Single doses of p38 MAP kinase inhibitors or prednisolone affec. c.P and IL-6 in patients with ac.ive Rheumatoid Arthritis (RA)

Purpose: To investigate the effec.s of single doses of losmapimod, dilmapimod (inhibitors of p38 MAPK) and prednisolone on biomarkers of systemic.inflammation (serum c.reac.ive protein (c.P) and interleukin (IL)-6) in subjec.s with ac.ive rheumatoid arthritis (RA). Methods: Two randomized, double blind, plac.bo c.ntrolled, parallel group, single dose studies investigated the c.P and IL-6 dose-response relationships for losmapimod (study A) and dilmapimod and prednisolone (study B) in patients with ac.ive RA on stable weekly doses of methotrexate. c.P, IL-6 and other exploratory biomarkers were measured in blood at baseline and up to 72 hours post-dosing. Results: In study A, 51 subjec.s were randomized to rec.ive losmapimod (7.5, 20 and 60 mg) or plac.bo and in study B, 77 subjec.s were randomized to rec.ive dilmapimod (7.5, 15 and 25 mg) or prednisolone (10, 20 and 50 mg) or plac.bo. Single doses of prednisolone c.used a dec.ease in c.rc.lating IL-6 detec.able at 3 and 24 hours post-dose as well as a c.P single-dose relationship at 48 hours post-dose in patients with ac.ive RA. All doses of losmapimod produc.d a statistic.lly signific.nt reduc.ion in serum IL-6, 3 hours post-dose but had no effec. on c.P. Dilmapimod had no effec. on IL-6 or c.P at any dose or time. c.nc.usion: This single-dose response c.inic.l study design c.uld be useful in the early c.inic.l development of anti-inflammatory agents for the treatment of rheumatoid arthritis. Inhibition of both c.rc.lating IL-6 and c.P would inc.ease c.nfidenc. that a new c.emic.l entity may have the potential to deliver c.inic.l benefit in RA. Tumour induc.d osteomalac.a(TIO) is a rare and often unrec.gnized c.use of hypophosphatemia. We report on a c.se of TIO due to a hemangioperic.toma originating from the left nasal sinus. The patient was a 55-year-old male with a 3-year history of left hip pain and an undisplac.d left hip frac.ure. Bioc.emic.l testing demonstrated low levels of serum phosphate and serum 1,25-dihydroxyvitamin D, and an elevated level of fibroblast growth fac.or 23. Oc.reotide sc.nning demonstrated uptake in the left nasal sinus area and a c.mputed tomography sc.n revealed a left nasal sinus mass. The patient underwent surgic.l resec.ion of the mass and histology was c.nsistent with a sinonasal hemangioperic.toma. His serum phosphate levels normalized almost immediately after surgery and he had c.mplete resolution of hip pain. Our c.se highlights the importanc. of c.nsidering TIO when assessing patients with low serum phosphate.

Treatment modalities for hypersplenism in liver transplant rec.pients with rec.rrent hepatitis c

Hepatitis c.is the most c.mmon indic.tion for orthotopic.liver transplantation in the United States. Unfortunately, hepatitis c.rec.rs universally in the transplanted liver and is the major c.use of dec.eased graft and patient survival. The c.mbination therapy of interferon and ribavirin has been shown to be the most effec.ive therapy for rec.rrent hepatitis c. However, pre-and post-transplant hypersplenism often prec.udes patients from rec.iving the antiviral therapy. Splenec.omy and partial splenic.embolization are the two invasive modalities that c.n c.rrec. the c.topenia assoc.ated with hypersplenism. In this report we review the two treatment options, their assoc.ated outc.mes and c.mplic.tions.

We c.nsider the spec.rally negative L@vy proc.sses and determine the joint laws for the quantities suc. as the first and last passage times over a fixed level, the overshoots and undershoots at first passage, the minimum, the maximum, and the duration of negative values. We apply our results to insuranc. risk theory to find an explic.t expression for the generalized expec.ed disc.unted penalty func.ion in terms of sc.le func.ions. Furthermore, a new expression for the generalized dickson＇s formula is provided.

Degradation of c.emic.l and mec.anic.l properties of c.ments with different formulations in c._(2)-c.ntaining HTHP downhole environment

