Cognitive impairment and its association with circulating biomarkers in patients with acute decompensated heart failure

BACKGROUND Cognitive impairment(CI)is common in patients with heart failure(HF),but the association between CI and biomarkers related to HF or cognitive decline in patients with HF remains *** This prospective observational study investigated the incidence of CI,subsequent cognitive changes,and the asso-ciation between CI and novel biomarkers in patients with left ventricular ejection fractionhospitalized for acute decompensated *** were evaluated for CI,depressive symptoms,and quality of life with the Mini-Mental State Examin-ation(MMSE)and the Mini-Cog,Beck Depression Inventory(BDI)-II,and Kansas City Cardiomyopathy Questionnaire(KCCQ),*** primary endpoint was a composite of all-cause mortality or hospitalization for HF at one *** Among the 145 patients enrolled in this study,54 had CI(37.2%)at *** mean MMSE increased signific-antly at the 3-month and 1-year follow-up,accompanied by decreased BDI-II and increased KCCQ *** improvement in the MMSE scores mainly occurred in patients with *** the biomarkers assayed,only growth/differentiation factor(GDF)-15>1621.1 pg/mL was significantly associated with CI(area under the curve=0.64;P=0.003).An increase in GDF-15 per 1000 units was associated with an increased risk of the primary endpoint(hazard ratio=1.42;95%confidence interval:1.17-1.73;P<0.001).CONCLUSIONS In patients with HF with CI,cognitive function,depression,and quality of life measures improved at the 3-month and 1-year ***-15 predicted CI with moderate discrimination capacity and was associated with worse HF out-comes.

Usefulness of fecal lactoferrin in predicting and monitoring the clinical severity of infectious diarrhea

AIM:To explore the value of fecal lactoferrin in predicting and monitoring the clinical severity of infectious ***:Patients with acute infectious diarrhea ranging from 3 mo to 10 years in age were enrolled,and one to three stool samples from each subject were *** parameters,including white blood cells /differential count,C-reactive protein,fecal mucus,fecal pus cells,duration of fever,vomiting,diarrhea and severity(indicated by Clark and Vesikari scores),were recorded and *** lactoferrin was determined by enzyme-linked immunosorbent assay and compared in different pathogen and disease *** estimating equations(GEE) were also used for ***:Data included 226 evaluations for 117 individuals across three different time *** lactoferrin was higher in patients with Salmonella(11.17 μg/g ± 2.73 μg/g) or Campylobacter(10.32 μg/g ± 2.94 μg/g) infections and lower in patients with rotavirus(2.82 μg/g ± 1.27 μg/g) or norovirus(3.16 μg/g ± 1.18 μg/g) *** of fecal lactoferrin were significantly elevated in patients with severe(11.32 μg/g ± 3.29 μg/g) or moderate(3.77 μg/g ± 2.08 μg/g) disease activity compared with subjects with mild(1.51 μg/g ± 1.36 μg/g) disease activity(P < 0.05).GEE analysis suggests that this marker could be used to monitor the severity and course of gastrointestinal infections and may provide information for disease ***:Fecal lactoferrin increased during bacterial infection and with greater disease severity and may be a good marker for predicting and monitoring intestinal inflammation in children with infectious diarrhea.

Potential prognostic,diagnostic and therapeutic markers for human gastric cancer

The high incidence of gastric cancer (GC) and its consequent mortality rate severely threaten human health. GC is frequently not diagnosed until a relatively advanced stage. Surgery is the only potentially curative treatment. Thus, early screening and diagnosis are critical for improving prognoses in patients with GC. Gastroscopy with biopsy is an appropriate method capable of aiding the diagnosis of specific early GC tumor types; however, the stress caused by this method together with it being excessively expensive makes it difficult to use it as a routine method for screening for GC on a population basis. The currently used tumor marker assays for detecting GC are simple and rapid, but their use is limited by their low sensitivity and specificity. In recent years, several markers have been identiﬁed and tested for their clinical relevance in the management of GC. Here, we review the serum-based tumor markers for GC and their clinical significance, focusing on discoveries from microarray/proteomics research. We also review tissue-based GC tumor markers and their clinical application, focusing on discoveries from immunohistochemical research. This review provides a brief description of various tumor markers for the purposes of diagnosis, prognosis and therapeutics, and we include markers already in clinical practice and various forthcoming biomarkers.

