Metastatic Spinal Tumors: Diagnostic Methods, Management and Prognosis at the Yaounde Central Hospital and Yaounde General Hospital

Introduction: Metastatic spinal tumors (MST) refer to secondary involvement of the vertebral column by hematogenously-disseminated metastatic cells. They could affect either the bony structures or the spinal cords. Mechanical instability and neurologic deficits resulting from spinal cord compression are the most common manifestations. Surgical intervention remains the most effective treatment for about 20% of patients who present with spinal cord compression. The prognosis is relatively poor. This work has as objectives to describe: the diagnostic tools, the different modalities of management and the prognostic elements of spine metastasis. Methodology: We conducted an ambispective cross-sectional descriptive study;with retrospective data collection from January 2015 to December 2021 and prospective collection from January to April 2022 in the “Neurosurgery” unit of the Yaounde Central Hospital and the “Oncology and Neurosurgery” units of Yaounde General Hospital. Result: We included 101 patients. The M/F sex ratio was 1.66. The average age of the participants was 56.44 years (±14.19 SD) with a median of 58 years. Metastatic spinal tumors were discovered in 61.39% of patients with a previously known primary tumor and 21.78% of patients had newly discovered tumors. The neurologic examination revealed a vertebral syndrome in 79.21% of cases, radicular syndrome in 60.40% and sub-lesional syndrome in 59.89%. Sensory disorders accounted for 39.60% and sphincter disorders accounted for 34.65%. According to the degree of severity, the lesions were classified as Frankel e (37.62%) followed by Frankel D (21.78%). Metastatic lesions were mostly found at the thoracic vertebrae (68.25%) and lumbar vertebrae (22.22%). The most represented primary tumors were: prostate tumors (41.58%) and breast tumors (23.76%);followed by malignant hemopathies (15.84%). The hemodynamic management of elderly patients with sepsis

Sepsis is among the most common reason for admission to intensive care units throughout the world. In the US and most Western nations sepsis is largely a disease of the elderly. Management of elderly patients with severe sepsis is challenging. early recognition of this syndrome, together with the early administration of appropriate antibiotics and cautious fluid resuscitation is the cornerstone of therapy. echocardiography together with non-invasive or invasive hemodynamic monitoring is recommended in patients who have responded poorly to fluids or have significant underlying cardiac disease. This paper reviews the hemodynamic changes that characterize sepsis, particularly as they apply to elderly patients and provides recommendations for the management of these patients.

This paper explores the concept of physical literacy and highlights the foundational work in this area, particularly that of Margaret Whitehead. The relationship of physical literacy to physical education is also discussed with potential ramifications for teachers and learners who adopt physical literacy as the goal of the subject area.

Predicting the effects of selected reservoir petrophysical properties on bottomhole pressure via three computational intelligence techniques

This study investigates the effects of selected petrophysical properties on predicting flowing well bottomhole *** efficiently situate the essence of this investigation,genetic,imperialist competitive and whale optimization algorithms were used in predicting the bottomhole pressure of a reservoir using production data and some selected petrophysical properties as independent input variables.A total of 15,633 data sets were collected from Volvo field in Norway,and after screening the data,a total of 9161 data sets were used to develop apt computational intelligence *** data were randomly divided into three different groups:training,validation,and testing *** case scenarios were considered in this *** first scenario involved the prediction of flowing bottomhole pressure using only eleven independent variables,while the second scenario bothered on the prediction of the same flowing bottomhole pressure using the same independent variables and two selected petrophysical properties(porosity and permeability).each of the two scenarios involved as implied in the first scenario,the use of three(3)heuristic search optimizers to determine optimal model *** optimizers were allowed to choose the optimal number of layers(between 1 and 10),the optimal number of nodal points(between 10 and 100)for each layer and the optimal learning rate required per task/*** results,showed that the models were able to learn the problems well with the learning rate fixed from 0.001 to 0.0001,although this became successively slower as the leaning rate *** the chosen model configuration,the results suggest that a moderate learning rate of 0.0001 results in good model performance on the trained and tested data *** the three heuristic search optimizers based on minimum MSe,RMSe,MAe and highest coefficient of determination(R^(2))for the actual and predicted values,shows that the imperialist competitive algorithm optimizer predicted