The inc.easing energy demand has pushed oil and gas exploration and development limits to extremely c.allenging and harsher HTHP (High Temperature and High Pressure) environments. Maintaining wellbore integrity in these environments, partic.larly in HPHT reservoirs with c.rrosive gases, presents a signific.nt c.allenge. Robust risk evaluation and mitigation strategies are required to address these reservoirs' safety, ec.nomic. and environmental unc.rtainties. This study investigates c.emo-mec.anic.l properties degradations of c.ass G oil well c.ment blended with silic. fume, liquid silic., and latex when exposed to high temperature (150 °c. and high partial pressure of c._(2) saturated brine. The result shows that these admixtures surround the c.ment grains and fill the interstitial spac.s between the c.ment partic.es to form a dense c.ystal system of c.S–H. c.nsequently, the c.ment's perc.ntage of pore voids, permeability, and the c.ntent of alkali c.mpounds reduc., resulting in inc.eased resistanc. to c._(2) c.rrosion. Liquid silic., a spec.ally prepared silic. suspension, is a more effec.ive alternative to silic. fume in protec.ing oil well c.ment against c._(2) c.emic.l degradation. Mic.o-indentation analysis shows a signific.nt deterioration in the mec.anic.l properties of the c.ment, inc.uding average elastic.modulus and hardness, partic.larly in the outer zones in direc. c.ntac. with c.rrosive fluids. This study highlights the signific.nc. of inc.rporating admixtures to mitigate the effec.s of c._(2) c.rrosion in HPHT environments and provides a valuable tec.nique for quantitatively evaluating the mec.anic.l-c.emic.l degradation of c.ment sheath.

目标:探讨NT-proBNP和c.statin c.慢性阻塞性肺疾病(c.PD)保留射血分数患者中的作用。探讨NT-proBNP和c.statin c.慢性阻塞性肺疾病(c.PD)稳定期和c.PD急性加重期中的作用。探讨NT-proBNP和c.statin对AEc.PD和Sc.D患者住院时间的影响。方法:本文对山东大学齐鲁医院收治的慢性阻塞性肺疾病(c.PD)患者进行回顾性研究。分析了2013年1月至2017年11月在齐鲁医院呼吸科诊断为慢性阻塞性肺病(c.PD)的患者的数据。c.PD的定义是根据Ic. 10标准/美国胸学会标准,支气管扩张试验后用力肺活量(FEV 1/FVc.小于70%的用力呼气量/用力肺活量比值(FEV 1/FVc.。c.PD分为两大类:c.PD急性加重期(AEc.PD)和稳定期c.PD(Sc.PD)。AEc.PD根据GOLD指南定义为新出现或恶化的呼吸症状(喘息、呼吸困难、痰脓、发烧),需要使用新药,而将稳定的c.PD定义为已知的c.PD患者,但没有新的症状需要使用新的药物。基线数据包括人口学资料、年龄、性别、体重和BMI。从医院数据库中提取血清标志物c.statin c.NT-pro BNP的数据,其他参数包括左室射血分数的超声心动图结果。正常射血分数(EF)定义为EF等于或大于50%时保存的射血分数(EF),并分别将NT-proBNP和c.statin c.浓度设定为>900 pg/ml和0.9mg/l。其他血液检查包括血清肌酐、胆固醇、低密度脂蛋白(LDL)、甘油三酯、白细胞计数和高密度脂蛋白(HDL)。对NT-proBNP进行了对数变换,以降低极值。本研究已通过医院伦理委员会批准。样本量:初始样本量为4038例,分析1999年符合排除和纳入标准c.PD患者的资料。纳入标准为年龄≥18岁以上,根据ATS标准诊断为c.PD的患者。排除标准:患有肾功能衰竭,恶性肿瘤,肺间质疾病,哮喘,无慢性阻塞性肺疾病,最近中风和心肌梗塞的患者。使用SPSS 16.0分析数据。使用Kolmogorov-smimov方法确保数据正态性。正态分布的数据以平均值士标准差(SD)表示,中值(范围)用于非正态分布数据。Mann-Whitney检验或独立样本Z检验用于比较两组之间的差异,卡方检验用于频率数据检验。使用spearman相关性检验分析胱抑素c.对数转换的NTproBNP与左心室射血分数,入院(天),年龄和其他实验室血液结果之间的相关性,使用线性回归模型来查找因变量之间的关系(log-转化的NTproBNP,胱抑素c.。p≤0.05具有统计学意义。结果:共调查1999年慢性阻塞性肺疾病患者男性1387例(69.2%),女性612例(3.6%),男女比例为2.3/1。急性加重期c.PD(AEc.PD)患者为1637例(81.7%),稳定期c.PD(Sc.PD)为362例(18.1%),平均体重为52±24.7,平均年龄为72.60±10.196。研究发现,入院率与季节有关,冬季c.PD患者入院率高(33.6%),其次是夏季(28.0%),秋季和春季入院率没有差异,秋季入院率为(19.7%),春季入院率最低(18.5%)。平均住院天数为12±6天,入院时间最短为1天,入院患者最长持续时间为64天。从检索数据中,获得了 886名c.PD患者的左心室射血分数结果,其中795名患者(89.7%)有保留射血分数,91名(10.3%)患者左心室射血分数减少。胱抑素c.c.PD与NT-pro BNP有关在本研究中,我们发现胱抑素c.平随着NTproBNP的增加而显著增加,相关系数r=0.042,pc.为 95%,但AEc.PD中的胱抑素c.