Viral hepatitis remains a worldwide public health *** hepatitis D virus(HDV)must either coinfect or superinfect with the hepatitis B virus(HBV).HDV contains a small RNA genome(approximately 1.7 kb)with a single open reading frame(ORF)and requires HBV supplying surface antigens(HBsAgs)to assemble a new HDV *** HDV replication,two isoforms of a delta antigen,a small delta antigen(SDAg)and a large delta antigen(LDAg),are produced from the same ORF of the HDV *** SDAg is required for HDV replication,whereas the interaction of LDAg with HBsAgs is crucial for packaging of HDV *** clinical outcomes of HBV/HDV dual infection have been reported,but the molecular interaction between HBV and HDV is poorly understood,especially regarding howHBV and HDV compete with HBsAgs for assembling *** this paper,we review the role of endoplasmic reticulum stress induced by HBsAgs and the molecular pathway involved in their promotion of LDAg nuclear *** the nuclear sublocalization and export of LDAg is regulated by posttranslational modifications(PTMs),including acetylation,phosphorylation,and isoprenylation,we also summarize the relationship among HBsAg-induced endoplasmic reticulum stress signaling,LDAg PTMs,and nuclear export mechanisms in this review.

Hepatocellular carcinoma occurred in a Hepatitis B carrier clinic cohort during a mean follow up of 10 years

Aim:Chronic persistent hepatitis B virus carriers are generally asymptomatic until the advanced stage of the ***"Hepatitis B-Carrier Clinics"of chang gung memorial hospital has been using alpha-fetoprotein(AFP)and liver ultrasound for early detection of hepatocellular carcinoma(HCC)in hepatitis B surface antigen(HBsAg)carriers since ***:We evaluated the results of surveillance between 1980 and 2012 by collecting clinic data,matched cancer registry status,and national mortality database ***:Of 15,235 HBsAg carriers,238 instances of HCC(1.5%or 156.2/100,000 person-years)were detected over a mean follow-up period of 10.0±7.6 *** were more men(89.1%)and patients with liver cirrhosis(70.2%)in the HCC group(P<0.001),and both the initial and maximal alanine aminotransferase(ALT)levels were higher in this group(P<0.001).One hundred and thirty cases of HCC(54.6%)were identified during regular follow-up sessions,55(23.1%)were detected after the regular schedule had lapsed("out-of-schedule"),and 53(22.3%)were lost to follow-up *** mean tumor size was smaller in the regular group than in the out-of-schedule group(2.72 cm vs.4.59 cm,P<0.001),and the survival rate was higher(43.8%vs.30.9%,P<0.001).Conclusion:The incidence of HCC was relatively low in the HBsAg-Carrier Clinics *** for early diagnosis of HCC improved the survival of high-risk HBsAg *** ensure cost-effectiveness,we suggest using different screening strategies according to the individual risk of hepatocarcinogenesis.

临床证据显示高果糖摄取与代谢综合征现象产生有高度相关性,然而关于母体在怀孕与哺乳期高果糖摄取对子代代谢综合征的发育程序化影响的研究却相对缺乏。本研究利用第二代测序(next-generation sequencing)技术分析母鼠高果糖摄取对新生第一天、离乳后一天(出生后三周)、以及成年期(出生后三个月)子代大鼠脑部、心脏、肾脏与膀胱四个器官转录组的影响。结果显示:(1)母鼠在怀孕与哺乳期高果糖摄取,会程序化胎鼠使其于成年期产生代谢综合征;(2)同时,在仔鼠脑部、心脏、肾脏与膀胱四个器官引起不同程度的转录组变化;(3)在新生第一天子代大鼠,有两个基因,分别是Errfi1与Ctgf,同时在四个器官中表现受到影响,此现象在子代离乳与成年期则消失;(4)参与果糖代谢、糖酵解及糖异生、脂肪代谢、以及胰岛素信号传导通路的基因转录也受到不同程度影响,此现象在新生第一天尤为显著。该结果提示,可利用第二代测序技术寻找不同年龄层子代器官特异性受母体高果糖摄取影响的基因转录变化,进而研发代谢综合征发育程序化在不同器官引起病变的可能机制与治疗策略。

Manganese antagonizes iron blocking mitochondrial aconitase expression in human prostate carcinoma cells