How Do Sugars Regulate Plant Growth and Development？ New Insight into the Role of Treha Iose-6-Phosphate

Plant growth and development are tightly controlled in response to environmental conditions that influence the availability of photosynthetic carbon in the form of sucrose. Trehalose-6-phosphate （T6P）, the precursor of trehalose in the biosynthetic pathway, is an important signaling metabolite that is involved in the regulation of plant growth and development in response to carbon availability. In addition to the plant＇s own pathway for trehalose synthesis, formation of T6P or trehalose by pathogens can result in the reprogramming of plant metabolism and development. Developmental processes that are regulated by T6P range from embryo development to leaf senescence. Some of these processes are regulated in interaction with phytohormones, such as auxin. A key interacting factor of T6P signaling in response to the environment is the protein kinase sucrose non-fermenting related kinase-1 （SnRK1）, whose catalytic activity is inhibited by T6R SnRK1 is most likely involved in the adjustment of metabolism and growth in response to starvation. The transcription factor bZlP11 has recently been identified as a new player in the T6P/SnRK1 regulatory pathway. By inhibiting SnRK1, T6P promotes biosynthetic reactions. This regulation has important consequences for crop production, for example, in the developing wheat grain and during the growth of potato tubers.

MRI-determined fat content of human liver,pancreas and kidney

AIM:To assess and correlate the lipid content of various organs in obese subjects and in persons with a normal body ***:Magnetic resonance spectroscopy and a previously validated gradient echo magnetic resonance imaging method with Dixon's two point technique were used in this study to quantify fat in liver,pancreas as well as ***:In 36 volunteers with body mass index(BMI) ranging from 20.0 to 42.9 kg/m2,the median fat content of liver,pancreas and kidney was 2.3%(interquartile range:0.2%-7.8%),2.7%(1.0%-6.5%) and 0.7%(0.1%-1.4%),*** and subcutaneous fat correlated significantly with liver and pancreas fat *** show for the first time the significant correlation of liver and pancreas fat content in healthy controls(r = 0.43,P ese observations are related to body weight as measured by BMI and the amount of subcutaneous *** fat content is very low and correlates with none of the other fat ***:Renal lipid accumulation,unlike the coupled accumulations of fat in liver and pancreas,is not observed in obese *** suggestions made in previous studies,renal lipid accumulation appears not to be involved in the pathogenesis of renal disease in humans.

AIM： To assess the characteristics and quality of endoscopic ultrasonography-guided fine needle aspiration （eUS-FNA） in a large panel of endosonographers.MeTHODS： A survey was conducted during the 13th annual live course of endoscopic ultrasonography （eUS） held in Amsterdam, Netherlands. A 2-page question- naire was developed for the study. Content validity of the questionnaire was determined based on input by experts in the field and a review of the relevant literature. It contained 30 questions that pertained to demographics and the current practice for eUS-FNA of responders, including sampling technique, sample processing, cytopathological diagnosis and sensitivity of eUS-FNA for the diagnosis of solid mass lesions. One hundred and sixty-one endosonographers whoattended the course were asked to answer the survey. This allowed assessing the current practice of eUS-FNA as well as the self-reported sensitivity of eUS-FNA for the diagnosis of solid mass lesions. We also examined which factors were associated with a self-reported sensitivity of eUS-FNA for the diagnosis of solid mass lesions 〉 80%.ReSULTS： Completed surveys were collected from 92 （57.1%） of 161 endosonographers who attended the conference. The endosonographers had been practio ing endoscopy and eUS for 22.5 4. 7.8 years and 4.8 4- 4.1 years, respectively; one third of them worked in a hospital with an annual caseload 〉 100 eUS-FNA. endoscopy practices were located in 29 countries, in- cluding 13 countries in Western europe that totaled 75.3% of the responses. Only one third of endosonog raphers reported a sensitivity for the diagnosis of solid mass lesions 〉 80% （interquartile range of sensitivities, 25.0%-75.0%）.
