This work aims at characterizing organic matter produced by an alga Euglena gracil~ and a cyanobacteria Microcystis aeruginosa and assessing the evolution of its characteristics during growth. A culture medium was optimized. The species growth phases were monitored using both visible spectrophotometry and flow cytometry cell counting. Organic matter fractionation according to hydrophobicity and specific UV absorbance （SUVA） index were used to specifically characterize the produced algal organic matter （AOM）. The AOM characteristics were both growth phase and species dependent. However, a similar evolution was observed. The hydrophilic fraction （HPI） was the major fraction whatever the growth phases and was almost the only one produced during lag and exponential phases. It represented around 75% of AOM during exponential phase and then decreased when the stationary phase appeared. It represented 46% and 60% of the AOM during late decline phase for the cyanobacteria and the alga respectively. The hydrophobic （HPO） and transphilic （TPH） fractions started to appear from the beginning of the stationary phase with more hydrophobic compounds coming from intracellular organic material of dying cells. HPO and TPH percentages still increased during the decline phase probably because of two additional processes： photo-dissolution and leaching of particulate organic matter from cells fragments. A comparison of AOM during late decline phase and natural organic matter （NOM） from Glane River （France） underlined that AOM was more fiydrophilic and presented a lower SUVA for each fractions than NOM. However, the difference between NOM and AOM hydrophobicity narrowed during decline phase.

●AIM:To study if one of the two molecules could lead to a lower number of follow up visits and intra-vitreous injection(IVI)with the same efficacy.●METHODS:ELU(or"elected"in French)study is a retrospective study conducted in real life in patients presenting suboptimal response after ranibizumab IVI(phase 1)and secondary switched to aflibercept(phase 2).The number of follow up visits and IVI were compared in both *** acuity(VA)evolution and"switching"reasons were secondary analyzed.●RESULTS:We retrospectively included data of 33 patients(38 eyes)with age-related macular degeneration(AMD;mean age:77±7.7 y).The number of monthly follow up visits[median(Q1;Q3)]:was significantly lower with aflibercept(phase 2),respectively 1.0(0.81;1.49)visits in phase 1,versus 0.79(0.67;0.86)visits in phase *** median number of monthly IVI also significantly decreased in phase 2,respectively 0.67(0.55;0.90)IVI in phase 1,versus 0.55(0.45;0.67)IVI in phase *** mean VA evolution(VA final-VA initial)was similar in both phases,(P>0.05).Whatever the reason for"switching"(loss of efficacy,tachyphylaxis,tolerance problems),there was no incidence on VA evolution over the time.●CONCLUSION:Our results show that switching from ranibizumab to aflibercept in"suboptimal"patients significantly reduce the number of follow up visits and IVI,with a comparable *** decrease in visit number could improve patients’quality of life and reduce surgical risk by reducing the number of injections.

This study reviewed programs to improve the efficiency of hospital utilization in the metropolitan area of Syracuse, New York between 1998 and 2015. It involved indicators that were largely under the control of hospitals and their nursing and administrative staffs, such as inpatient stays and post admission complications, as well as programs where there was less provider control such as inpatient admissions and readmissions. Large reductions in inpatient lengths of stay were generated by the Syracuse hospitals, contributing to a decline in the average daily adult medicine and adult surgery census of 140 patients. Reductions in post admission complications contributed to these developments. The study suggested that efforts to reduce inpatient admissions in the Syracuse hospitals had limited results. The areas hospital admission rate was conservative, but approximately 2000 resident discharges per year above that of a neighboring community. The need for reduction of hospital admissions resulted from the absence of provider or payor efforts to develop alternative resources in the community. If the experience of the Syracuse hospitals is typical, improvement of the efficiency of community health systems will require creativity and resources from providers. Perhaps more importantly, health care payors will need to assume an active role in these efforts.

Objective:In patients with head and neck squamous cell carcinoma(HNSCC),cetuximab[a monoclonal antibody targeting epidermal growth factor receptor(EGFR)]has been shown to improve overall survival when combined with radiotherapy in the locally advanced setting or with chemotherapy in first-line recurrent and/or metastatic(R/M)setting,*** biomarkers of resistance to cetuximab have been identified in metastatic colorectal cancer,no biomarkers of efficacy have been identified in ***,we aimed to identify biomarkers of cetuximab sensitivity/resistance in ***:HNSCC patients treated with cetuximab at the Curie Institute,for whom complete clinicopathological data and formalin-fixed paraffin-embedded(FFPE)tumor tissue collected before cetuximab treatment were available,were *** analyses of PTEN and EGFR were performed to assess protein expression ***3 CA and H/N/KRAS mutations were analyzed using high-resolution melting(HRM)and Sanger *** evaluated the predictive value of these alterations in terms of progression-free survival(PFS).Results:Hot spot activating PIK3 CA and KRAS/HRAS mutations were associated with poor PFS among HNSCC patients treated with cetuximab in the first-line R/M setting,but not among HNSCC patients treated with cetuximab in combination with *** of PTEN protein expression had a negative predictive value among HNSCC patients treated with cetuximab and *** EGFR expression did not predict cetuximab sensitivity in our patient ***:Hot spot activating PIK3 CA and RAS mutations predicted cetuximab resistance among HNSCC patients in the firstline R/M setting,whereas loss of PTEN protein expression predicted resistance to cetuximab when combined to radiotherapy.
