Bispectral index monitoring as an adjunct to nurse-administered combined sedation during endoscopic retrograde cholangiopancreatography

AIM:To determine whether bispectral index (BIS) monitoring is useful for propofol administration for deep sedation during endoscopic retrograde cholangiopancreatography (ERCP). METHODS:Fifty-nine consecutive patients with a variety of reasons for ERCP who underwent the procedure at least twice between 1 July 2010 and 30 November 2010. This was a randomized cross-over study, in which each patient underwent ERCP twice, once with BIS monitoring and once with control monitoring. Whether BIS monitoring was done during the first or second ERCP procedure was random. Patients were intermittently administered a mixed regimen including midazolam, pethidine, and propofol by trained nurses. The nurse used a routine practice to monitor sedation using the Modified Observer's Assessment of Alertness/Sedation (MOAA/S) scale or the BIS monitoring. The total amount of midazolam and propofol used and serious side effects were compared between the BIS and control groups. RESULTS:The mean total propofol dose administered was 53.1 ± 32.2 mg in the BIS group and 54.9 ± 30.8 mg in the control group (P = 0.673). The individual propofol dose received per minute during the ERCP procedure was 2.90 ± 1.83 mg/min in the BIS group and 3.44 ± 2.04 mg in the control group (P = 0.103). The median value of the MOAA/S score during the maintenance phase of sedation was comparable for the two groups. The mean BIS values throughout the procedure (from insertion to removal of the endoscope) were 76.5 ± 8.7 for all 59 patients in using the BIS monitor. No significant differences in the frequency of < 80% oxygen saturation, hypotension (< 80 mmHg), or bradycardia (< 50 beats/min) were observed between the two study groups. Four cases of poor cooperation occurred, in which the procedure should be stopped to add the propofol dose. Utilizing 3,4-ethylenedioxythiophene(EDOT)-bridged non-fullerene acceptors for efficient organic solar cells

A rational design of efficient low-band-gap non-fullerene acceptors(NFAs)for high-performance organic solar cells(OSCs)remains challenging;the main constraint being the decrease in the energy level of the lowest unoccupied molecular orbitals(LUMOs)as the bandgap of A-D-A-type NFAs ***,the short current density(J_(sc))and open-circuit voltage(V_(oc))result in a trade-off relationship,making it difficult to obtain efficient ***,three NFAs(IFL-ED-4 F,IDT-ED-4 F,and IDTT-ED-2 F)were synthesized to address the above-mentioned issue by introducing 3,4-ethylenedioxythiophene(EDOT)as aπ-*** NFAs exhibit relatively low bandgaps(1.67,1.42,and 1.49 eV,respectively)and upshifted LUMO levels(-3.88,-3.84,and-3.81 eV,respectively)compared with most reported low-band-gap ***,the photovoltaic devices based on IDT-ED-4 F blended with a PBDB-T donor polymer showed the best power conversion efficiency(PCE)of 10.4%with a high J_(sc) of 22.1 mA cm^(-2) and Voc of 0.884 V among the examined *** contrast,IDTT-ED-4 F,which was designed with an asymmetric structure of the D-p-A type,showed the lowest efficiency of 1.5%owing to the poor morphology and charge transport properties of the binary ***,when this was introduced as the third component of the PM6:BTP-BO-4 Cl,complementary absorption and cascade energy-level alignment between the two substances could be ***,the IDTT-ED-4 F-based ternary blend device not only improved the Jscand Voc,but also achieved a PCE of 15.2%,which is approximately 5.3%higher than that of the reference device with a minimized energy loss of 0.488 *** addition,the universality of IDTT-ED-2 F as a third component was effectively demonstrated in other photoactive systems,specifically,PM6:BTPe C9 and PTB7-Th:IEICO-4 *** work facilitates a better understanding of the structure–property relationship for utilizing efficient EDOT-bridged NFAs in high-performance OSC applications.

Weekend and nighttime effect on the prognosis of peptic ulcer bleeding

AIM:To evaluate whether weekend or nighttime admission affects prognosis of peptic ulcer bleeding despite early ***:Retrospective data collection from four referral centers,all of which had a formal out-of-hours emergency endoscopy service,even at weekends.A total of 388 patients with bleeding peptic ulcers who were admitted via the emergency room between January 2007 and December 2009 were *** parameters included time from patients' arrival until endoscopy,mortality,rebleeding,need for surgery and length of hospital ***:The weekday and weekend admission groups comprised 326 and 62 patients,*** were no significant differences in baseline characteristics between the two groups,except for younger age in the weekend *** patients (97%) had undergone early endoscopy,which resulted in a low mortality rate regardless of point of presentation (1.8% overall vs 1.6% on the weekend).The only outcome that was worse in the weekend group was a higher rate of rebleeding (12% vs 21%,P = 0.030).However,multivariate analysis revealed nighttime admission and a high Rockall score (≥ 6) as significant independent risk factors for rebleeding,rather than weekend ***:Early endoscopy for peptic ulcer bleeding can prevent the weekend effect,and nighttime admission was identified as a novel risk factor for rebleeding,namely the nighttime effect.

Silica thin films synthesized sol–gel process are proposed as flexible encapsulation materials.A sol–gel process provides a dense and stable amorphous silica structure,yielding an extremely high elastic deformation limit of 4.9%and extremely low water vapor transmission rate(WVTR)of 2.90×10^(−4)g/(m^(2)∙day)at 60℃and relative humidity of 85%.The WVTR is not degraded by cyclic bending deformations for the bending radius corresponding to a tensile strain of 3.3%in the silica encapsulation film,implying that the silica thin film is robust against the formation of pinhole-type defects by cyclic bending *** organic solar cells encapsulated with the silica films operate without degradation in power conversion efficiency for 50,000 bending cycles for a bending radius of 6 mm.
