Natural killer cells activity in a metastatic colorectal cancer patient with complete and long lasting response to therapy

Here we report a case of a 70-year-old man who received adjuvant chemotherapy with fluorouracile, folinic acid and oxaliplatin after a left hemicolectomy for a stage Ⅲb adenocarcinoma in May 2009. During followup he de-veloped abdominal lymphnodes metastases evidenced by positron emission tomography-computed tomography(PET-CT) scan and increase of carcinoembryonic antigen(CEA) level. Chemotherapy with capecitabine, oxaliplatin and bevacizumab was started in April 2012. Restaging showed a complete response and normalization of CEA. The patient received maintenance therapy with bevacizumab which was stopped in December 2013 for patient choice. In October 2014, a new increase in CEA was documented and PET-CT scan showed lung metastases. Analysis of RAS status revealed the absence of mutations, then the patient started a second-line chemotherapy with fluorouracile, folinic acid, irinotecan(folfiri) and panitumumab achieving, in January 2015, a complete response and normalization of CEA. Thereafter, folfiri was discontinued for toxicity; furthermore, upon the third occurrence of a grade 3 dermatologic toxicity, panitumumab was continued from June 2015 at 60% of the original dose and it was administered every three weeks. Until presentation of this case, the patient maintains a complete response, has no symptoms of disease and CEA is normal. Interestingly, this patient presented a high proportion of circulating natural killer(NK) cells(35.1%) with high cytotoxic activity against tumor cells. Study on the role of NK in patients with advanced colorectal cancer are ongoing.

AIM:To investigate the usefulness of mild iron depletion and the factors predictive for histological improvement following phlebotomy in Caucasians with chronic hepatitis C(CHC). METHODS:We investigated 28 CHC Caucasians with persistently elevated serum aminotransferase levels and non responders to,or unsuitable for,antiviral therapy who underwent mild iron depletion(ferritin≤70 ng/mL) by long-term *** improvement,as defined by at least one point reduction in the staging score or,in case of unchanged stage,as at least two points reduction in the grading score(Knodell),was evaluated in two subsequent liver biopsies(before and at the end of phlebotomy,48±16 mo apart).RESULTS:Phlebotomy showed an excellent safety *** improvement occurred in 12/28 phlebotomized *** males responded to *** univariate logistic analysis alcohol intake(P=0.034),high histological grading(P=0.01) and high hepatic iron concentration(HIC)(P=0.04) before treatment were associated with histological *** logistic analysis showed that in males high HIC was the only predictor of histological improvement following phlebotomy(OR=1.41, 95%CI:1.03-1.94,P=0.031).Accordingly,12 out of 17(70%)patients with HIC≥20μmol/g showed histological improvements at the second biopsy. CONCLUSION:Male CHC Caucasian non-responders to antiviral therapy with low-grade iron overload can benefit from mild iron depletion by long-term phlebotomy.

Conventional colonoscopy and magnified chromoendoscopy for the endoscopic histological prediction of diminutive colorectal polyps: A single operator study

AIM： To accurately differentiate the adenomatous from the non-adenomatous polyps by colonoscopy. METHODS： All lesions detected by colonoscopy were first diagnosed using the conventional view followed by chromoendoscopy with magnification. The diagnosis at each step was recorded consecutively. All polyps were completely removed endoscopically for histological evaluation. The accuracy rate of each type of endoscopic diagnosis was evaluated, using histological findings as gold standard. RESULTS： A total of 240 lesions were identified, of which 158 （65.8%） were non-neoplastic and 82 （34.2%） were adenomatous. The overall diagnostic accuracy of conventional view, and chromoendoscopy with magnification was 76.3% （183/240） and 95.4% （229/240）, respectively （P〈 0.001） CONCLUSION： The combination of colonoscopy and magnified chromoendoscopy is the most reliable nonbiopsy method for distinguishing the non-neoplastic from the neoplastic lesions.

