Virtual chromoendoscopy in small bowel capsule endoscopy: New light or a cast of shadow?

AIM: To evaluate whether virtual chromoendoscopy can improve the delineation of small bowel lesions previously detected by conventional white light small bowel capsule endoscopy(sBCE). METHODs: Retrospective single center study. One hundred lesions selected from forty-nine consecutive conventional white light sBCE(sBCE-WL) examinations were included. Lesions were reviewed at three Flexible spectral Imaging Color Enhancement(FICE) settings and Blue Filter(BF) by two gastroenterologists with ex-perience in sBCE, blinded to each other's findings, whoranked the quality of delineation as better, equivalent or worse than conventional sBCE-WL. Inter-observer percentage of agreement was determined and analyzed with Fleiss Kappa(k) coefficient. Lesions selected for the study included angioectasias(n = 39), ulcers/ero-sions(n = 49) and villous edema/atrophy(n = 12). REsULTs: Overall, the delineation of lesions was im-proved in 77% of cases with FICE 1, 74% with FICE 2, 41% with FICE 3 and 39% with the BF, with a percent-age of agreement between investigators of 89%(k = 0.833), 85%(k = 0.764), 66%(k = 0.486) and 79%(k = 0.593), respectively. FICE 1 improved the delineation of 97.4% of angioectasias, 63.3% of ulcers/erosions and 66.7% of villous edema/atrophy with a percentage of agreement of 97.4%(k = 0.910), 81.6%(k = 0.714) and 91.7%(k = 0.815), respectively. FICE 2 improved the delineation of 97.4% of angioectasias, 57.1% of ulcers/erosions and 66.7% of villous edema/atrophy, with a percentage of agreement of 89.7%(k = 0.802), 79,6%(k = 0.703) and 91.7%(k = 0.815), respectively. FICE 3 improved the delineation of 46.2% of angioecta-sias, 24.5% of ulcers/erosions and none of the cases of villous edema/atrophy, with a percentage of agreement of 53.8% [k = not available(NA)], 75.5%(k = NA) and 66.7%(k = 0.304), respectively. PillCam~ sB3 capsule: Does the increased frame rate eliminate the risk of missing lesions?

since its emergence in 2000, small bowel capsule endoscopy(sBCE) has assumed a pivotal role as an investigation method for small bowel diseases. The PillCam sB2-ex offers 12 h of battery time, 4 more than the previous version(sB2). Rahman et al recently found that the PillCam sB2-ex has a significantly increased completion rate, although without higher diagnostic yield, compared with the sB2. We would like to discuss these somewhat surprising results and the new potentialities of the PillCam sB3 regarding the diagnostic yield of small bowel studies. PillCam sB3 offers improved image resolution and faster adaptable frame rate over previous versions of sBCE. We recently compared the major duodenal papilla detection rate obtained with PillCam sB3 and sB2 as a surrogate indicator of diagnostic yield in the proximal small bowel. The PillCam sB3 had a significantly higher major duodenal papilla detection rate than the PillCam sB2(42.7% vs 24%, P = 0.015). Thus, the most recent version of the PillCam capsule, sB3, may increase diagnostic yield, particularly in the proximal segments of the small bowel.

Role of Cognitive Behavioral Therapy in Fibromyalgia: A systematic Review

A literature search of articles from 2002-2019 was performed using Medline, Embase, Cochrane, LILACs, IBECs, CRD, and Epistemonikos databases, to analyze the effects of Cognitive-Behavioral Therapy (CBT) in the treatment of fibromyalgia. Twenty-seven articles were selected in which CBT was performed exclusively by specialist physicians, associated or not with conventional pharmacological treatment and/or physical exercise. In most articles, CBT worked with self-knowledge and cognitive restructuring, attempting to reduce pain perception, and it showed a general improvement in daily activities by decreasing patient’s limitations, such as morning stiffness. The literature showed significant correlations of CBT in pain processing over time. When CBT was compared to conventional pharmacological therapy, a certain superiority of CBT could be observed concerning the quality of life, catastrophizing, and acceptance of pain. However, when they were simultaneously applied, this improvement in quality of life was not observed.

