Candida albicans isolated from urine:Phenotypic and molecular identification,virulence factors and antifungal susceptibility

Objective: To isolate Candida albicans(C. albicans) from the urine of hospitalized patients and assess the virulence factors and antifungal susceptibility profiles of the ***: Yeasts were identified using the chromogenic medium CHROMagar?, the VITEK~?2 system, hypertonic Sabouraud broth, tobacco agar, polymerase chain reaction,and DNA sequencing. The evaluated virulence factors were proteinase production,phospholipase production, and biofilm production on polystyrene. The broth microdilution technique was used to determine the minimum inhibitory ***: All yeasts isolated from urine were identified as C. albicans using both classical and molecular methods. Although 91.3% of the isolates showed no phospholipase activity, 56.5% showed strong proteinase activity and 91.7% produced biofilm. All microorganisms were sensitive to fluconazole, voriconazole and amphotericin B, but 56.5% of the yeasts showed resistance to ***: C. albicans isolates from urine have a high capacity for virulence and can be associated with infectious processes. Furthermore, the high percentage of isolates resistant to itraconazole is important because this antifungal agent is commonly used to treat fungal infections in the hospital environment.

The upsurge of candidemia in the past years has been an immense encumbrance on public health and the number of deaths caused by candidemia particularly in critical care unit patients is devastating. Candida species harbor a 30% - 60% mortality rate and compared to stable people or those with less serious illnesses, this ranges from 60% to 80% of those who are chronically ill patients. Grounded on a recent report from a tertiary care hospital in Kenya showing the emergence of previously unobserved species: Candida auris, this study aimed to determine the prevalence, species distribution, and antifungal susceptibility profile of candidemia in critical care unit patients of the hospital. 378 Critical Care Unit patients were enrolled for the study from January 2019 to January 2020. Positive archived isolates were sub-cultured using Saboraud Dextrose Agar. Candida species were identified utilizing API20C AUX and Vitek-2. CandidaCo-Infection with Mycobacterium tuberculosisin Tuberculosis Patients and antifungal susceptibility of the Isolates

It had been observed that tuberculosis (TB) subjects can be co-infected with Candida sp. which was previously assumed as normal flora of the oral cavity. Candida sp. might become an opportunistic pathogen in immune compromised individuals. Candida co-infection with Mycobacterium tuberculosis in TB patients might complicate underlying disease process in the lungs. Materials and Methods: A total of 400 sputum samples were collected from TB patients and examined using Ziehl-Neelsen staining technique and MDR/RIF Genexpert system for TB. Samples positive for Mycobacterium tuberculosis were cultured on Sabouraud Dextrose Agar with gentamicin and examined for the presence of budding yeast cells and pseudohyphae on Gram’s stain. Candida sp. isolated from TB positive sputa were cultured on CHROMager Candida for identification to species level and subjected to antifungal susceptibility testing. Results: Out of 400 sputum samples examined for TB 93 (23.3%) were positive and 32 (34.4%) out of 93 TB positive cases were co-infected with Candida sp. Candida albicans was the most predominant species with a prevalence of 23 (67.6%), C. tropicals 4 (11.8%), C. krusei 4 (11.8%) and C. parapsilosis 3 (8.8%). One sample had dual infection. Female subjects had high prevalence (19.4%) than the male (15.7%). Age group 31 - 40 years had both high prevalence of TB 32.3% and Candida 25.0%. antifungal susceptibility testing showed that isolated Candida sp. were more susceptible to vericonazole and fluconazole compared to nystatin. Conclusions: Tuberculosis weakens the immune systems of infected persons especially when prophylactic administration proves abortive or there is non-adherence to treatment prescriptions. In the present study,the performance of ATB Fungus 3(bioMerieux,France),with the endpoints read visually and automatically,were evaluated for testing the antifungal susceptibility of Candida *** fluconazole, voriconazole,itraconazole and amphotericin B.A collection of 218 Candida isolates from CHIF-NET10 were tested, including 81 isolates of Candida albicans,42 of Candida glabrata sensu stricoto,41 of Candida tropicalis,38 of Candida parapsilosis complex and 16 of Candida *** microdilution(BMD) method was used as’goldstandard’ following CLSI *** results were considered to be in essential agreement(EA) when the test results were within two dilutions of the reference value. The EA between ATB Fungus 3 with results read visually and BMD results was 95.0%-100%.Amphotericin B(100%) presented the highest concordance for Candida *** the lowest EA(95.0%) was found for itraconazole. Discrepant EAs between the two methods for drug-organism combinations are as follows;fluconazole,3 isolates of *** sensu stricoto;voriconazole,1 of *** sensu stricoto,2 of ***,2 of *** complex;itraconazole,6 of *** sensu stricoto,4 of ***,1 of *** was also calculated for individual Candida *** the two *** drug-organism combinations for the two methods showed EAs of>90%,with the exceptions of itraconazole and *** sensu stricoto(85.7%EA). In contrast,the results read automatically showed higher discrepancies with BMD *** highest agreement was observed for amphotericin B(99.5%) and the lowest for itraconazole(71.6%).That’s because the highest MICs obtained were due to the trailing phenomenon,particularly with *** and some strains of ***. In conclusion,ATB Fungus 3 is a simple and accurate method for the determination of MICs for Candida *** the results read visually were more reliable than the results read automatically.

