Prenatal metal mixture exposure and birth weight: A two-stage analysis in two prospective cohort studies

The understanding of the impact of prenatal exposure to metal mixtures on birth weight is limited.We aimed to identify metal mixture components associated with birth weight and to determine additional pairwise interactions between metals showing such associations.Concentrations of 18 metals were measured using inductively coupled plasma mass spectrometry in urine samples collected in the 3rd trimester from a prenatal cohort(discovery;n=1849)and the Healthy Baby cohort(replication;n=7255)in Wuhan,China.In the discovery set,we used two penalized regression models,i.e.,elastic net regression for main effects and a lasso for hierarchical interactions,to identify important mixture components associated with birth weight,which were then replicated.We observed that 8 of the 18 measured metals were retained by elastic net regression,with five metals(vanadium,manganese,iron,cesium,and barium)showing negative associations with Z-scores for birth weight and three metals(cobalt,zinc,and strontium)showing positive associations.In replication set,associations remained significant for vanadium(β=-0.035;95%confidence interval[CI],-0.059 to0.010),cobalt(β=-0.073;95%CI,0.049 to 0.097),and zinc(β=-0.040;95%CI,0.016 to 0.065)after Bonferroni correction.We additionally identified and replicated a single pairwise interaction between iron and copper exposure on birth weight(Pbirth weight.The approach could be used in other studies estimating the effect of complex mixtures on human health.

Assessing the Effectiveness of Strategies in US birth cohort Screening for Hepatitis C Infection

Chronic hepatitis C infection in the USA is a highly morbid condition and current guidelines recommend one-time screening among the birth cohort(1945-1965).Understand-ing strategies to optimize screening can help inform future hepatitis C virus(HCV)screening guidelines.A focused literature search was performed using PubMed and manual abstract review from major hepatology conferences over the past 2 years.The search strategy involved using Medical Subject Headings terms for hepatitis C,screening,birth cohort,baby boomers,and 1945-1965.The review was limited to data from the USA.A total of 327 articles were identified and 36 abstracts were included,with studies published between 2012-2019.Strategies including clinician education,electronic medical record alerts,reflex HCV RNA testing,point-of-care testing,multisite(outpatient,inpatient,emergency department,endoscopy suite)initiatives,direct patient solicitation,and utilization of non-physician providers have increased HCV screening rates.However,broad imple-mentation remains less than optimal.Barriers include lack of patient acceptance to screening and engagement in the HCV care cascade.The Veterans Affairs Healthcare System has achieved higher birth cohort screening rates through an integrated approach requiring high-level engagement by leadership and institutional commitment.Multiple strategies for increasing birth cohort screening have been successful,but overall rates of HCV screening remain low.These strat-egies can inform public health efforts to implement emerging national recommendations for expansion of HCV screening to all U.S.adults age 18 or older.

Background birth weight is a strong determinant of infant short-and long-term health outcomes.Family socioeconomic position(SEP)is usually positively associated with birth weight.Whether this association extends to abnormal birth weight or there exists potential mediator is unclear.Methods We analyzed data from 14,984 mother-infant dyads from the Born in Guangzhou cohort Study.We used multi-variable logistic regression to assess the associations of a composite family SEP score quartile with macrosomia and low birth weight(LBW),and examined the potential mediation effect of maternal pre-pregnancy body mass index(BMI)using causal mediation analysis.Results The prevalence of macrosomia and LBW was 2.62%(n=392)and 4.26%(n=638).Higher family SEP was associated with a higher risk of macrosomia(OR 1.30,95%CI 0.93-1.82;OR 1.53,95%CI 1.11-2.11;and OR 1.59,95%CI 1.15-2.20 for the 2nd,3rd,and 4th SEP quartile respectively)and a lower risk of LBW(OR 0.69,95%CI 0.55-0.86;OR 0.76,95%CI 0.61-0.94;and OR 0.61,95%CI 0.48-0.77 for the 2nd,3rd,and 4th SEP quartile respectively),compared to the 1st SEP quartile.We found that pre-pregnancy BMI did not mediate the associations of SEP with macrosomia and LBW.Conclusions Socioeconomic disparities in fetal macrosomia and LBW exist in Southern China.Whether the results can be applied to other populations should be further investigated.

