Evaluation of participants' feedback after a simulation-based training in neonatal resuscitation using a realistic delivery room

BACKGROUND: Low-frequency and high-risk situations, such as neonatal resuscitation, are the ideal targets for simulation-based learning. The aim of this paper is to present the structure of our internal neonatal resuscitation training program, using a realistic, simulated delivery room, and to present the participants' opinions about teamwork, emotional stress, and their subjective ability to face a resuscitation. METHODS: We administered a training course to 24 doctors and midwives. One of the simulation classrooms was modified to appear similar to a real delivery room. Four scenarios were conducted using a previously designed checklist of primary and secondary goals. Upon completion, all students participated in a debriefing session with the help of a video review. RESULTS: Students rated the achievement of their previously defined goals on a scale of 1 to 5. Grouping together the percentages of the highest ratings (Categories 4 and 5), 83.4% (20/24) of the students considered the course useful for acquiring clinical skills. For 87.5% (21/24) of the students, the scenarios simulated real clinical situations, the room properly simulated a real delivery room, and the course improved the students' ability to work in a team. For 66.6% (16/24) of the students, the course improved their stress in confronting neonatal resuscitation. Initially, only 33.3% (8/24) of the students considered themselves very capable or fully able to cope with a resuscitation. After the course, that percentage rose to 62.5% (15/24). CONCLUSIONS: The incorporation of simulation-based learning into neonatal resuscitation teaching programs, using realistic scenarios, is useful and offers the possibility of acquiring technical skills, but it also allows for the improvement of teamwork and the adoption of different roles and positive attitudes towards emotional stress.

Synthesis and Characterization of a Polymeric Material Blended to Bone Forming Elements

A synthetizing material blended with two distinct proteins (collagen and casein) and mineral mixture, was developed in order to evaluate their properties suitable for possible applications in the biomedical such as inducing the regeneration of damaged bone, either due to an accident or illness. Samples were evaluated by 1) Mechanical properties tests under the bending, 2) Scanning electronic microscopy and 3) Infrared spectroscopy were carried out. The results showed that the developed material has breaking strength and structure characteristics associated with the protein used in their composition. This fact suggests that the used protein determines the resistance of the material, in such a way according to the required use, being able to choose appropriate strength and duration either short or long time. The material composition for specific use, in order to find the most suitable mixture for bone replacement, or induce bone recovery, according to the required properties similar to those of damaged living tissue.

Effect of Chia Seed on Physicochemical and Sensory Characteristics of Common Carp Restructured as Functional Food

Physicochemical and sensory characteristics of restructured meat of common carp （Cyprinus carpio） fortified with 0-8 g/100 g of chia seed flour （CSF） was evaluated. It had a higher nutritional value （higher fibre content and protein retention] （p 〈 0.05） and better cooking characteristics （higher cooking yield and moisture retention）（p 〈 0.05） than the control. The colour （a＊, b＊） increased; lightness and whiteness index decrease （p 〈 0.05）. Hardness increase （p 〈 0.05） occurred because of CSF addition. Differential scanning calorimetry showed that fibre fortification did not interfere with the thermal transitions of the restructured meat. No significant differences were detected with the preference test scores of 4% or 8% CSF compared with the control. Restructured （4%-8% CSF） had a higher content of fibre and fat, which could be linoleic and linolenic acid, and an increase in the content of protein compared with those of commercial products, among had 1.62 and 2.25 mg AGE/g. Therefore, the restructured properties of common carp were governed by CSF addition.

Effects of Microselective Neurotomy on Focal Spasticity and Description of the Surgical Technique

We studied the microselective neurotomy (MSN) and its advantages to alleviate disabling spasticity. The best indication for MSN is focal spasticity. We performed 298 MSN, 87 nerves on the upper limb, 211 nerves on the lower limb) in 68 patients. The initial causes were: stroke, cranial trauma, postoperative complications and multiple sclerosis. MSN procedures were performed on the median nerve (n = 40) for wrist and finger flexion;musculo-cutaneus nerve (n = 38) for elbow flexion;ulnaris nerve (n = 9) for cubital deviation of the hand;gastrocnemius nerve (n = 98) and soleus nerve (n = 49) for equinus foot, tibial posterior nerve (n = 45) for varus foot, and fascicles (or bundles) of the flexor digitorum for “claw” toes (n = 19). The main preoperative test to identify the responsible nerve was a neuromuscular block with local anesthesia (lidocaine or bupivacaine) injected into the site of the nerve connecting the spastic muscle. During surgery, the identified nerve was exposed and its epineurium opened. Nerve bundles were teased apart into individual rootlets and a number of rootlets cut were previously planned, according to the spasticity. Follow-up was performed for up to 10 years, with a mean period of 29 months. Results demonstrated a reduction of limb spasticity of 2 to 3 points: modified Ashworth scale (MAS). Pain and clonus were also diminished in the affected limb. In some cases, voluntary movement was once again possible. MSN is a useful alternative in those cases of focal spasticity where physiotherapy and nerve block with botulinum toxin or phenol no longer produce satisfactory results.

