AIM:To evaluates the effectiveness and safety of the first generation,NS3/4A protease inhibitors(PIs) in clinical practice against chronic C virus,especially in patients with advanced fibrosis. METHODS:Prospective study and non-experimental analysis of a multicentre cohort of 38 Spanish hospitals that includes patients with chronic hepatitis C genotype 1,treatment-na?ve(TN) or treatment-experienced(TE),who underwent triple therapy with the first generation NS3/4A protease inhibitors,boceprevir(BOC) and telaprevir(TVR),in combination with pegylated interferon and ribavirin. The patients were treatment in routine practice settings. Data on the study population and on adverse clinical and virologic effects were compiled during the treatment period and during follow ***:One thousand and fifty seven patients were included,405(38%) were treated with BOC and 652(62%) with TVR. Of this total,30%(n = 319) were TN and the remaining were TE:28%(n = 298) relapsers,12%(n = 123) partial responders(PR),25%(n = 260) null-responders(NR) and for 5%(n = 57) with prior response unknown. The rate of sustained virologic response(SVR) by intention-to-treatment(ITT) was greater in those treated with TVR(65%) than in those treated with BOC(52%)(P < 0.0001),whereas by modified intention-to-treatment(m ITT) no were found significant differences. By degree of fibrosis,56% of patients were F4 and the highest SVR rates were recorded in the non-F4 patients,both TN and TE. In the analysis by groups,the TN patients treated with TVR by ITT showed a higher SVR(P = 0.005). However,by m ITT there were no significant differences between BOC and TVR. In the multivariate analysis by m ITT,the significant SVR factors were relapsers,IL28 B CC and non-F4; the type of treatment(BOC or TVR) was not significant. The lowest SVR values were presented by the F4-NR patients,treated with BOC(46%) or with TVR(45%). 28% of the patients interrupted the treatment,mainly by non-viral response(51%):this outcome was

Correlations between corneal and optic nerve head variables in healthy subjects and patients with primary open angle glaucoma

AIMTo correlate corneal variables (determined using the Pentacam) with optic nerve head (ONH) variables determined using the Heidelberg retina tomograph (HRT) in healthy subjects and patients diagnosed with primary open angle glaucoma (POAG).METHODSMeasurements were made in 75 healthy eyes and 73 eyes with POAG and correlations examined through Pearson correlation coefficients between the two sets of variables in the two subject groups. The corneal variables determined were corneal volume (CVol), central corneal thickness (CCT), overall corneal thickness (OvCT), the mean thickness of a circular zone centered at the corneal apex of 1 mm radius (zone I) and the mean thickness of several concentric rings, also centered at the apex until the limbus, each of 1 mm width (zones II to VI respectively). The ONH variables were determined using the *** following pairs of variables were correlated in the control group: CCT-disc area (DAr) (-0.48; P<0.0001), Zone I-DAr (-0.503; P<0.0001) and Zone II-DAr (-0.443; P<0.0001); and in the POAG group: CCT-cup-to-disc area ratio (CDRa) (-0.402; P<0.0001), Zone I-CDRa (-0.418; P<0.0001), Zone II-CDRa (-0.405; P=0.006), Zone I-cup shape measure (CSM) (-0.415; P=0.002), Zone II-CSM (-0.405; P=0.001), Zone IV-height variation contour (HVC) (0.378; P=0.002); Zone V-HVC (0.388, P<0.0001).CONCLUSIONSIn the healthy subjects, significant negative correlation was detected between central and paracentral corneal thickness and optic disc area. In contrast, the POAG patients showed significant negative correlation between central and paracentral corneal thickness and the cup-disc ratio and CSM, and positive correlation between peripheral corneal thickness and HVC.

The 2-oxoglutarate and Fe （ll）-dependent dioxygenase （2OGD） superfamily includes oxidative enzymes with an active site con- taining two histidines and （in most cases） one aspartic or glutamic acid residue. This conserved motif is termed the 2-His-1-carboxylate facial triad, chelates iron, and is housed within a double-stranded β-helix fold, also known as the DSBH, jelly-roll, cupin, or Jumonji C fold （Martinez and Hausinger, 2015）. The reactions catalyzed by 2OGDs （also called 2ODDs, 2ODQs, and 2OGXs） include but are not limited to demethylation, demethylenation, hydroxytation, halogenation, desaturation, ring cleavage, ring closure, and epimerization （Farrow and Facchini, 2014）.

