heart disease,8.9 million cases of heart failure,4.9 million cases of atrial fibrillation,2.5 million cases of rheumatic heart disease,2 million cases of congenital heart disease,45.3 million cases of lower extremity artery disease,and 245 million cases of hypertension.With the dual challenges of population aging and a steady increase in the prevalence of metabolic risk factors,the burden of CVD in China is expected to continue rising.Consequently,new demands arise for CVD prevention,treatment,and the allocation of medical resources.Emphasizing primary prevention to reduce disease prevalence,increasing the allocation of medical resources for CVD emergency and critical care,and providing rehabilitation services and secondary prevention to reduce the risk of recurrence,rehospitalization,and disability among CVD survivors are of paramount importance.Hypertension,dyslipidemia,and diabetes affect millions of individuals in China.Since blood pressure,blood lipids,and blood sugar levels often rise insidiously,vascular disease and serious events such as myocardial infarction and stroke occur by the time they are detected in this population.Therefore,it is crucial to implement strategies and measures to prevent risk factors such as hypertension,dyslipidemia,diabetes,obesity,and smoking.Furthermore,greater efforts should be directed towards assessing Cardiovascular health status and conducting research on early pathological changes to enhance prevention,treatment,and understanding of CVD.

BACKGROUND Growth differentiation factor 15(GDF-15)has been explored as a potential biomarker for various inflammatory diseases and Cardiovascular events.This study aimed to assess the predictive role of GDF-15 levels in Cardiovascular events and all-cause mortality,considering traditional risk factors and other biomarkers.METHODS A prospective study was conducted and 3699 patients with stable coronary artery disease(CAD)were enrolled into the research.Baseline GDF-15 levels were measured.Median follow-up was 3.1 years during the study.We analyzed clinical variables and several biomarkers.Multivariable Cox regression analysis was performed to evaluate prognostic performance of GDF-15 levels in predicting myocardial infarction(MI),heart failure,stroke,Cardiovascular death,and non-Cardiovascular death.RESULTS Baseline GDF-15 levels for 3699 patients were grouped by quartile(≤1153,1153-1888,1888-3043,>3043 ng/L).Higher GDF-15 levels were associated with older age,male gender,history of hypertension,and elevated levels of N-terminal pro Btype natriuretic peptide(NT-pro BNP),soluble suppression of tumorigenesis-2(sST2),and creatine(each with PCardiovascular and non-Cardiovascular death[HR=2.48,95%CI(1.49-4.11)]during the median follow up of 3.1 years.CONCLUSIONS Higher levels of GDF-15 consistently provides prognostic information for Cardiovascular events and all cause death,independent of clinical risk factors and other biomarkers.GDF-15 could be considered as a valuable addition to future risk prediction model in secondary prevention for predicting clinical events in patient with stable CAD.

In recent decades,life expectancy has been increasing significantly.In this scenario,health interventions are ne-cessary to improve prognosis and quality of life of elderly with Cardiovascular risk factors and Cardiovascular disease.However,the number of elderly patients included in clinical trials is low,thus current clinical practice guidelines do not include specific re-commendations.This document aims to review prevention recommendations focused in patients≥75 years with high or very high Cardiovascular risk,regarding objectives,medical treatment options and also including physical exercise and their inclusion in cardiac rehabilitation programs.Also,we will show why geriatric syndromes such as frailty,dependence,cognitive impair-ment,and nutritional status,as well as comorbidities,ought to be considered in this population regarding their important pro-gnostic impact.