Sex-specific effects of organophosphate ester exposure on child growth trajectories in the first two years

The connections between urinary organophosphate ester(OPE)metabolites and child growth have been identified in prior research,but there is currently a dearth of epidemiological evidence regarding the sex-specific impact of OPEs on child growth trajectories.This study enrolled 804 maternal-child pairs,and five OPE congeners were quantified in maternal serum during pregnancy.In this study,the impact of prenatal OPE exposure on child growth trajectories was assessed using linear mixed-effect models and a group-based trajectory model(GBTM),with consideration given to sex-specific effects.Fetuses were frequently exposed to OPEs in utero,and tris(2-butoxyethel)phosphate(TBEP)exhibited the highest concentration levels in maternal serum.Among male children,an increase of 2.72 ng/g lipid in TBEP concentration was associated with a 0.11-unit increase in head circumference-for-age z-score(HCAZ),and the effect was mainly concentrated at 1 and 2 months of age.Among female children,an increase of 2.72 ng/g lipid in tris(2-chloro-1-(chloromethyl)ethyl)phosphate(TDCPP)concentration was associated with a 0.15-unit increase in length-for-age z-score(LAZ)and a 0.14-unit increase in weight-for-age z-score(WAZ),and the effects were mainly concentrated at 9 months of age.For HCAZ trajectories,higher prenatal TBEP exposure was associated with higher odds for the fast growth group in male children.For the LAZ and WAZ trajectories,higher prenatal TDCPP exposure was associated with higher odds for the fast growth group in female children.The trajectory analysis approach provided insight into the complex associations between OPE exposure and child growth.

Comparative Analyses of Height Growth Velocities of School Boys in South Korea and Japan in the Past 50 Years

The paper compares the height growth velocities of male schoolchildren in South Korea and Japan over the period 1961-2018. Growth in height was measured with the same birth cohorts, not by comparing mean height of ascending ages in the same year. Starting from a lower economic base and mean height in the 1960s, high school males aged 17 in South Korea became 3 cm taller in mean height than their Japanese peers in the mid-2000s versus 2 - 3 cm shorter in the 1960s through 1970s. Children in Japan ceased to grow taller by the end of the 1980s, not because they quit taking more animal-sourced foods, meat and milk, but because they had drastically steered away from fruit and vegetables in their diets since the end of the 1970s. Having largely converged economically with Japan, South Korean children ceased to grow any taller in the mid-2000s. More importantly, it was discovered in this study that successive cohorts in South Korea started to fall gradually but steadily in height growth velocity from 1st graders in middle school, aged 12 years to 3rd graders in high school, and aged 17 years, to be once again 3 cm below their Japanese peers in the early-2010s. Analysis of Korea Household Expenditure Surveys classified by age groups of household head, decomposed by the author, revealed that children under 20 years of age in South Korea began to steer away from fruit and, particularly, vegetables in their at-home consumption in the mid-1990s, to average only 15% of the level of older adults in their 50s in the mid-2010s. 自20世纪80年代中期到现在的三十多年时间里，中国的出生人口、出生率和总和生育率数据存在巨大争议。官方公布的全国历年出生人口和出生率均不是直接来自人口普查、1％人口抽样调查或年度人口抽样调查，而是在直接调查基础上进行的调整。三十多年过去了，计划生育也即将落幕，但经官方修正后公布的历年出生人口数据是否合适，我们不得而知。2020年第七次全国人口普查漏登率只有0.05％，为我们重新估计以往出生人口提供了基础和依据。利用本次人口普查，结合2000年和2010年人口普查数据，通过用逆存活率方法回推从1986年到2020年各日历年的出生人口。估计结果显示，1991年以前官方公布的数据明显偏低，从1991年到2005年出生率存在明显高估，甚至影响了2000年第五次全国人口普查总人口的准确性；从2005年以后再次出现明显低估。2020年普查以后官方随即对近十年来公布的出生率进行了调整，调整结果是合适的。