Fluconazole susceptibility and Genotypic Heterogeneity of Oral Candida albicans Colonies from the Patients with Cancer Receiving Chemotherapy in China

Aim To identify heterogeneity of Candida albicans （C. albicans） isolated from the population with cancer in China by using identification medium, subculture molecular typing, and antifungal susceptibility test. Methodology Oral cheek mucosal specimens from 52 cancer patients receiving chemotherapy were cultured on CHROMagar CandidaTM plates for Candida identification. All the C. albicans colonies on the plates were subcultured and reconfirmed by API20C, then submitted to the antifimgal drug susceptibility test with fluconazole and molecular typing using randomly amplified polymorphic DNA-PCR （RAPD） with primers RSD6 and *** 54% （28/52） patients were oral yeast carriage in which C. albicans predominated. More than 7 C. albicans colonies were isolated from each of 12 patients （Group A）, while less than 5 colonies were isolated from each of 16 patients （Group B）. RSD6 and RSD12 were successful in eliciting 17 （A1-A17） and 2 （B1-B2） genotypes, respectively from among the 205 isolates. The two primers were combined to generate 21 genotypes. The C. albicans isolates obtained from the same patient and episode showed a diversity for fluconazole revealed by MIC50 and MIC90. Conclusion The heterogeneity of the C. albicans colonies isolated from the same patients can be detected. C. albicans with varied fluconazole susceptibility and genotypic characteristics may coexist in the same oral Candida population.

Morphological and physiological comparison of taxa comprising the Sporothrix schenckii complex

Based on recent molecular data, it has been suggested that Sporothrix globosa is the main causal agent of sporotrichosis in China. The objective of this study was to compare the morphology, growth characteristics, patterns of carbon source usage, and susceptibility to antifungal agents among Sporothrix strains. A total of 15 clinical strains confirmed to be S. globosa, from three different regions of China, and 11 ex-type strains from the CBS-KNAW biodiversity center were obtained. The elongated conidia of S. pal/ida, S. variecibatus, S. schenckii, and S. schenckii luriei were clearly different from the subglobose and globose conidia of S. globosa strains. S. schenckii is able to assimilate sucrose, raffinose, and ribitol. susceptibility profiles of these Sporothrix species were evaluated by measuring minimum inhibitory concentrations （MICs）. Fluconazole, itraconazole, terbinafine, and amphotericin B showed good activity against most S. globosa clinical isolates from China. Potassium iodide also showed a low MIC against S. pal/ida, while fluconazole showed a high MIC for S. mexicana, S. humicola, S. g/obosa, S. schenckii, and S. inflata; these strains might be considered tolerant. The species showed differences in susceptibility to antifungal drugs and should therefore be properly identified during diagnosis prior to designing therapeutic strategies.

antifungal activity of ibuprofen against aspergillus species and its interaction with common antifungal drugs

Background The incidence of invasive aspergillosis （IA） has increased in frequency in immunocompromised patients with a variety of diseases. The poor prognosis might be due to limited treatment option. This study aimed to evaluate antifungal activity of ibuprofen against clinical isolates of aspergillus species, as well as its interaction with azoles or with amphotericin B or with micafungin. Methods antifungal activity of ibuprofen against 10 strains of Aspergillus fumigatus, Aspergillus flavus, and Aspergillus terreus were tested with both disk diffusion assay and standard broth microdilution method. To determine whether ibuprofen combined with itraconazole, voriconazole, amphotericin B, or micafungin had interactive effects on aspergillus spp., we used both disk diffusion assay and Chequerboard method. Results As for disk diffusion method, ibuprofen produced a zone of growth inhibition with diameters of （20.1±3.9） mm at 48 hours of incubation. As for broth microdilution method, the minimal inhibitory concentration （MIC） ranges of ibuprofen against aspergillus spp. were 1000-2000 μg/ml, and the minimal fungicidal concentration （MFC） ranges of that was 2000-8000 μg/ml. For 2 of 5 isolates, when ibuprofen combined with itraconazole or voriconazole, the zones of growth inhibition were larger than those of the individual drug. The results of Chequerboard method showed that fractional inhibitory concentration index （FICI） ranges were 1.125-2.500. Conclusions Ibuprofen is active against aspergillus spp.. And ibuprofen does not affect the in vitro activity of itraconazole, voriconazole, amphotericin B or micafungin against aspergillus spp.
