BACKGROunD Post-transplant lymphoproliferative disorder(PTLD) is a fatal complication of solid organ transplantation or allogenic hematopoietic stem cell transplantation that is associated with immunosuppressive therapy. Potential manifestations are diverse, ranging from reactive lymphoid hyperplasia to high-grade *** is usually of B-cell origin and associated with Epstein-Barr virus(EBV)infection. Herein, we describe a case of PTLD involving the peritoneal *** has been only case of PTLD as a diffuse large B-cell lymphoma(DLBCL) in the *** SUMMARY The patient was a 62-year-old man who had been receiving immunosuppressive therapy with tacrolimus since undergoing a liver transplant 15 years prior. He reported that he had experienced abdominal discomfort and anorexia 1 month prior to the current admission. Abdominal pelvic computed tomography(CT)revealed peritoneal and omental mass-like lesions without bowel ***-guided biopsy was performed, and he was histologically diagnosed with EBV-negative DLBCL. Positron emission tomography(PET)-CT depicted peritoneum and omentum involvement only, without any lymphadenopathy or organ masses, including in the gastrointestinal tract. Six cycles of chemotherapy with a "R-CHOP" regimen(rituximab-cyclophosphamide, doxorubicin, vincristine, prednisolone) were administered,and PET-CT performed thereafter indicated complete *** This is the first report of isolated peritoneal lymphomatosis defined as PTLD in a liver transplant recipient.

BACKGROunD Research exploring the influence of healthier lifestyle modification(LSM)on the risk of hepatocellular carcinoma(HCC)in patients with chronic hepatitis B(CHB)is *** To emulate a target trial to determine the effect of LSM on HCC incidence and mortality among patients with CHB by large-scale population-based observational *** Among the patients with CHB enrolled in the Korean National Health Insurance Service between January 1,2009,and December 31,2017,those aged≥20 years who drank alcohol,smoked cigarettes,and were sedentary were *** included at least one LSM,including alcohol abstinence,smoking cessation,and regular *** primary outcome was HCC development,and the secondary outcome was liver-related *** used 2:1 propensity score matching to account for *** With 48766 patients in the LSM group and 103560 in the control group,the adjusted hazard ratio(HR)for incident HCC and liver-related mortality was 0.92[95%confidence interval(CI):0.87-0.96]and 0.92(95%CI:0.86-0.99)in the LSM group,respectively,compared with the control *** the LSM group,the adjusted HR(95%CI)for incident HCC was 0.84(0.76-0.94),0.87(0.81-0.94),and 1.08(1.00-1.16)for alcohol abstinence,smoking cessation,and regular exercise,*** adjusted HR(95%CI)for liver-related mortality was 0.92(0.80-1.06),0.81(0.72-0.91),and 1.15(1.04-1.27)for alcohol abstinence,smoking cessation,and regular exercise,*** LSM lowered the risk of HCC and mortality in patients with ***,active LSM,particularly alcohol abstinence and smoking cessation,should be encouraged in patients with CHB.

Bound states in the continuum(BICs)and Fano resonances in planar photonic lattices,including metasurfaces and photonic-crystal slabs,have been studied extensively in recent ***,the BICs and Fano resonances are associated with the second stop bands open at the second-order Γ *** paper addresses the fundamental properties of the fourth stop band accompanied by BICs at the third-order Γ point in one-dimensional leakymode photonic *** the fourth stop band,one band edge mode suffers radiation loss,thereby generating a Fano resonance,while the other band edge mode becomes a nonleaky *** fourth stop band is controlled primarily by the Bragg processes associated with the first,second,and fourth Fourier harmonic components of the periodic dielectric constant *** interplay between these three major processes closes the fourth band gap and induces a band flip whereby the leaky and BIC edges transit across the fourth band *** the fourth stop band,an accidental BIC is formed owing to the destructive interplay between the first and second Fourier *** the fourth band gap closes with strongly enhanced radiative Q factors,Dirac cone dispersions can appear at the third-order Γ point.