AIM： To assess the late outcome of teen-agers with a previous history of recurrent abdominal pain （RAP） or irritable bowel syndrome （IBS）. METHODS： A group of 67 children with RAP referred to the department from January 1986 to December 1995 was followed up between 5 and 13 years after the initial diagnosis by means of a structured telephone interview. We hypothesized that those patients with persistent adult IBS-like symptoms would be significantly more likely to report a family history oflBS in comparison with adults with no persistent abdominal complaint. RESULTS： Out of the 52 trackable subjects, 15 were found to present IBS-like symptoms at follow-up （29%） whereas the majority （37 subjects） did not. Subjects with IBS-like symptoms were almost three times more likely to present at least one sibling with similar symptoms compared to subjects not complaining （40.0% vs 16.0%）, respectively （P 〈 0.05 at Student t test）. Subjects with IBS-like symptoms also reported a higher prevalence of extra-intestinal symptoms, such as back pain, fibromyalgia, headache, fatigue and sleep disturbances. CONCLUSION： The study confirms previous observations indicating that pediatric RAP can predict later development of IBS. The latter appears to be greatly influenced by intrafamilial aggregation of symptoms, possibly through the learning of a specific illness behavior.

Phosphorus(P)deficiency is one of the main constraints on crop production in Arenosols(acid sandy soil).The high cost of P fertilizers may represent an insurmountable obstacle in many poor countries,leaving the exploitation of their own calcareous and phosphate rocks as the only low-cost and long-term *** is suggested to have positive effects on soil properties;however,there is no published research on the synergistic effects of biochar and rocky materials in modifying soil *** aim of this study was to investigate the chemical and biochemical responses of an acid Arenosol treated with phosphate rock(PR),calcareous rock(CR),and biochar(BC),and the implications for corn yield.A soil from Marracuene District,Mozambique was used,where corn was grown for 90 d with the soil treated with:no addition(control),water-soluble zinc phosphite fertilizer(WSP),PR,WSP+CR,WSP+BC,WSP+CR+BC,PR+BC,and PR+CR+*** was produced by pyrolysis of babycorn peels for 4 h at 450?C and applied at 11 g *** soil pHH2 Oincreased from about 4.54 in the control to 7.38 in the PR+CR+BC *** oxidizable organic carbon,cation exchange capacity,and available P were higher in the treatments containing BC than in the *** treatments containing CR and/or BC led to the highest activities of alkaline phosphomonoesterase,phosphodiesterase,andα-glucosidase,which increased P availability and gave the greatest biomass and *** suggest that biochar provides additional soluble P and supplies adsorption sites for phosphate,preventing its evolution to unavailable ***,PR applied together with BC contributed to an 840%yield increase compared to the *** treatments containing WSP and BC facilitated phosphite oxidation to phosphate and increased crop yield.

Gastric cancer is one of the leading causes of cancerrelated deaths worldwide, although the incidence has gradually decreased in many Western countries. Two main gastric cancer histotypes, intestinal and diffuse, are recognised. Although most of the described genetic alterations have been observed in both types, different genetic pathways have been hypothesized. Genetic and epigenetic events, including 1q loss of heterozygosity (LOH), microsatellite instability and hypermethylation, have mostly been reported in intestinal-type gastric carcinoma and its precursor lesions, whereas 17p LOH, mutation or loss of E-cadherin are more often implicated in the development of diffuse-type gastric *** this review, we summarize the sometimes contradictory findings regarding those markers which influence the progression of gastric adenocarcinoma.

Polygynous males can change their mating tactics across their lifetime, but information is scarce on the flexibility of this trait within a given season and the relative costs and benefits of using different tactics. Here, we monitored individually marked male reindeer Rangifer tarandus and classified their mating tactics as harem-defense, sneaking, or mixed. The costs of the male reproductive effort were assessed using both direct （i.e. percentage of body mass lost） and indirect measures （i.e. activity patterns such as feeding, standing, and walking）, while mating group size and reproductive success were recorded as mating ef- fort benefits. Our results show that reindeer males may switch between the harem-defense and sneaking tactics throughout the same breeding season, providing further support to the notion that reproductive tactics are flexible in ungulates. The costs and benefits of male mating effort vary according to the mating tactic, reaching the highest values in harem-holders and the lowest values in sneaking males. Moreover, males who switched between the sneaking tactic and the harem-defence tactic tended to achieve higher mating success than males who consistently used the least costly tactic. Indeed, all harem-holders successfully sired offspring, whereas only two out of three mixed-tactic males sired one calf, and sneaking males did not sire any calves. In conclusion, our results show that reindeer males can modulate their mating efforts during the same breeding season by switching between the most costly harem-defense tactic and the least costly sneaking tactic, suggesting individual solutions to the balance between reproductive effort and mating opportunities [Current Zoology 61 （5）： 802-810, 2015].