The principal factor to determine the economical value of the products manufactured in the electronics industry is due to the productive yielding. This is important for the cost of the articles fabricated in this type of industrial plants installed in Mexicali city, where around 80% of companies are, and which fabricate electronic devices and systems, or have industrial electronic systems and machines to their manufacturing process. Mexicalicity is located in theBaja CaliforniaStateof the northwest ofMexico, which is a border city with Calexico in theCaliforniaStateof the United States of America (USA). The region located in Mexicali, is a desert area. Geothermal plant is located in this area, which is an important industry and supplies electricity to this city and its valleys and some cities on southwest of United States for daily activities. This company emits hydrogen sulfide (H2S) as a main air pollutant that reacts with oxygen in the atmosphere, generating sulfur oxides (SOX). This chemical is dispersed to the city of Mexicali in which industrial plants are located with electronic control systems, and penetrates to indoor rooms. Those cause the corrosion process. The presence of corrosion leads to the deterioration of electrical connectors, the connections of electronic systems and the decreasing of the lifetime of these control systems. Other air pollutants that are considered as chemical agents which cause damage to materials used in the electronics industry, are the sulfurs and nitrogen oxides (NOX), emitted from the traffic vehicle and some industries. This causes the low productive yielding of electrical and electronic devices and systems used in the companies of this city, and is a major concern to specialized people, managers and owners. Resectability of hepatocellular carcinoma in patients with chronic liver disease is dramatically limited by the need to preserve sufficient remnant liver in order to avoid postoperative liver insufficiency. Preoperative treatments aimed at downsizing the tumor and promoting hypertrophy of the future remnant liver may improve resectability and reduce operative morbidity. Here we report the case of a patient with a large hepatocellular carcinoma arising from chronic liver disease. Preoperative treatment, including tumor downsizing with transarterial radioembolization and induction of future remnant liver hypertrophy with right portal vein embolization, resulted in a 53% reduction in tumor volume and compensatory hypertrophy in the contralateral liver. The patient subsequently underwent extended right hepatectomy with no postoperativesigns of liver decompensation. Pathological examination demonstrated a margin-free resection and major tumor response. This new therapeutic sequence, combining efficient tumor targeting and subsequent portal vein embolization, could improve the feasibility and safety of major liver resection for hepatocellular carcinoma in patients with liver injury.

Laurel forests are quite relevant for biodiversity conservation and are among the island ecosystems most severely damaged by human *** the past,Canary laurel forests have been greatly altered by logging,livestock and *** remains of laurel forests are currently protected in the Canary Islands(Spain).However,we miss basic information needed for their restoration and adaptive management,such as tree longevity,growth potential and responsiveness to natural and anthropogenic *** dendrochronological methods,we studied how forest dynamic is related to land-use change and windstorms in two well-preserved laurel forests on Tenerife *** cores were collected from over 80 trees per stand at three stands per *** used ring-width series to estimate tree ages and calculate annual basal area increments(BAI),cumulative diameter increases,and changes indicative of released and suppressed *** tree species were found in all stands,with Laurus novocanariensis,Ilex canariensis and Morella faya being the most common *** some individuals were over 100 years old,61.8%-88.9% of the trees per stand established between 1940 and 1970,coinciding with a post-war period of land abandonment,rural exodus and the onset of a tourism *** trees have shown growth rates larger than 1 cm diameter per year and most species have had increasing BAI trends over the past *** growth releases occurred after windstorms at both sites,but the effects of windstorms were site-dependent,with the 1958 storm affecting mainly the eastern tip of the island(Anaga massif)and the 1991 storm the western tip(Teno massif).Given the great ability of laurel forest trees to establish after land use cessation and to increase growth after local disturbances such as windstorms,passive restoration may be sufficient to regenerate this habitat in currently degraded areas.