Risk factors of electrocoagulation syndrome after esophageal endoscopic submucosal dissection

AIM To investigate post endoscopic submucosal dissection electrocoagulation syndrome(PEECS) of the *** We analyzed 55 consecutive cases with esophageal endoscopic submucosal dissection for superficial esophageal squamous neoplasms at a tertiary referral hospital in South Korea. Esophageal PEECS was defined as "mild" meeting one of the following criteria without any obvious perforation: fever(≥ 37.8 ℃), leukocytosis(> 10800 cells/μl), or regional chest pain more than 5/10 points as rated on a numeric pain intensity scale. The grade of PEECS was determined as "severe" when meet two or more of above *** We included 51 cases without obvious complications in the analysis. The incidence of mild and severe esophageal PEECS was 47.1% and 17.6%, respectively. Risk factor analysis revealed that resected area, procedure time, and muscle layer exposure were significantly associated with PEECS. In multivariate analysis, a resected area larger than 6.0 cm^2(OR = 4.995, 95%CI: 1.110-22.489, P = 0.036) and muscle layer exposure(OR = 5.661, 95%CI: 1.422-22.534, P = 0.014) were independent predictors of esophageal PEECS. All patients with PEECS had favorable outcomes with conservative management approaches, such as intravenous hydration or *** Clinicians should consider the possibility of esophageal PEECS when the resected area exceeds 6.0 cm^2 or when the muscle layer exposure is noted.

The clustered regularly interspaced short palindromic repeats(CRISPRJ/associated protein 9(CRISPR/Cas9)gene editing technology,as a revolutionary breakthrough in genetic engineering,offers a promising platform to improve the treatment of various genetic and infectious diseases because of its simple design and powerful ability to edit different loci ***,failure to conduct precise gene editing in specific tissues or cells within a certain time may result in undesirable consequences,such as serious off-target effects,representing a critical challenge for the clinical translation of the ***,some emerging strategies using genetic regulation,chemical and physical strategies to regulate the activity of CRISPR/Cas9 have shown promising results in the improvement of spatiotemporal ***,in this review,we first summarize the latest progress of these advanced strategies involving cell-specific promoters,small-molecule activation and inhibition,bioresponsive delivery carriers,and optical/thermal/ultrasonic/magnetic ***,we highlight the advantages and disadvantages of various strategies and discuss their obstacles and limitations in clinical ***,we propose viewpoints on directions that can be explored to further improve the spatiotemporal operability of CRISPR/Cas9.

Freezing and thawing effects of sand-silt mixtures on elastic waves

Freezing and thawing during the winter season change soil properties such as density. The density change in the particulate media influences soil stiffness. In addition, freezing of partially or fully saturated soils changes the soil matrix from a particulate media to a continuum. The goal of this study is to investigate the cyclic freezing and thawing effects on elastic waves. Sand-silt mixtures with 10% silt fraction in weight and 40% saturation are prepared. The sand-silt mixtures are placed in a nylon cell, onto which a pair of bender elements and a pair of piezoelectric disk elements are installed for the measurement of shear and compressional waves, respectively. The temperature of the mixtures decreases from 20 ℃ to -10 ℃ to freezing. The frozen sample is gradually thawed at room temperature （20 ℃）, These freezing-thawing processes are repeated three times. The test result shows that the shear and compressional wave velocities significantly increase when the specimen is frozen. When the temperature is greater than 0 ℃, the elastic wave velocities are lower during thawing than during freezing due to soil structure change. This study demonstrates that soil strucre change during the winter season may be effectively estimated from elastic waves.

Thermodynamic Assessment of UO<sub>2</sub>Pellet Oxidation in Mixture Atmospheres under Spent Fuel Pool Accident

For an analysis of the oxidation behavior of UO2 nuclear fuel pellet under a loss of water coolant accident in a spent nuclear fuel pool of an LWR, thermodynamic assessments of UO2 oxidation were carried out under various atmospheric conditions. In a steam atmosphere, it was assessed that UO2 would not be fully oxidized into U3O8 due to the relatively lower oxygen partial pressure, while UO2 will be fully oxidized into U3O8 in an air atmosphere. In an air and steam mixture atmosphere, the UO2 oxidation was dominantly affected by the air volumetric fraction, because of the relatively higher oxygen partial pressure of air. In addition, the effect of H2 volumetric fraction on the oxygen partial pressure under a mixture atmosphere was calculated, and it was revealed that UO2 pellet oxidation could be reduced above the critical value of H2 volumetric fraction.
