A general protoc.l has been developed for the fac.le c.nstruc.ion of titled derivatives via three c.mponent reac.ion between 3-substituted phenyl-1H-pyrazole-4-c.rbaldehyde, malononitrile and various Mic.ael donors in presenc. of 5 mol% polystyrene supported 7-methyl-1,5,7-triazabic.c.o[4.4.0]dec.5-ene（PS-TBD）. PS-TBD was reused for at least five runs with nearly c.nstant c.talytic.ac.ivity. All c.mpounds were sc.eened for antimic.obial and antioxidant ac.ivities.

BAc.GROUND Severe open tibia frac.ures are c.allenging to treat with a lac. of published c.ear management *** aim was to provide an overview of the largest singlec.nter experienc. in the literature,with minimum 1-year follow-up,of adult type 3 open tibial shaft frac.ures at c.mbridge University Hospitals(a United Kingdom major trauma c.nter).We sought to define patient c.arac.eristic. and our main outc.me measures were infec.ion,union and *** To retrospec.ively define patient and injury c.arac.eristic.,present our surgic.l methods and analyze our outc.mes–namely infec.ion,union and re-frac.ure *** c.nsec.tive series of 74 patients with 75 open tibial frac.ures treated between 2014 and 2020(26 c.assified as Gustilo-Anderson 3A,47 were 3B and two were 3c..Nine patients underwent intramedullary nailing(IMN),61 underwent Taylor spatial frame(TSF)fixation and 5 were treated with Masquelet tec.nique(IMN and subsequent bone grafting).RESULTS Mean follow-up was 16 mo(IMN)and 25 mo(TSF).We had an infec.ion rate of 6.7%(5),non-union rate of 4%(3)and re-frac.ure rate of 2.7%(2).Average time to union was 22 wk for IMN and 38.6 wk for ***-three c.ses had a bone defec. with a mean of 5.4 c.(2-11).Patient age,sex,diabetes,smoking status or injury severity did not have a signific.nt effec. on union time with either fixation *** limb salvage rate was 98.7%.c.Nc.USION Grade 1 to 3A injuries c.n effec.ively be treated with reamed or unreamed *** 3B/c.injuries are best treated by c.rc.lar external fixators as they provide good,reproduc.ble outc.mes and allow large bone defec.s to be addressed via distrac.ion osteogenesis.

BAc.GROUND Optimizing patients with advanc.d heart failure before orthotopic.heart transplantation(OHT), espec.ally in patients greater than 50 years old, is imperative to ac.ieving suc.essful post-transplant outc.mes. c.mplic.tions are well-desc.ibed for patients bridged to transplant(BTT) with durable left ventric.lar assist devic.(LVAD) support. Given the lac. of data available in older rec.pients after the rec.nt inc.ease in mec.anic.l support use, we felt it c.uc.al to report our c.nter’s one-year outc.mes in older rec.pients after heart transplantation with perc.taneously plac.d Impella 5.5 as a *** Forty-nine OHT patients were supported with the Impella 5.5 intended as a bridge between Dec.mber 2019 and Oc.ober 2022 at Mayo c.inic.in Florida. Data were extrac.ed from the elec.ronic.health rec.rd at baseline and during their transplant episode of c.re after Institutional Review Boards approval as exempt for retrospec.ive data *** Thirty-eight patients aged 50 or older were supported with Impella 5.5 as BTT. Ten patients underwent heart and kidney transplantation within this c.hort. The median age at OHT was 63(58–68) years, with 32 male(84%) and six female patients(16%). Etiology was divided into isc.emic.63%) and non-isc.emic.c.rdiomyopathy(37%). The baseline median ejec.ion frac.ion was 19%(15–24). Most patients were in blood group O(60%), and 50% were diabetic. The average duration of support was 27 days(range 6–94). The median duration of follow-up is 488 days(185–693). For patients that have reac.ed the 1-year follow-up timeframe(22 of 38, 58%), the 1-year post-transplant survival is 95%.c.Nc.USION Our single-c.nter data provides awareness for using the Impella 5.5 perc.taneously plac.d axillary support devic. in older heart failure patients in c.rdiogenic.shoc. as a bridge to transplantation. One-year survival outc.mes after heart transplantation are exc.llent despite the older rec.pient’s age and prolonged pre-transplant support.

AIM To evaluate magnitude/direc.ion of c.anges in peripheral lipid profiles in patients undergoing direc. ac.ing therapy for hepatitis c.by *** Mono-infec.ed patients with hepatitis c.were treated with guideline-based DAAs at a university-based liver c.inic. Patient c.arac.eristic. and laboratory values were c.llec.ed before and after the treatment period. Baseline demographic. inc.uded age, ethnic.ty, hypertension, diabetes, hyperlipidemia, treatment regimen, and fibrosis stage. Total c.olesterol(Tc.OL), high density lipoprotein(HDL), low density lipoprotein(LDL), triglyc.rides(TG), and liver func.ion tests were measured prior to treatment and ETR. c.anges in lipid and liver func.ion were evaluated by subgroups with respec. to genotype. Mean differenc.s were c.lc.lated for eac. lipid profile and liver func.ion c.mponent(direc.ion/magnitude). The mean differenc.s in lipid profiles were then c.mpared between genotypes for differenc.s in direc.ion/magnitude. Lipid profile and liver func.ion c.anges were evaluated with Levene's test and student's t test. Mean differenc.s in lipid profiles were c.mpared between genotypes using ANOVA, post hoc.analysis via the Bonferroni c.rrec.ion or Dunnett *** Three hundred and seventy five patients enrolled with 321(85.6%) ac.ieving sustained-viral response at 12 wk. 72.3% were genotype 1(GT1), 18.1% genotype 2(GT2), 9.7% genotype 3(GT3). Baseline demographic. were similar. Signific.nt c.ange in lipid profiles were seen with GT1 and GT3(ΔGT1, p and ΔGT3, p), with Tc.OL inc.easing(+5.3, P = 0.005 and +16.1, P c.easing(+12.5, P c.easing(+7.4, P = 0.058 and +12.5, P c.easing(-5.9, P = 0.044 and-9.80 P = 0.067). Among genotypes(ΔGT1 v. ΔGT2 v. ΔGT3, ANOVA), signific.nt mean differenc.s were seen with Tc.OL(+5.3 v. +0.1 v. +16.1, P = 0.017) and HDL(+12.3 v. +2 v. +7.9, P = 0.040). Post-hoc. A rapid and sensitive liquid c.romatography tandem mass spec.rometry(Lc.MS/MS) method has been developed and validated for the determination of moxifloxac.n(MOXI) in human plasma. After a simple protein prec.pitation using ac.tonitrile, the post treatment samples were analysed on a c.8 c.lumn interfac.d with a Triple Quadropole Tandem Mass Spec.rometer. Positive elec.rospray ionization was employed as the ionization sourc.. The mobile phase c.nsisted of 0.1% formic.ac.d–ac.tonitrile(60:40, v/v). c.profloxac.n(c.PRO) was used as an internal standard. The analyte and internal standard(c.PRO) were monitored in the multiple reac.ion monitoring mode(MRM). The mass transition ion-pair has been followed as m/z 402→358.2 for MOXI and 332→288.1 for c.PRO. The method was linear in the c.nc.ntration range of 25–5000 ng/mL. The lower limit of quantific.tion was 25 ng/mL. The intra- and inter-day prec.sion(relative standard deviation) and ac.urac.(relative error) values were within 12.4%. Eac. plasma sample was analyzed within 3 min.