The CBM （chromoblastomycosis） is a disease caused by dematiaceous fungi, which has the species Fonsecaea pedrosoi as main agent. This fungus is found in warm and moist climates, characteristically found in the amazonic lands of the region, where its environmental isolation was once described. This research aimed to identify the healthy population exposed to agent F. pedrosoi in four villages located in the Legal Amazon. In order to clarify the risk factors for allergic immune exposure, a survey was conducted by the technique of delayed skin reaction （IDR） with the metabolic antigen （chromomycin） in 449 healthy individuals. The results showed that 14.9% （67 subjects） were IDR positive, with induration 〉 5 mm. strong>Introduction:strong> Post-hospital syndrome (PHs) is defined as a period of vulnerability during the first 30 days after a patient is discharged from hospital, in which multiple factors come into play. Hyponatremia is the most frequent hydroelectrolytic disorder in hospitalized patients and may be related to the appearance of PHs. strong>Objective: strong>The objective is to estimate the prevalence of PHs that is assessed as the rate of readmissions in the first 30 days after discharge, in patients with hyponatremia. strong>Material and Methods:strong> It is a descriptive observational study of patients with hyponatremia who were discharged from 1 september 2010 to 2 February 2020 at the Internal Medicine service of the Hospital University of san Juan (Alicante, spain). strong>Results:strong> Of the 25 included patients, 5 (20%) were readmitted within a month of discharge, after a mean of 11.4 days (standard deviation [sD] 5.1). The overall mortality of the study was 20% (n = 5), with one case of death in the first 30 days post-hospitalization (4%). In 12 patients (48%) the origin of the hyponatremia was undetermined. The most frequently recorded etiology for the condition was pharmacological (n = 7, 28%), and there was pronounced variability in its clinical and laboratory study. The most widely used corrective measure was drug withdrawal, in 16 patients (64%). Water intake restriction was the most common treatment after discharge (5 patients, 20%), followed by urea (2 patients, 8%), while tolvaptan was not used. strong>Conclusion: strong>Hyponatremia may be the cause of PHs, which could increase the rate of early readmission. Hyponatremia is an underdiagnosed and undertreated entity, so it is necessary to apply an appropriate system to optimize its management and, in future studies, to assess its impact on PHs.

Laurel forests are quite relevant for biodiversity conservation and are among the island ecosystems most severely damaged by human *** the past,Canary laurel forests have been greatly altered by logging,livestock and *** remains of laurel forests are currently protected in the Canary Islands(spain).However,we miss basic information needed for their restoration and adaptive management,such as tree longevity,growth potential and responsiveness to natural and anthropogenic *** dendrochronological methods,we studied how forest dynamic is related to land-use change and windstorms in two well-preserved laurel forests on Tenerife *** cores were collected from over 80 trees per stand at three stands per *** used ring-width series to estimate tree ages and calculate annual basal area increments(BAI),cumulative diameter increases,and changes indicative of released and suppressed *** tree species were found in all stands,with Laurus novocanariensis,Ilex canariensis and Morella faya being the most common *** some individuals were over 100 years old,61.8%-88.9% of the trees per stand established between 1940 and 1970,coinciding with a post-war period of land abandonment,rural exodus and the onset of a tourism *** trees have shown growth rates larger than 1 cm diameter per year and most species have had increasing BAI trends over the past *** growth releases occurred after windstorms at both sites,but the effects of windstorms were site-dependent,with the 1958 storm affecting mainly the eastern tip of the island(Anaga massif)and the 1991 storm the western tip(Teno massif).Given the great ability of laurel forest trees to establish after land use cessation and to increase growth after local disturbances such as windstorms,passive restoration may be sufficient to regenerate this habitat in currently degraded areas.

Characterization of oral yeasts isolated from healthy individuals attended in different Colombian dental clinics

The aim of this study was to identify the most frequent yeasts in the oral cavity of adult individuals without immune disorders and to associate the presence of these oral yeasts with different characteristics of each individual. Oral rinse samples were obtained from 96 healthy adults and cultured in sabouraud dextrose agar media and CHROMagar. Yeasts were identified by sequencing the D1/D2 region of the 28 s rRNA gene. Probable association among the socio-demographic characteristics, body mass index, family and personal medical history,oral hygiene, tobacco and/or alcohol consumption habits and presence of oral fungi was analyzed. Contingency tables and logistic regression were employed to evaluate possible relationships between the presence of oral fungi and mixed colonization with these variables. 57.3% of the healthy individuals had oral yeasts and 21.8% had mixed colonization. The most prevalent yeasts were Candida albicans(52%), C. parapsilosis(17.9%), and ***(7.57%). Yeasts with most frequently mixed colonization were C. albicans and C. parapsilosis. No relationships were found among the variables analyzed. However, the presence of mixed colonization was related to the presence of dental prostheses(Pses(P=0.016) and O’Leary index(P=0.012). This is the first study that characterized oral yeasts in Colombian healthy individuals, determined the most prevalent oral yeasts C. albicans, C. parapsilosis and C. dublinensis and an association of mixed colonization with the use of dental prostheses and aparatology and poor hygiene.
