AIM:Portopulmonary hypertension is a serious complication of chronic liver *** aim was to search into the effect of terlipressin on systolic pulmonary artery pressure among cirrhotic patients. METHODS:Twelve patients(6 males and 6 females)with liver cirrhosis were recruited in the *** blood gas samples were obtained in sitting position at *** enhanced echocardiography and measurements of systolic pulmonary artery pressure were performed before and after the intravenous injection of 2 mg terlipressin. RESULTS:Of 12 patients studied,the contrast enhanced echocardiography was positive in 5,and the positive findings in contrast enhanced echocardiography were reversed to normal in two after terlipressin *** mean systolic pulmonary artery pressure was 25.5±3.6 mmHg before terlipressin injection,and was 22.5+2.5 mmHg after terlipressin(P=0.003).The systolic pulmonary artery pressure was above 25 mmHg in seven of these 12 patients. After the terlipressin injection,systolic pulmonary artery pressure was<25 mmHg in four of these cases(58.3% vs 25%,P=0.04). CONCLUSION:Terlipressin can decrease the systolic pulmonary artery pressure in patients with liver cirrhosis.

In a previous paper, we proposed that a QCD gas that may be a possible candidate for the general theory of gravity (GR) ether may be comprised of ud~du~ exotic mesons. A method to determine the effective mass of the ud~du~ exotic meson and the Friedmann-Robertson-Walker (FRW) metric scale factor equation of state dimensionless parameter, w, by measuring the pseudo-first order β decay rates expected to be inversely proportional to the QCD gas atmospheric density was given. Here, we propose to measure the β decay rate, t1/2, and the earth distance to the milky-way galaxy super massive black hole (SMBH), hSMBH, at the earth aphelion each year for several years, and fit the data with the linear curve: -lnt1/2 = ahSMBH + b. The slope parameter, a, and the free parameter, b, may be used to calculate the Kerr spin parameter and determine if the QCD gas density on the ergosphere remains constant in time, or alternatively, grows in time according to Corley and Jacobson’s proposed black hole laser process.

We propose that the exotic meson tetraquark ud~dũintroduced in previous papers, may be a pseudo-Goldstone boson having a tetrahedron geometry and symmetry. The transition from the neutral pion superposition of two free mesons, dd~ and uũ, to the tetrahedron geometry with optional two chiral states may be the symmetry breaking of the QCD ground state. The ud~dũtetrahedron mass may be calculated by measuring the β decay rate variability. We assume that electrons and positrons are composite particle exotic tetraquarks, dũdd~ for the electrons and ud~dd~ for the positrons and confined by the strong force. We propose that the QCD tetrahedrons play a central role in electron pairing mechanism in both chemical bond forming and superconductor Cooper pairs. We propose a hypothesis where the QCD ground state tetrahedrons play a central role in low energy physics where quark exchange reactions between particles and the QCD tetrahedrons via gluon junctions transfer all the forces. The QCD ground state ud~dũtetrahedrons hypothesis provides a symmetry breaking and a mass gap may be created by the ground state QCD tetrahedrons Bose-Einstein condensate.

Flash floods(FFs)are amongst the most devastating hazards in arid regions in response to climate change and can cause the loss of agricultural land,human lives and *** of the major challenges is the high-intensity rainfall events affecting low-lying areas that are vulnerable to *** works in this field have been conducted using ensemble machine learning models and geohydrological ***,the current advancement of eXtreme deep learning,which is named eXtreme deep factorisation machine(xDeepFM),for FF susceptibility mapping(FSM)is lacking in the *** current study introduces a new model and employs a previously unapplied approach to enhance FSM for capturing the severity of *** proposed approach has three main objectives:(i)During-and after-flood effects are assessed through flood detection techniques using Sentinel-1 data.(ii)Flood inventory is updated using remote sensing-based *** derived flood effects are implemented in the next step.(iii)An FSM map is generated using an xDeepFM ***,this study aims to apply xDeepFM to estimate susceptible areas using 13 factors in the emirates of Fujairah,*** performance metrics show a recall of 0.9488),an F1-score of 0.9107),precision of(0.8756)and an overall accuracy of 90.41%.The accuracy of the applied xDeepFM model is compared with that of traditional machine learning models,specifically the deep neural network(78%),support vector machine(85.4%)and random forest(88.75%).Random forest achieves high accuracy,which is due to its strong performance that depends on factors contribution,dataset size and quality,and available computational ***,the xDeepFM model works efficiently for complicated prediction problems having high non-collinearity and huge *** obtained map denotes that the narrow basins,lowland coastal areas and riverbank areas up to 5 km(Fujairah)are highly prone to FF,whilst the alluvial plains in Al Dhaid and hilly regio

