Background Large-river decision-makers are charged with maintaining diverse ecosystem services through unprecedented social-ecological transformations as climate change and other global stressors intensify. The interconnected, dendritic habitats of rivers, which often demarcate jurisdictional boundaries, generate complex management challenges. Here, we explore how the Resist–Accept–Direct(RAD) framework may enhance large-river management by promoting coordinated and deliberate responses to social-ecological trajectories of change. The RAD framework identifies the full decision space of potential management approaches, wherein managers may resist change to maintain historical conditions, accept change toward different conditions, or direct change to a specified future with novel conditions. In the Upper Mississippi River System, managers are facing social-ecological transformations from more frequent and extreme high-water events. We illustrate how RAD-informed basin-, reach-, and site-scale decisions could:(1) provide cross-spatial scale framing;(2) open the entire decision space of potential management approaches; and(3) enhance coordinated inter-jurisdictional management in response to the trajectory of the Upper Mississippi River *** The RAD framework helps identify plausible long-term trajectories in different reaches(or subbasins) of the river and how the associated social-ecological transformations could be managed by altering site-scale conditions. Strategic reach-scale objectives may reprioritize how, where, and when site conditions could be altered to contribute to the basin goal, given the basin's plausible trajectories of change(e.g., by coordinating action across sites to alter habitat connectivity, diversity, and redundancy in the river mosaic).Conclusions When faced with long-term systemic transformations(e.g., > 50 years), the RAD framework helps explicitly consider whether or when the basin vision or goals may no longer be achievable,

Psoriasis is a chronic inflammatory skin condition that has a fairly wide range of clinical *** psoriasis,which is the most common manifestation of psoriasis,is located on one end of the spectrum,dominated by adaptive immune responses,whereas the rarer pustular psoriasis lies on the opposite end,dominated by innate and autoinflammatory immune *** recent years,genetic studies have identified six genetic variants that predispose to pustular psoriasis,and these have highlighted the role of IL-36 cytokines as central to pustular psoriasis *** this review,we discuss the presentation and clinical subtypes of pustular psoriasis,contribution of genetic predisposing variants,critical role of the IL-36 family of cytokines in disease pathophysiology,and treatment perspectives for pustular *** further outline the application of appropriate mouse models for the study of pustular psoriasis and address the outstanding questions and issues related to our understanding of the mechanisms involved in pustular psoriasis.

Definition of colorectal anastomotic leakage: A consensus survey among Dutch and Chinese colorectal surgeons

AIM To determine the level of consensus on the definition of colorectal anastomotic leakage(CAL) among Dutch and Chinese colorectal *** Dutch and Chinese colorectal surgeons were asked to partake in an online questionnaire. Consensus in the online questionnaire was defined as > 80% agreement between respondents on various statements regarding a general definition of CAL,and regarding clinical and radiological diagnosis of the *** Fifty-nine Dutch and 202 Chinese dedicated colorectal surgeons participated in the online survey. Consensus was found on only one of the proposed elements of a general definition of CAL in both countries: ‘extravasation of contrast medium after rectal enema on a CT scan'. Another two were found relevant according to Dutch surgeons: ‘necrosis of the anastomosis found during reoperation',and ‘a radiological collection treated with percutaneous drainage'. No consensus was found for all other proposed elements that may be included in a general *** There is no universally accepted definition of CAL in the Netherlands and China. Diagnosis of CAL based on clinical manifestations remains a point of discussion in both countries. Dutch surgeons are more likely to report ‘subclinical' leaks as CAL,which partly explains the higher reported Dutch CAL rates.

An automated dispensing system for improving medication timing in the emergency department

BACkGROUND:Numerous medical conditions require timely medication administration in the emergency department(ED).Automated dispensing systems(ADSs)store premixed common doses at the point-of-care to minimize time to administration,but the use of such automation to improved time to medication administration has not been *** vancomycin is a commonly used empiric antimicrobial,we sought to quantify the effect of using an ADS on time to drug delivery in patients presenting to the *** study aimed to determine the efficacy of utilizing an ADS to improve time to administration of vancomycin and determine any negative effects on dosing ***:The institional review board approved the retrospective quality improvement study took place in a single,urban academic tertiary care ED with an annual census of 80 *** subjects were all patients receiving vancomycin for the management of sepsis between March 1 to September 30,2008 and the same time period in *** primary outcome was the proportion of patients who received vancomycin within one hour of bed placement and the secondary outcome was dosing ***:Sixty-three patients had weight and dosing information available(29 before and 34after intervention)and were included in the *** intervention,no patient received vancomycin in less than 60 minutes,while after intervention 14.7%of the patients received it in less than 60minutes(difference in proportions 14.7%,95%CI 0.39%-30.0%,P=0.04).A similar proportion of the patients received correct dosing before and after intervention(44.8%vs.41.2%,difference in proportions 3.7%,95%CI-20.0%-26.7%,P=0.770).CONCLUSION:The use of an ADS may improve the timing of medication administration in patients presenting to the ED without affecting dosing appropriateness.

目的应用认知访谈了解汉化后全身麻醉患者满意度问卷在目标人群中的理解情况,从而促进中英版问卷语言表达的一致性。方法 2017年6-8月,采用目的抽样法选取在上海市某三级甲等综合性医院住院行全身麻醉手术的15例患者作为访谈对象,对其进行2轮认知访谈,了解受访者对各条目的理解情况,并根据访谈结果对问卷条目进行修订。结果第1轮访谈结果显示,5个条目因"语法结构不完善、书面化语言"进行修订;第2轮访谈结果显示,受访者对汉化及修订后条目的语言理解与原问卷表达均一致。结论认知访谈有助于全面掌握全身麻醉患者满意度问卷汉化版问卷在目标人群中的理解情况,并有效指导课题小组对目标人群存在疑惑的条目进行修订,从而减少受访者对原问卷条目表达的理解差异,使汉化后问卷语言通俗易懂,表达方式本土化,促进中英版本表达的一致性。

[背景]已证实住宅杀虫剂暴露与成人和儿童的不良健康结局相关。高质量的暴露评估是确认这些关联的关键。以往的流行病学研究采用一次测量地毯灰尘中杀虫剂浓度来描述个体长期暴露的平均值。如果浓度随着时间的推移而变化，那么这种方法会导致对暴露明显的错误分类，削弱对风险的估计。[目的]评估地毯灰尘样本中杀虫剂浓度的可重复性和流行病学研究中依赖于一个样本的潜在衰减偏倚。[方法]在2003-2005年期间，从加州Fresno县的21个家庭中，重复收集地毯灰尘样本（中位数=3，范围1～7）。使用气相色谱-质谱法分析灰尘中的13种杀虫剂；采用混合效应模型估计家庭之间和家庭内的方差。计算每一种杀虫剂的组内相关系数（ICC），并估计收集单次灰尘样品的假设病例对照研究衰减回归系数。[结果]ICC的中位数为0．73（范围0．37—0．95），表明大多数杀虫剂家庭之间的变异性高于家庭内部的变异性。对于13种化合物中进行评估的7种，与使用单一灰尘样本相关的预期衰减偏倚估计为≤30％。[结论]对于研究的几种杀虫剂，使用一个灰尘样本代表约为2年的暴露期，其比值比不会大幅衰减。还需要进一步研究，以确定上述研究结果是否可以代表更长的暴露时间并且适用于其他杀虫剂。