Hypoxia,characterized by reduced oxygen concentration,is a significant stressor that affects the survival of aerobic species and plays a prominent role in Cardiovascular diseases.From the research history and milestone events related to hypoxia in Cardiovascular development and diseases,The"hypoxia-inducible factors(HIFs)switch"can be observed from both temporal and spatial perspectives,encompassing the occurrence and progression of hypoxia(gradual decline in oxygen concentration),the acute and chronic manifestations of hypoxia,and the geographical characteristics of hypoxia(natural selection at high altitudes).Furthermore,hypoxia signaling pathways are associated with natural rhythms,such as diurnal and hibernation processes.In addition to innate factors and natural selection,it has been found that epigenetics,as a postnatal factor,profoundly influences the hypoxic response and progression within the Cardiovascular system.Within this intricate process,interactions between different tissues and organs within the Cardiovascular system and other systems in the context of hypoxia signaling pathways have been established.Thus,it is the time to summarize and to construct a multi-level regulatory framework of hypoxia signaling and mechanisms in Cardiovascular diseases for developing more therapeutic targets and make reasonable advancements in clinical research,including FDA-approved drugs and ongoing clinical trials,to guide future clinical practice in the field of hypoxia signaling in Cardiovascular diseases.

BACKGROUND Epidemiologic studies have explored the association between a single Cardiovascular risk factor(CVRF)and resting heart rate(RHR),but the research on the relation of multiple risk factors with RHR remains scarce.This study aimed to explore the associations between CVRFs clustering and the risk of elevated RHR.METHODS In this cross-sectional study,adults aged 35-75 years from 31 provinces were recruited by the China PEACE Million Persons Projects from September 2015 to August 2020.We focused on seven risk factors:hypertension,diabetes mellitus,dyslipidemia,obesity,smoking,alcohol use,and low physical activity.Multivariate logistic regression was used to calculate odds ratios(OR)for elevated RHR(>80 beats/min).RESULTS Among 1,045,405 participants,the mean age was 55.67±9.86 years,and 60.4%of participants were women.The OR(95%CI)for elevated RHR for the groups with 1,2,3,4 and≥5 risk factor were 1.11(1.08-1.13),1.36(1.33-1.39),1.68(1.64-1.72),2.01(1.96-2.07)and 2.58(2.50-2.67),respectively(Ptrend<0.001).The association between the CVRFs clustering number and elevated RHR was much more pronounced in young males than in other age-sex subgroups.Clusters comprising more metabolic risk factors were associated with a higher risk of elevated RHR than those comprising more behavioral risk factors.CONCLUSIONS There was a significant positive association between the CVRFs clustering number and the risk of elevated RHR,particularly in young males.Compared clusters comprising more behavioral risk factors,clusters comprising more metabolic risk factors were associated with a higher risk of elevated RHR.RHR may serve as an indicator of the cumulative effect of multiple risk factors.

Mineral and bone disorder(MBD)in chronic kidney disease(CKD)is tightly linked to Cardiovascular disease(CVD).In this study,we aimed to compare the prognostic value of nine MBD biomarkers to determine those associated best with adverse Cardiovascular(CV)outcomes and mortality.In 5217 participants of the German CKD(GCKD)study enrolled with an estimated glomerular filtration rate(eG FR)between 30–60 mL·min-1 per 1.73 m2 or overt proteinuria,serum osteoprotegerin(OPG),C-terminal fibroblast growth factor-23(FGF23),intact parathyroid hormone(iP TH),bone alkaline phosphatase(BAP),cross-linked C-telopeptide of type 1 collagen(CTX1),procollagen 1intact N-terminal propeptide(P1NP),phosphate,calcium,and 25-OH vitamin D were measured at baseline.Participants with missing values among these parameters(n=971)were excluded,leaving a total of 4246 participants for analysis.During a median follow-up of6.5 years,387 non-CV deaths,173 CV deaths,645 nonfatal major adverse CV events(MACEs)and 368 hospitalizations for congestive heart failure(CHF)were observed.OPG and FGF23 were associated with all outcomes,with the highest hazard ratios(HRs)for OPG.In the final Cox regression model,adjusted for CV risk factors,including kidney function and all other investigated biomarkers,each standard deviation increase in OPG was associated with non-CV death(HR 1.76,95%CI:1.35–2.30),CV death(HR 2.18,95%CI:1.50–3.16),MACE(HR 1.38,95%CI:1.12–1.71)and hospitalization for CHF(HR 2.05,95%CI:1.56–2.69).Out of the nine biomarkers examined,stratification based on serum OPG best identified the CKD patients who were at the highest risk for any adverse CV outcome and mortality.