Polycyclic aromatic hydrocarbons(PAHs) are ubiquitous air pollutants generated by combustion of organic material, including fossil fuel. It has been an open question whether prenatal exposure to PAHs significantly increases the risk of offspring neurobehavioral disorders. And whether DNA hydroxy methylation is a biomarker Here, we have examined this hypothesis in a birth cohort in Taiyuan City and a benzo[a] pyrene exposure animal model. We enrolled 400 mother-newborns pairs and collected theirs biological samples after written informed consent. Neonatal behavioral neurological assessment(NBNA) tests were conducted when the infants were three days old. In the follow-up, 221 mother-child pairs with 2-year-old children were included in this study. We used the Gesell Developmental Scales(GDS) for children neurodevelopmental assessments 2 years of age. Multiple linear regressions were used to analyze the associations of maternal urinary PAH metabolites and cord blood BPDE with child neurodevelopmental scores and 5-hmC levels, and restricted cubic spline models were further used to examine the shapes of dose-response relationships. A mediation analysis was also conducted. We observed dose-response associations of maternal urinary 1-hydroxypyrene(1-OHPyr)and sum of PAH metabolites( ∑-OHPAHs) with decreased neurodevelopmental scores(NBNA and GDS scores), and the 5-hmC levels of the BDNF and MeCP2 gene promoter(P<0.05). The 1-OHPyr was positively associated with cord blood global DNA 5-hmC levels. The 5-hmC level of BDNF and MeCP2 gene promoter were positively associated with neurodevelopmental scores. Mediation analysis showed the 5-hmC levels of BDNF gene promoter could explained 19.95% and 13.79% of the effect of motor scores change related to prenatal exposure to 1-OHPyr and ∑-OHPAHs(P<0.05), respectively.In animal study, Pregnant rats were exposed to benzo[a] pyrene at three dosages(10 mg·kg B[a]P,20 mg·kg B[a]P, 40 mg·kg B[a]P). Polycyclic aromatic hydrocarbons（PAHs） are ubiquitous air pollutants generated by combustion of organic material,including fossil fuel.It has been an open question whether prenatal exposure to air pollution in general and PAHs in particular significantly increases the risk of neonatal neurobehavioral disorders.Here,we have examined this hypothesis in a birth cohort in Taiyuan City and a benzo[a]pyrene exposure animal model.297 pregnant women enrolled in Taigang Hospital and the eighth Taiyuan people’s hospital in Taiyuan city from November 2009 to May 2010.A cohort of the mothers and their neonates participated in the study and information on each subject was obtained by questionnaire.PAHs metabolites were detected in the urine of mothers during pregnancy to evaluate the exposure level to PAHs.Neonate neurobehavioral developmental levels were assessed according to the standards of the Neonatal Behavioral Neurological Assessment（NBNA）.A 1 natural-log（ln）-unit increase in prenatal PAH exposure was associated with a 0.44 score decrease in total Score and with a 0.143 score decrease in behavior score as well as 0.153 passive tone score and 0.158 primary reflexes score of NBNA.NBNA total scores,behavior scores,passive tone scores,and primary reflexes scores between male and female newborns were not significantly different.In animal study,Pregnant rats were exposed to benzo[a]pyrene at three dosages（25 mg/kg B[a]P,50 mg/kg B[a]P,100 mg/kg B[a]P）.Neonatal development was evaluated at ages of 4,7,14 and 28 days.Physical development time of ear opening,body weight),behavior development（surface righting reflex,olfactory discrimination） and learning and memory in offspring were impaired in a B[a]P dose dependent manner.Our results in human population and animal model showed that Prenatal PAHs Exposure was the predominant risk factor for neonatal neurobehavioral development.

近20年来，第二次人口转变是用于解释全球范围内家庭领域发生新变化的最主要的理论框架。文章通过分析人口普查、1％人口抽样调查和中国家庭追踪调查数据，对我国第二次人口转变的主要指标进行了全面的评估，包括初婚年龄、同居率、离婚率以及生育方面的指标。研究结果表明，第二次人口转变在我国的发展与西方有着不同的轨迹，不同方面的家庭行为的变化程度差异较大。结婚与同居这些更为个体化的家庭行为经历了较为剧烈的变化，男性与女性不断推迟进入初婚的时间，同居也变得越来越普遍。在养育子女方面，婚外生育与已婚不育仍然非常少见。离婚率虽然有所上升，但整体还维持在较低的水平。文章提出，对我国家庭领域变化的理解应该充分考虑我国独特的社会、历史与文化背景。
