Over the years,our growing experience with endoscopic submucosal dissection along with technological advances has so-lidiﬁed our comfort and knowledge on working in the submucosa,also referred to as the“third space.”Per-oral endoscopic myotomy(POEM)was theﬁrst prototype third-space endoscopy(TSE)procedure,demonstrating the feasibility and clinical utility of endoscopic esophagogastric myotomy via submucosal *** launch of POEM accelerated the evolution of TSE from a vanguard concept to an expandingﬁeld with a wide range of clinical *** this review,we discuss the status and future directions of multiple TSE interventions.

The wave of global financial crises(2008-2009)caused a surge in the capital flows of developed countries particularly,between developed and developing *** crunch hit all financial sectors with unanticipated *** study evaluates the role of a country’s political practices in moderating the impact of global financial crunch on microfinance *** the fixed effect panel regression method on the dataset comprising of 95 MFIs operating in South Asia from 2003 to 2012,we determine that microfinance operational capability shares a positive relationship with the institutional attributes of a country and our output reveals that impact of country’s political practices is pervasive on the financial output of MFIs,liable to different levels of *** findings further reveals that MFIs situated in countries having vigorous political practices are less severely affected by the economic crunch.

Brain tumors are considered as most fatal *** reduce the risk of death,early identification of the disease is *** of the best available methods to evaluate brain tumors is Magnetic resonance Images(MRI).Brain tumor detection and segmentation are tough as brain tumors may vary in size,shape,and *** makes manual detection of brain tumors by exploring MRI a tedious job for radiologists and doctors’.So an automated brain tumor detection and segmentation is *** work suggests a Region-based Convolution Neural Network(RCNN)approach for automated brain tumor identification and segmentation using MR images,which helps solve the difficulties of brain tumor identification efficiently and *** methodology is based on the accurate and efficient selection of tumorous *** reduces computational complexity and *** have validated the designed experimental setup on a standard dataset,BraTS *** used binary evaluation matrices based on Dice Similarity Coefficient(DSC)and Mean Average Precision(mAP).The segmentation results are compared with state-of-the-art methodologies to demonstrate the effectiveness of the proposed *** suggested approach attained an averageDSC of 0.92 andmAP 0.92 for 10 patients,while on the whole dataset,the scores are DSC 0.89 and mAP *** following results clearly show the performance efficiency of the proposed methodology.

This study aimed to explore the lived experiences of university teachers who participated in online teaching for the first time during the COVID-19 pandemic.A qualitative method of data collection and inductive thematic analysis were *** data were collected through a semistructured interview from 11 faculty members who teach in various universities in *** analysis presented five themes of the online teaching experience:culture and gender-related issues,teaching effectiveness,challenges in online teaching,coping strategies,and faculty's'post-COVID-19 *** findings reflect that the faculty prioritized to focus on immediate online instructional matters in the wake of the pandemic with a lack of emphasis on global practices for online *** and young faculty reported disparities in striking a balance between pedagogy and ***,despite the limitations,the experience itself prepared the faculty for a blended learning approach and increased their awareness of global and future *** implications were reported based on the findings.

Knowledge and practices of primary care physicians on the current referral system of diabetic retinopathy in Islamabad and Rawal-Pindi, Pakistan

AIM: To assess the current knowledge and practices in diabetic eye care and referral system regarding diabetic retinopathy(DR) in health centers of Islamabad and RawalPindi. METHODS: A cross-sectional study was carried out in 4 government and private health centers in RawalpindiIslamabad from May 2018 to Oct. 2018. A total of 38 Primary Care Physicians(general practitioners, family physicians, and internists) were recruited out of which data for 2 were either not returned, or were missing partially. Data were collected through a 27-item consented & validated, multiple-choice questionnaire based on physician characteristics, knowledge and practice of diabetic eye care and challenges faced due current DR referral system. Descriptive analyses for all variables were performed including, mean and standard deviation. Analytical analyses were also conducted to study association between different study variables. RESULTS: Mean scores of knowledge for general practitioners, family physicians, and internists were 41.7%, 42.0% and 46.6% respectively. A lack of knowledge, and suboptimal practices were observed regarding signs, symptoms, screening, testing, evaluation and referral of DR regardless of physicians’ specialty, or years in practice. Lack of expertise regarding direct ophthalmoscopy, interpretation of findings, and referral to an ophthalmologist were noted. Physicians who performed consultation and counselling according to patients’ needs referred more patients to an ophthalmologist than those who restricted their consultationto a fixed amount of time and had more patients per unit time(P=0.01). Physicians who had taken care of less than 5 number of patients with DR marked less incorrect answers with no significantly greater number or correct answers compared to physicians who had taken care of more than 5 number of patients with DR(P=0.044).
