Fatty liver in hepatitis C patients post-sustained virological response with direct-acting antivirals

AIM To determine steatosis and fibrosis prevalence in hepatitis C patients after a sustained virological response achieved with direct-acting *** Transient elastography with controlled attenuation parameter(CAP) was used to assess hepatic steatosis post-sustained virological response(SVR);the CAP technology was not available in the United States at study *** stiffness/fibrosis was measured before and 47 wk after treatment *** with genotype 3 and patients with cirrhosis were *** One hundred and one patients were included in the ***-SVR there were decreases from baseline in alanine aminotransferase(ALT)(63.1 to 17.8 U/L),aspartate aminotransferase(51.8 to 21.5 U/L) and fibrosis score(7.4 to 6.1 k Pa)(P < 0.05).Post-SVR,48 patients(47.5%) had steatosis on CAP;of these,6.25% had advanced *** with steatosis had higher body mass index(29.0 vs 26.1 kg/m2),glucose(107.8 vs 96.6 mg/d L),ALT(20.4 vs 15.3 mg/d L),CAP score(296.3 vs 212.4 d B/m) and fibrosis score(7.0 vs 5.3 k Pa);P < ***,compared to baseline,both patients with and without steatosis had change in fibrosis score post-SVR(7.7 k Pa vs 7.0 k Pa and 7.0 k Pa vs 5.3 k Pa);alternatively,(P < 0.05) and therefore patients with steatosis continued to have clinically significant stiffness(≥ 7 k Pa).CONCLUSION Fatty liver is very common in hepatitis C virus(HCV) patients *** patients continue to have elevated mean fibrosis score(≥ 7 k Pa) compared to those without fatty liver;some have advanced *** term follow up is needed to assess steatosis and fibrosis in HCV patients post-SVR.

Nonalcoholic fatty liver disease(NAFLD)is the accumulation of fat in the liver in the absence of secondary *** is a multifactorial disease that results from the interaction of genetic predisposition and metabolic,inflammatory and environmental *** these factors,dysregulation of gut microbiome has been linked to the development of fatty liver *** microbiome composition can be modified by dietary habits leading to gut microbiome dysbiosis,especially when a diet is rich in saturated fats,animal products and fructose *** species of bacteria in the gut metabolize nutrients differently,triggering different pathways that contribute to the accumulation of fat within the liver and triggering inflammatory cascades that promote liver *** this review,we summarize the current understanding of the roles of gut microbiota in mediating NAFLD development and discuss possible gut microbiota-targeted therapies for *** summarize experimental and clinical evidence,and draw conclusions on the therapeutic potential of manipulating gut microbiota to decrease the incidence and prevalence of fatty liver disease.

Occult hepatitis B infection is characterized by loss of hepa-titis B surface antigen(HBsAg)and persistence of low levels of hepatitis B virus(HBV)replication that may or may not be detectable in plasma/*** present a case of HBV reactivation in a male patient who underwent orthotopic liver transplant for hepatocellular carcinoma secondary to active hepatitis C(HCV)***-transplant,he was HBsAg-negative and hepatitis B core antibody-positive,with an undetectable HBV viral load that was incidentally found to be positive at a very low HBV viral load on the day of ***-transplant,his HBsAg remained undetect-able,with an undetectable HBV viral load,until eradication of his HCV infection with direct acting antiviral ***-ter eradication of HCV,there was reactivation of HBV,with a high viral load and emergence of serum HBsAg.A deep sequencing genetic analysis of his HBV both pre-and post-transplant revealed the presence of a mutation in the"a"determinant of the HBV surface *** role of HBV genotype'a'determinant mutation in HBV reactivation post-transplant is unknown and needs further *** experience suggests a possible role for antiviral prophylaxis in these patients or monitoring of HBV viral loads post-transplant.

背景与目的肝移植术后原发性胆源性胆管炎(PBC)的复发很常见并且会损害移植物以及影响患者的存活。熊去氧胆酸(UDCA)是目前治疗PBC的标准药物。本研究旨在探究肝移植术后预防性使用UDCA对PBC的复发率和长期预后的影响。方法本研究为回顾性队列研究,研究对象包括1983年至2017年间在16个中心和9个国家接受肝移植术的780例PBC患者,中位随访时间为11年。其中,有190位患者术后预防性使用UDCA (10~15mg/kg/d)治疗。主要结局为经组织学证实的PBC复发,次要结局为移植物丢失、肝脏相关死亡和各种原因死亡。

有色金属工业减碳是工业过程减碳的重要抓手。本工作综述了有色金属工业碳排放现状和特点,在此基础上提出了有色金属工业尤其是重点冶炼行业的低碳技术路径。分析表明,铝冶炼行业是有色金属工业二氧化碳减排的核心,预计2025年有色金属工业将实现碳达峰,碳达峰时二氧化碳排放量为7.5亿吨;有色金属工业减碳技术路径主要包括清洁能源替代、发展先进的低碳技术与装备、金属再生利用及碳捕集利用(CCU)等,其中金属再生利用是有色金属工业支撑实现国家“双碳”目标的重要路径。

Type 2 diabetes across populations of different ancestry: deconstructing the genomic landscape

In a recent study published in Nature Medicine,Smith and colleagues used data from 1.4 million individuals across 37 genome-wide association studies(GWAS)reflecting different genetic ancestral populations to identify 12 genetic clusters,mostly shared among the different populations,and associated the weighted sums of genetic variants of each cluster—termed as partitioned polygenic scores(pPSs)—with various lab-based,anthropometric and cardiometabolic *** analyses extended previous studies on biological mechanisms linking genetics to type 2 diabetes(T2D)risk and pointed to both similarities of genetic clusters across multiple populations and differences regarding their contributions to overall T2D risk among different ethnicities.
