Prevention of Deep Vein Thrombosis by Panax Notoginseng Saponins Combined with Low-Molecular-Weight heparin in Surgical Patients

Objective:To evaluate the efficacy of deep vein thrombosis(DVT)prevention among realworld surgical inpatients who received panax notoginseng saponins(PNS)combined with low-molecularweight heparin(LMWh).Methods:A prospective cohort study was conducted among surgical patients between January 2016 and November 2018 in Xuanwu hospital,Capital Medical University,Beijing,*** received LMWh alone or PNS combined with LMWh for preventing *** primary outcome was incidence of lower extremity DVT,which was screened once a *** in the LMWh group were given LMWh(enoxaparin)via hypodermic injection,4000-8000 AxalU once *** in the exposure group received PNS(Xuesaitong oral tablets,100 mg,3 times daily)combined with LMWh given the same as LMWh ***:Of the 325 patients screened for the study,281 participants were included in the final *** cohort was divided into PNS+LMWh group and LMWh group with 134 and 147 participants,*** was a significant difference of DVT incidence between two groups(P=0.01),with 21(15.7%)incident DVT in the PNS+LMWh group,and 41(27.9%)incident DVT in the LMWh *** with participants without DVT,the participants diagnosed with DVT were older and had higher D-dimer *** multivariate logistic regression moclel showed a significant lower risk of incident DVT among participants in the PNS+LMWh group compared with the LMWh group(odds ratio 0.46,95%confidence interval,0.25-0.86).There were no significant differences in thromboelaslography values(including R,K,Angle,and MA)and differences in severe bleeding between two *** symptomatic pulmonary embolism occurred during the ***:Combined application of PNS and LMWh can effectively reduce the incidence of DVT among surgical inpatients compared with LMWh monotherapy,without increased risk of bleeding.

Application of plant DNA metabarcoding of lake sediments for monitoring vegetation compositions on the Tibetan Plateau

Benefiting from the rapid development of environmental DNA(eDNA) technologies, sedimentary DNA(sedDNA)emerges as a promising tool for monitoring plant compositions in remote regions. The Tibetan Plateau(TP), renowned for its harsh environment and numerous ponds and lakes, presents a potentially demanding region for the application of sedDNA on vegetation investigations. here, we used the g and h universal primers for the P6 loop region of the chloroplast trn L(UAA)intron to amplify plant DNA in surface sediments from 59 ponds and small lakes on the southwestern TP. The applicability and limitations of using plant DNA metabarcoding for modern vegetation monitoring and palaeo-vegetation reconstructions have been assessed by comparing sedDNA, pollen, and vegetation survey data. Our results showed that plant DNA metabarcoding recorded 186 terrestrial taxa, of which 30.1% can be identified at the species level. The plant sedDNA approach can effectively disclose the dominant plant taxa(including Asteraceae, Cyperaceae and Poaceae) and significant vegetation assemblages in the vicinity of the investigated sites. The number of taxa and taxonomic resolution of plant sedDNA exceeded that of pollen analysis(75 taxa detected, 5.3% can be identified at species level). Unlike pollen that retains a broad spectrum of regional plant signals(including Pinus and Artemisia), plant sedDNA mirrors very local plants, underscoring its utility in local vegetation monitoring and reconstructions. To conclude, plant DNA metabarcoding of(small) lake sediments warrant increased attention in the future for local vegetation monitoring and reconstructions on the TP.

Estimation of Population-Size Changes and hIV Prevalence among Female Sex Workers from 2006 to 2009 in Kaiyuan,Yunnan,China

Objective This study is to estimate the population size and prevalence of hIV and herpes simplex virus type 2 （hSV-2） infections among female sex workers （FSWs） in Kaiyuan, Yunnan Province, China. Methods Eight cross-sectional studies were conducted among FSWs in Kaiyuan from 2006 to 2009. Based on the data from two study time points each year, the total numbers of FSWs and hIV positive FSWs were estimated by using the capture-recapture technique （CR）. Results Estimated sizes of FSWs in Kaiyuan were 1 629, 1 672, 1 186, and 1 014 in the respective years from 2006 to 2009. Although the crude prevalence rates of hIV and hSV-2 varied over time, the adjusted prevalence among this population was relatively stable at 10%-12% and the adjusted hSV-2 prevalence ranged from 67% to72%. Conclusion The reason for the slight decrease of the size of the FSW population is unknown. The adjusted prevalence rates of hIV and hSV-2 among them were stable over the course of this study.

Estimation of Probability of Unprotected heterosexual Vaginal Transmission of hIV-1 from Clients to Female Sex Workers in Kaiyuan,Yunnan Province, China

Objective To estimate the probability of unprotected heterosexual vaginal transmission of hIV-1 from clients to Female Sex Workers （FSWs） in Kaiyuan County, Yunnan province, China, and analyze factors which impact the transmission probability. Methods An open cohort research of FSWs in Kaiyuan was created from surveys conducted in April 2008, October 2008, and April 2009. Two cross-sectional surveys of local clients were also carried out in May 2008 and November 2008. A model was developed to estimate the probability of unprotected heterosexual vaginal transmission of hIV-1 from clients to FSWs. Results The transmission probability from clients to FSWs was estimated as 0.0023 [95% CI 0.0014-0.0032] per unprotected heterosexual act. Conclusion The transmission probability among this group engaging in commercial sex in Yunnan province differs from that found in studies from other countries, and future studies should estimate the influence factors of hIV-1 transmission probability, such as stage of hIV infection, co-morbid sexually transmitted infections （STIs）.