A double-well potential model is proposed for the pionic deuterium that enables to calculate the energy split, the potential barrier height and estimate the pion tetrahedron edge length. We propose that pion tetrahedrons, πTd = ud~dũ, play a central role in the Yukawa interaction by enabling quark exchange reactions between protons and neutrons by tunneling through a potential barrier. A vacuum polarization Feynman diagram is proposed for the πTd having chains of fermion loops for the two valence quarks and anti-quarks connected by gluons. With a higher order vacuum polarization diagram, the d and u quark loops are interleaved and the chiral symmetry is broken dynamically. The proposed πTd vacuum polarization integral does not diverge in both the IR and UV limits and vanishes in the limit of an infinite pion tetrahedron condensate. We propose a new Delbruck scattering Feynman diagram that includes d and u quark and anti-quark interleaved loops. We further propose that conversion of gravitons to photons may occur via quark and anti-quark loops that describe the pion tetrahedrons dynamics in the vacuum and may also transfer gravitational waves.

We propose that the trapped antimatter in super massive black hole ergoregions acts as detonators that triggers black hole to white hole transitions creating huge BHs explosions that generate BH spray that acts as seeds for new galaxies creation. We propose that by mapping and simulating the cosmic web structure, it may be possible to learn if the universe was created in a single big bang that started a single chain of BH explosions mini-creation event cycles, or alternatively, the BH explosions mini-creation event cycles are uncorrelated spacelike events, and the universe had no single primeval atom beginning. .

Effective resource allocation and accountability are critical for public services in countries facing a crisis,to ensure minimum inequality and even save *** innovation is introduced as public sector reform to improve the performance and accountability of public sector *** the said benefit,little has been done to study public administrative innovation in countries under *** study extends the literature by examining the attitude of civil service managers to the administrative innovation in the Yemeni government representing a country in crisis from an integrated perspective of technology-organization-environment(TOE)and diffusion of innovation(DOI)*** was collected from a survey of 200 civil service managers of the Yemeni central *** result shows that the attitude of managers to private sector management style is scored the highest representing the key tool of the public administrative *** the other hand,the downsizing elements were ranked the least significant as public sector reform *** explanatory model highlights the use of information technology,quality of human resources,and budget management orientation as significant factors for the applicability of public administrative innovation in the Yemeni central government.

Mathematical analysis of corneal remodelling after intracorneal ring surgery in keratoconus

AIM：To represent mathematically the intersection between the ectatic corneal geometry and the plane of intracorneal ring implants（ICRS）in order to determine the corneal response to ICRS surgery in keratoconus（KC）.Thereafter,to present the concept and early results of a newly derived topography-guided nomogram for ICRS surgery for the treatment of keratectasia. METHODS：The corneal rings plane intersection was modelled to a conic section. Ring effect was the result of：the ring size,position（steep vs flat）,location（distance from the geometric centre of the cornea）,and the discrepancy between the ring＇s curvature and the tunnel＇s *** laser was used to create the tunnels and the incision sites were chosen according to the nomogram in order to place the thickest ring in the steepest portion of the cornea regardless of the astigmatism axis of ***：The conic section had a more prolate shape in the steep area of the cornea than in the flat area,depending on the corneal sagittal curvature. Equal ring size had more flattening effect in the steep area than in the flat area. Thick segment should be implanted under the steep portion of the cornea regardless of the cylinder axis of refraction. Single segment in the steep area was sufficient in early and moderate cases of KC. The new nomogram provided more topographic regularity with significant reduction of astigmatism and better improvement in uncorrected visual acuity（UCVA）and best-corrected visual acuity（BCVA）than the conventional nomogram. CONCLUSION：The newly derived nomogram can produce better results than the conventional nomogram. Moreover,based on this concept,a new nomogram can be integrated into the femtosecond laser software to create topography-guided,customized,elliptical tunnels with modified focal asphericity that allows for customized focal flattening of the irregularly steepened ectatic cornea.

Management of cataract in keratoconus: early visual outcomes of different treatment modalities

A review of 31 eyes with keratoconus who developed cataract and underwent phacoemulsification. Visual acuities were measured 1 mo postoperatively. Six eyes with a history of good corrected distance visual acuity(CDVA) and a similar refractive and topographic astigmatic axis were implanted with toric intraocular lenses(IOLs). The mean postoperative uncorrected distance visual acuity(UDVA) was 0.2 logMAR with a spherical equivalent(SE): 0.75 D. Eleven eyes with a history of good CDVA and different refractive and topographic axis were implanted with monofocal IOL+/-Toric implantable collamer lenses to treat anisometropia and ametropia;mean UDVA was 0.25 logMAR with a mean SE:-0.51 D postoperatively. Six eyes with poor CDVA were first treated with intra-corneal ring segments, followed by phacoemulsification, the mean postoperative UDVA was 0.82 logMAR with an SE: 0.22 D. Eight eyes had advanced ectesia and received combined phacoemulsification and penetrating keratoplasty. Our approach is efficient in addressing ametropia after cataract surgery in keratoconic eyes.
