BACKGrOUND A collecting duct carcinoma is a very rare, malignant renal epithelial *** metastases are present in one third of cases at the time of diagnosis. It is known to have a poor *** SUMMArY A 42-year-old male was sent to our surgery clinic for removal of a 119.2 mm ×108.3 mm encapsulated cystic mass, which was localized in the 8th segment of the right liver lobe. The lesion was first identified on ultrasonography. A computed tomography scan confirmed the presence of a Bosniak type Ⅲ cystic lesion,which affected the liver and convexity of the right kidney. Surgical intervention involved a right nephrectomy, with removal of the cystic mass. The patient was mobilized on the first postoperative day and was discharged after 7 d. The histological and immunohistochemical examination revealed a low-grade collecting duct renal carcinoma, which is a rare variant of papillary carcinoma,with low malignant potential. The patient did not receive chemotherapy and after 21 mo of follow-up, a radiological examination and laboratory analyses showed normal aspects. AIM： To study molecular and morphological changes in lens epithelial cells following femtosecond laser-assisted and manually performed continuous curvilinear capsulotomy（CCC） in order to get information about these methods regarding their potential role in the induction of development of secondary cataract. METHODS： Anterior lens capsules（ALC） were removed from 40 patients with age-related cataract by manual CCC and by femtosecond laser-assisted capsulotomy（FLAC）. Samples removed by manual CCC were assorted in group 1, FLAC samples were classified in group 2. Morphology of lens epithelial cells was examined with light and electron microscopes. Following capsulotomy, expressions of p53, Bcl-2 and cyclin D1 genes were analyzed with reverse transcriptase polymerase chain reaction. Immunohistochemistry was used to detect the pro-apoptotic p53 in the epithelial cells. rESULTS： Light and electron microscopic examination showed that ALC of group 1 contained more degenerating cells following manual CCC than after FLAC. The expression level of p53 was higher after manual than laser-assisted surgery. Immunocytochemistry indicated significantly higher number of cells containing p53 protein in the manual CCC group than following FLAC. Bcl-2 and cyclin D1 gene expression levels were slightly lower following manual CCC than after FLAC, but the difference was not significant. CONCLUSION： Manually removed ALC shows slightly, but not significantly larger damage due to the mechanical stretching and pulling of the capsule than those removed using FLAC.

The discovery that contracting skeletal muscle generates reactive oxygen species(rOS)was first reported over 40 years *** prevailing view in the 1980s was that exercise-induced rOS production promotes oxidation of proteins and lipids resulting in muscle ***,a paradigm shift occurred in the 1990s as growing research revealed that rOS are signaling molecules,capable of activating transcriptional activators/coactivators and promoting exercise-induced muscle *** evidence supports the notion that reduction-oxidation(redox)signaling pathways play an important role in the muscle remodeling that occurs in response to endurance exercise *** review examines the specific role that redox signaling plays in this endurance exercise-induced skeletal muscle *** begin with a discussion of the primary sites of rOS production in contracting muscle fibers followed by a summary of the antioxidant enzymes involved in the regulation of rOS levels in the *** then discuss which redox-sensitive signaling pathways promote endurance exercise-induced muscle adaptation and debate the strength of the evidence supporting the notion that redox signaling plays an essential role in muscle adaptation to endurance exercise *** hopes of stimulating future research,we highlight several important unanswered questions in this field.

Intraoperative complication rates in cataract surgery performed by resident trainees and staff surgeons in a tertiary eyecare center in Hungary

AIM: To compare the incidence of intraoperative complications during primary phacoemulsification(phaco) surgery between resident surgeons(residents) and staff surgeons(specialists) and to objectively determine the difficulty of stages in phaco surgery. METHODS: This retrospective study included cases of phaco cataract surgery performed between January and December 2019. There were no exclusion criteria. For each patient, demographics, clinical history, case complexity, type of surgeon, and operative details were reviewed. Primary outcomes included intraoperative complication rates and the objective measure of difficulty in the steps of the surgery performed by residents and ***: A total of 3272 cases were included;7.4%(n=241) of cases were performed by residents. The overall complication rate was 5.4%(n=177). The intraoperative complication rate was significantly higher(Presidents(n=33, 13.7%) than in specialists(n=144, 4.8%). The most frequent complications were posterior capsule tear(n=85, 2.6%), anterior capsule tear(n=50, 1.53%), zonular fiber loss(n=45, 1.38%), and dropped nucleus(n=15, 0.46%). Objectively, the most difficult steps during surgery were phaco in 66(60.0%), capsulorhexis in 21(19.1%), irrigation/aspiration in 13(11.8%), hydrodissection in 9(8.2%), and intraocular lens(IOL) implantation in 1(0.9%) case. CONCLUSION: Intraoperative complication rates are higher in residents than in specialists. The order of objective difficulty in phaco surgery steps is in line with the subjective findings of other surveys, revealing that the most challenging parts of phaco surgery are phaco and capsulorhexis.

According to normative-resource theory,the balance of marital power is influenced by the effect of socioeconomic resources and subcultural *** purpose of our study is to empirically test the effects of Japanese married individuals’comparative socioeconomic variables—education,age,occupation status,personal income,family income,urban/rural living area,years married,and the presence/number of children;and normative resources—the four dimensions of sex role orientation—on decision-making authority across the stages of automobile *** data of 500 demographically heterogeneous married individuals are analyzed by using a Lasso(least absolute shrinkage and selection operator)regression to select variables and to improve the accuracy of prediction and *** findings reveal that wives’authority is significantly influenced across all stages by their personal income and specific dimensions of their sex role *** the other hand,husbands’authority is not affected by their perception of role norms,but only by their *** to expectations,the effects of education,occupation,and employment status are not significant in any of the cases,opening a discussion for cross-cultural *** findings are interpreted and transformed into actionable items for managerial implications.

reactive oxygen species （rOS） are continuously generated during aerobic metabolism and at moderate level. They play a role in redox signaling, but in significant concentration they cause oxidative damage and neurodegeneration. Because of the enhanced sensitivity of brain to rOS, it is especially important to maintain the normal redox state in different types of neuron cells. In last decade it became clear that regular exercise beneficially affects brain function, and can play an important preventive and therapeutic role in stroke, Alzheimer, and Parkinson diseases. The effects of exercise appear to be very complex and could include neurogenesis via neurotrophic factors, increased capillariszation, decreased oxidative damage, and increased proteolyfic degradation by proteasome and neprilysin. Data from our and other laboratories indicate that exercise-induced modulation of rOS levels plays a role in the protein content and expression of brain-derived neurotrophic factor, tyrosinerelated kinase B （TrkB）, and cAMP response element binding protein, resulting in better function and increased neurogenesis. Therefore, it appears that exercise-induced modulation of the redox state is an important means, by which exercise benefits brain function, increases the resistance against oxidative stress, facilitates recovery from oxidative stress, and attenuates age-associated decline in cognition.
