Stenting versus non-stenting treatment of intermediate stenosis culprit lesion in acute ST-segment elevation myocardial infarction： a multicenter random- ized clinical trial

Background The benefit/risk ratio of stenting in acute ST-segment elevation myocardial infarction （STEMI） patients with single vessel intermediate stenosis culprit lesions merits further study, therefore the subject of the present study. Methods and results It was a pro- spective, multicenter, randomized controlled trial. Between April 2012 and July 2015, 399 acute STEMI patients with single vessel disease and intermediate （40%-70%） stenosis of the culprit lesion before or after aspiration thrombectomy and/or intracoronary tirofiban （15 pg/kg） were enrolled and were randomly assigned （h 1） to stenting group （n = 201） and non-stenting group （n = 198）. In stenting group, patients received pharmacologic therapy plus standard percutaneous coronary intervention （PCI） with stent implantation. In non-stenting group, pa- tients received pharmacologic therapy and PCI （thrombectomy）, but without dilatation or stenting. Primary endpoint was 12-month rate of major adverse cardiac and eerebrovascular events （MACCE）, a composite of cardiac death, non-fatal myocardial infarction （M1）, repeat re- vascularization and stroke. Secondary endpoints were 12-month rates of all cause death, ischemia driven admission and bleeding complica- tion. Median follow-up time was 12.4 ~ 3.1 months. At 12 months, MACCE occurred in 8.0% of the patients in stenting group, as compared with 15.2% in the non-stenting group （adjusted HR： 0.42, 95% Ch 0.19-0.89, P = 0.02）. The stenting group had lower non-fatal MI rate than non-stenting group, （1.5% vs. 5.5%, P = 0.03）. The two groups shared similar cardiac death, repeat revascularization, stroke, all cause death, ischemia driven readmission and bleeding rates at 12 months. Conclusions Stent implantation had better efficacy and safety in reducing MACCE risks among acute STEMI patients with single vessel intermediate stenosis culprit lesions.

Predicting dyslipidemia after liver transplantation:A significant role of recipient metabolic inflammation profile

What we know about ST13, a co-factor of heat shock protein, or a tumor suppressor?

This article is to summarize the molecular and functional analysis of the gene “suppression of tumorigenicity 13” (ST13). ST13 is in fact the gene encoding Hsp70 interacting protein (Hip), a co-factor (co-chaperone) of the 70-kDa heat shock proteins (Hsc/Hsp70). By collaborating with other positive co-factors such as Hsp40 and the Hsp70-Hsp90 organizing protein (Hop), or competing with negative co-factors such as Bcl2-associated athanogen 1 (Bag1), Hip facilitates may facilitate the chaperone function of Hsc/Hsp70 in protein folding and repair, and in controlling the activity of regulatory proteins such as steroid receptors and regulators of proliferation or apoptosis. Although the nomenclature of ST13 implies a role in the suppression of tumorigenicity (ST), to date available experimental data are not sufficient to support its role in cancer development, except for the possible down-regulation of ST13 in gastric and colorectal cancers. Further investigation of this gene at the physiological level would benefit our understanding of diseases such as endocrinological disorders, cancer, and neurodegeneration commonly associated with protein misfolding.

Characteristics of the magnetic-island-induced ion temperature gradient （MITG） mode are studied through gyrofluid simulations in the slab geometry, focusing on the effects of Landau damping, equilibrium magnetic shear （EMS）, and pressure flattening. It is shown that the magnetic island may enhance the Landau damping of the system by inducing the radial magnetic field. Moreover, the radial eigenmode numbers of most MITG poloidal harmonics are increased by the magnetic island so that the MITG mode is destabilized in the low EMS regime. In addition, the pressure profile flattening effect inside a magnetic island hardly affects the growth of the whole MITG mode, while it has different local effects near the O-point and the X-point regions. In comparison with the non-zero-order perturbations, only the quasi-linear flattening effect due to the zonal pressure is the effective component to impact the growth rate of the mode.

Characterizing the optimal operation of photocatalytic degradation of BDE-209 by nano-sized TiO_2

Brominated flame retardants have been widely used in industry. There is a rapid growing public concern for their availabilities in the environment. Advanced oxidation process （AOP） is a promising and efficient technology which may be used to remove emerging chemicals such as brominated flame retardants. This study aims at investigating optimal operational conditions for the removal of BDE-209 using nano-scaled titanium（IV） oxide. The residual PBDE congeners after photocatalytical degradation of BDE-209 by TiO2 were analysed by gas chromatography-mass spectrometry （GC-MS）. It was found that the degradability of BDE-209 by TiO2 was attributed to its photocatalytic activity but not the small size of the particles. The half-life of removing BDE-209 by TiO2 was 3.05 days under visible light. Tetra- and penta-BDEs were the major degraded products of BDE-209. Optimum conditions for photocatalytical degradation of BDE-209 was found to be at pH 12 （93% ± 1%）, 5, 10, 20 mg/L （93.0% ± 1.70%, 91.6% ± 3.21%, 91.9% ± 0.952%, respectively）, respectively of humic acid and in the form of anatase/rutile TiO2 （82% ± 3%）. Hence, the efficiency of removing BDE-209 can be maximized while being cost effective at the said operating conditions.

Comparison of the performance of the CRUSADE, ACUITY-HORIZONS, and ACTION bleeding scores in ACS patients undergoing PCI： insights from a cohort of 4939 patients in China

Background The CRUSADE, ACTION and ACUITY-HORIZONS scores are commonly used for predicting in-hospital major bleeding events in patients with acute coronary syndrome （ACS）, but the homogeneous nature of these models＇ population limits simple ex- trapolation to other local population. We aimed to compare the performance of the three risk models in Chinese patients. Methods We evaluated the performance of the three predicting scores for predicting in-hospital major bleeding events defined by thrombolysis in myocar- dial infarction （TIMI） serious （major and minor） episodes, in a cohort of Chinese ACS patients with either non-ST-elevation ACS （NSTE-ACS） or ST-elevation myocardial infarction （STEMI）. Calibration and discrimination of the three risk models were evaluated by the Hosmer-Lemeshow test and C-statistic, respectively. We compared the predictive accuracy of the risk scores by the Delong non-parametric test. Results TIMI serious bleeding rate was 1.1% overall （1.9% and 0.86% for STEMI and NSTE-ACS, respectively）. The CRUSADE, ACTION and ACUTIY-HOR/ZONS scores showed an adequate discriminatory capacity for major bleeding： in overall patients, the C-statistic was 0.80, 0.77, and 0.70, respectively; in NSTE-ACS patients, the C-statistic was 0.73, 0.72, and 0.64, respectively; in STEMI patients, the C-statistic was 0.91, 0.92, and 0.75, respectively. The C-statistic for the ACUITY-HORIZONS model was significantly lower than those of the CRUSADE and ACTION scores for the prediction of TIM/serious bleeding in overall patients （compared with CRUSADE, z = 3.83, P = 0.02; compared with ACTION, z = 3.51, P = 0.03）; in NSTE-ACS patients （compared with CRUSADE, z = 2.37, P = 0.01; compared with ACTION, z = 2.11, P = 0.04）, and in STEMI patients （compared with CRUSADE, z = 2.6.77, P = 0.02; compared with AC- TION, z = 7.91, P = 0.002）. Comparative features of colorectal and gastric cancers with microsatellite instability in Chinese patients