得益于环境DNA技术的快速发展,沉积DNA(sedDNA)正在成为偏远地区植被监测和调查的便捷方法,并具有较大的发展潜力.青藏高原气候环境条件恶劣,同时小型湖泊众多,利用sedDNA进行植被调查不仅有助于了解高原现代植被状况,也是进行高原古植被重建的潜在需求.本研究以中国青藏高原西南部59个泡子/湖泊表层沉积物为研究对象,使用通用植物引物g-h扩增叶绿体trn L(UAA)内含子P6环区,获取了sedDNA中的植物组成信息,并将其与野外植被调查记录和沉积物花粉数据进行对比分析,探讨了基于沉积物的植物DNA宏条形码技术在青藏高原地区现代植被监测和古植被重建中的适用性和局限性.研究结果表明,植物DNA宏条形码信号共记录了186个陆生植物类群,其中30.1%的序列可以鉴定到种水平.植物sedDNA方法可以揭示调查地点周围的主要植物类群(包括菊科、莎草科和禾本科)以及重要植被组合.植物sedDNA的类群数量和分类学分辨率均超过花粉分析(75个类群,其中5.3%的类群可以鉴定到种水平).与花粉保留大量的区域植物信号(包括松属和蒿属)不同,植物sedDNA反映了高度局地的植物信号,凸显了植物sedDNA在局地植被监测和重建中的重要性.综上所述,(小型)湖泊植物sedDNA宏条形码研究在未来的青藏高原区域植被监测和重建中值得更多的关注.

Resilience, the 6th Vital Sign: Conceptualizing, Contextualizing, and Operationalizing All Six Vital Signs

There are five vital signs that healthcare providers assess: temperature, pulse, respiration, blood pressure, and pain. Normal levels for the five vital signs are published by the American heart Association, and other specialty organizations, however, the sixth vital sign (resilience) which adopts the measure of immune resilience is suggested in this paper. Resilience is the ability of the immune system to respond to attacks and defend effectively against infections and inflammatory stressors, and psychological resilience is the capacity to resist, adapt, recover, thrive, and grow from a challenge or a stressor. Individuals with better optimal immune resilience had better health outcomes than those with minimal immune resilience. The purpose of this paper is to conceptualize, contextualize, and operationalize all six vital signs. We suggest measuring resilience subjectively and objectively. Subjectively, use a 5-item guided interview revised from the Connor-Davidson Resilience Scale (CDRC), a scale of 10 items. The revised CDRC scale is a 5-item scale. The scale is rated on a 5-point Likert scale from 0 (not true) to 4 (true all the time). The total score ranges from 0 to 20, with higher total scores indicating greater resilience. The scale demonstrated good construct validity and internal consistency (α = 0.85) during the development of the scale. The CD-RISC had a good Cronbach’s alpha level of 0.85. The Revised CD-RISC can be completed in 2 - 4 minutes. To measure resilience objectively, we suggest using Immune Resilience (IR) levels, the level of resilience to preserve and/or rapidly restore immune resilience functions that promote disease resistance and control inflammation and other inflammatory stress. IR levels are gauged with two peripheral blood metrics that quantify the balance between CD8 and CD4 T-cell levels and gene expression signatures tracking longevity-associated immunocompetence and mortality- or entropy-associated inflammation. Background Previous studies have shown evidence of health-related risk behaviors among women who have sex with women （WSW）, such as sex with men, multiple bisexual partners, and drug use. Women who have sex with women have also been known to avoid routine physical examinations and conceal their same-sex history from physicians, which can affect their ability to receive an accurate diagnosis and treatment. No previous research has targeted women who have sex with women in China. We sought to describe women who have sex with women in China and explore risk factors for their reproductive tract infections （RTI）/sexually transmitted infections （STI）. Methods Participants were recruited through outreach in venues and online for a cross-sectional study. Data were collected using interviews and laboratory tests. Results We recruited 224 women who have sex with women. In the year preceding their participation in the study, 92% （206/224） of women reported sexual relations with women. The RTI rates were： gonorrhea （15.8%）, chlamydia （3.5%）, syphilis （0.5%）, bacterial vaginosis （14.4%）, hepatitis B virus （hBV）（0.9%）, hepatitis C virus （hCV）（0.5%）, and candidiasis （6.9%）. No hIV or herpes simplex virus （hSV） positive cases were detected. Factors associated with gonorrhea infection were non-Beijing local residency （odds ratio （OR）=2.1, 95% confidence interval （C/）： 1.2-3.8） and genital-genital contact （OR=3.1, 95% CI： 1.3-7.2）; factors associated with curable STI （excluding bacterial vaginosis, candidiasis, hBV and hCV） were non-Beijing local residency （OR=1.9; 95% CI： 1.2-3.0） and bleeding during or after sex （OR=18.1; 95% CI： 5.2-62.6）; and the factor associated with RTI （including all the infections tested） was bleeding during or after sex （OR=37.8, 95% CI： 11.2-127.4）. Conclusions Behaviors that may cause RTI/STI exist among Chinese women who have sex with women. Researchers should consider these behaviors when planninq correspondin.q prevention and interventions.
