Caregiver burden is increased in Parkinson’s disease with mild cognitive impairment (PD-MCI)

Background:There is limited evidence on caregiver outcomes associated with mild cognitive impairment in patients with Parkinson’s disease(PD-MCI)and the coping strategies used by these ***:To investigate this relationship,we examined levels of burden,depression,anxiety,coping strategies and positive aspects of caregiving in the informal caregivers of 96 PD *** PD patients were classified using MDS-Task Force Level II criteria as showing either normal cognition(PD-N;n=51),PD-MCI(n=30)or with dementia(PDD;n=15).Results:Mean Zarit Burden Interview(ZBI)score increased significantly between carers of PD-N(M=13.39,SD=12.22)compared to those of PD-MCI patients(M=22.00,SD=10.8),and between carers of PD-MCI and PDD patients(M=29.33,SD=9.59).Moreover,the proportion of carers showing clinically significant levels of burden(ZBI score≥21)also increased as the patients’cognitive status declined(18% for PD-N;60% for PD-MCI;and 80%for PDD)and was mirrored by an increasing amount of time spent providing care by the *** ZBI score was independent of patient neuropsychiatric symptoms,motor function,disease duration and time that caregivers spent *** use of different coping strategies increased with worsening ***,we found only equivocal evidence that the use of problem-focused,emotion-focused and dysfunctional coping mediated the association between patient cognitive status and caregiver burden,because the inverse models that used caregiver burden as the mediator were also ***:The study highlights the impact of Parkinson’s disease on those providing care when the patient’s cognition is poor,including those with *** well-being has important implications for caregiver support,nursing home placement and disease course.

Mass-balance ecosystem model of the East China Sea

Using the Ecopath mass-balance trophodynamic model,this paper analyzed the trophic levels,flows,food web structure and ecosystem maturity of the East China Sea,and identified ecologically important functional groups in the *** model is based on fishery resource surveys of the East China Sea in 2000,studies on diet composition and global databases such as FishBase and the Sea Around Us Project *** results showed that trophic levels of the functional groups are between 2.86 and 4.37,with an average of ***(Engraulis japonicus),small fishes and benthic crustaceans such as shrimps and crabs are important groups in terms of the trophic structure and flow dynamics in the East China *** flows of most groups are between specific trophic levels,except file fish(Thamnaconus spp.),pomfret(Pampus spp.) and *** transfer efficiency of levels Ⅱ,Ⅲ,IV and more than V are 11.8%,21.1%,17.4% and 22.1-22.5%,*** of fishery-the largest ‘consumer’ of the ecosystemare much stronger than those exerted by biological groups in the *** model suggests that the current fishery can further reduce the complexity of the *** of the system indices suggest that maturity of the ecosystem is *** conclusion of this model indicates that it was the overfishing that caused the ecosystem of the East China Sea declined,which should be taken into account as a critical reference for fisheries management in the future.

Experimental investigation of a cook-off temperature in a hot barrel

Acupuncture for irritable bowel syndrome: A blinded placebo-controlled trial

AIM:Irritable bowel syndrome (IBS) is a common disorder and many patients fail to find adequate relief from conventional therapies for their symptoms. This study tests the claim that acupuncture is effective for a majority of these patients. METHODS: A prospective, blinded, sham acupuncture-controlled trial of traditional Chinese acupuncture was performed at a single postgraduate teaching hospital in Europe. Sixty patients with well-established IBS were recruited. The blinded comparator was sham acupuncture administered by the second of two acupuncturists who alone was aware of the randomization, and who otherwise followed the prescription of the first. The primary end-point was a defined fall in the symptom score at 13 wk (by intention to treat). The prior expectation was a 30% placebo response, and a response rate of 70% from acupuncture, for which the study was adequately powered. RESULTS: Patients in treated and sham groups improved significantly during the study-mean improvement in scores being equal (minus 1.9) and significant for both (P<0.05; one-tailed t test). There was a small numeric but nonsignificant difference between the response rate in patients receiving acupuncture (40.7%) and sham treatment (31.2%). Several secondary end-points marginally favored active treatment, but an improved symptom score of any degree of magnitude occurred more often with sham therapy (65.6% vs 59.2%). For no criterion was statistical significance approached. CONCLUSION: Traditional Chinese acupuncture is relatively ineffective in IBS in the European hospital setting, and the magnitude of any effect appears insufficient to warrant investment in acupuncture services.

End Organ Recovery and Survival with the QuadroxD Oxygenator in Adults on Extracorporeal Membran Oxygenation

Introduction: Extracorporeal Membrane Oxygenation (ECMO) is used in selected patient with cardiogenic and/or re- spiratory shock. We report our experience with standardized management protocols and the application of the Qua- droxD oxygenator with a centrifugal pump to maximize end-organ recovery and improve survival. Methods: This is an Internal Review Board (IRB) approved, single institution retrospective study of end-organ recovery and survival in pa- tients who required ECMO for cardiogenic and/or respiratory shock between July 2010 and June 2011. Results: Sixteen patients (median age: 46 years) were initiated on either Veno-Arterial (VA) or Veno-Venous (VV) ECMO. Cardiogenic shock, acute respiratory distress syndrome (ARDS) and a combined respiratory and cardiogenic compromise were the primary indications for ECMO in 8 (50%), 5 (31%) and 3 (19%) patients respectively. The median time on ECMO was 8 days (range: 4 - 26 days). Twelve patients (75%) were successfully weaned off ECMO, of which four (25%) were bridged to a ventricular assist device (VAD) and eight (50%) were weaned to recovery. All eight patients (100%) that were weaned to recovery and two patients (50%) that were bridged to a VAD were successfully discharged from the hospital, resulting in a discharge rate of 63%. There was an improvement in pre- vs. post-ECMO AST (449 IU/L vs. 63 IU/L, p Conclusion: ECMO using the QuadroxD oxygenator and a centrifugal pump, coupled with standardized management protocols is beneficial in carefully selected patients. Improvement or main- tenance of end-organ function is associated with successful bridge to device therapy and/or increased survival.