Multitemporal landslide inventory analysis of an intertropical mountain in west-central Mexico——Basis for hazard management

Landslides in intertropical mountainous areas of Mexico are a natural hazard that could potentially generate a disaster. Despite this, many areas lack landslide inventories, making it difficult to monitor and efficiently manage the associated risks for local populations. For this reason, the aim of this research was to undertake a multitemporal landslide inventory and analyze its conditioning factors in the Pico de Tancítaro stratovolcano, which, in recent decades, has presented several high-risk geomorphological events with disastrous consequences for nearby localities. The spatial distribution of landslides and its conditioning factors were mapped at a 1:50,000 scale for the 1995-2020 period, through visual interpretation of orthophotos and satellite images(Landsat, SPOT, and SENTINEL). The conditioning factors that link the most to landslides were Quaternary andesite and basaltic andesite rocks;volcanic cones, mountain river valleys, and very steep to steep slopes(30° to >45°), oriented to the South, North, and East;Andosols and Leptosols;and the oyamel fir and mixed forests. 505 landslide events classified into slides and flows that occurred in 1995, 2004, 2010, and 2015 were identified. New scars generated by the reactivation of landslide processes were also observed. The landslides in the study area were triggered by torrential rains caused by hurricanes and tropical storms;such as hurricanes Alex and Patricia. The inventory and analysis of conditioning factors highlighted the distribution and dynamics of slides and flows in the study area. In addition, the most important factors for studies of landslide susceptibility, hazards, and risks in the Pico de Tancítaro stratovolcano were identified, contributing to the management of geomorphological risks in intertropical mountain areas.

The detection of dysplasia in patients with inflammatory bowel disease(IBD)continues to be important given the increased risk of colorectal cancer in this ***,in 2017,we performed a review and update of the recommendations for the management and follow-up of patients with IBD based on the clinical practice guidelines of various scientific *** present manuscript focuses on new aspects of the detection,follow-up,and management of dysplasia according to the latest studies and *** chromoendoscopy with targeted biopsy continues to be the technique of choice for the screening and detection of dysplasia in IBD,the associated difficulties mean that it is now being compared with other techniques(virtual chromoendoscopy),which yield similar results with less technical ***,the emergence of new endoscopy techniques that are still being researched but seem promising(e.g.,confocal laser endomicroscopy and full-spectrum endoscopy),together with the development of devices that improve endoscopic visualization(e.g.,Endocuff Vision),lead us to believe that these approaches can revolutionize the screening and follow-up of dysplasia in patients with ***,further studies are warranted to define the optimal follow-up strategy in this patient population.

In this paper multistep methods for higher order dtherential systems of the type Y(r) = f(t, Y) are proposed. Such methods permit the numerical solutions of initial value problems for such systems, providing error bounds and avoiding the increase of the computational cost derived from the standard approach based on the collsideration of an equivalent extended first order system.

Effectiveness of a course in the control of tobacco use for university health students: A quasi-experimental community intervention trial

Introduction: The aim of the study was to evaluate the effectiveness of a tobacco control course in the reduction of prevalence and the improvement of knowledge and attitudes among university health students. Method: Quasi-experimental study on a community based intervention. Interventional campus students received a tobacco control course in the first year of the degree. Data were collected by Self-Answered Questionnaire during three academic years in two graduations years among health students in both campuses. Results: 84.4% of students participated in the study. In both campuses tobacco use was reduced, not significantly, above all in those joining courses from origins other than baccalaureate. In the intervention and control campus showed an increase in the nicotine dependency according to Fagerstr?m test and a little change in motivation according toRichmondtest. The regression model best explaining the improving of the knowledge included academic course mainly and campus and academic background too. With regard to opinions and beliefs tobacco use was not influenced, being associated in general to the academic course. Conclusions: A specific tobacco control course did not change prevalence of tobacco, nicotine dependence and motivation to give up. The improvement in knowledge was associated with academic course in both campuses. We recommend to commence smoking cessation strategies in university health students.

Diagnosis and management of bacterial infections in decompensated cirrhosis

Bacterial infections are one of the most frequent complications in cirrhosis and result in high mortality *** with cirrhosis have altered and impaired immunity,which favours bacterial *** of infections are more frequent in patients with decompensated cirrhosis than those with compensated liver *** most common and life-threatening infection in cirrhosis is spontaneous bacterial peritonitis followed by urinary tract infections,pneumonia,endocarditis and skin and soft-tissue *** with decompensated cirrhosis have increased risk of developing sepsis,multiple organ failure and *** factors associated with the development of infections are severe liver failure,variceal bleeding,low ascitic protein level and prior episodes of spontaneous bacterial peritonitis (SBP).The prognosis of these patients is closely related to a prompt and accurate *** appropriate treatment decreases the mortality *** strategies are the mainstay of the management of these *** antibiotics should be started immediately following the diagnosis of SBP and the first-line antibiotic treatment is third-generation ***,the efficacy of currently recommended empirical antibiotic therapy is very low in nosocomial infections including SBP,compared to community-acquired *** may be associated with the emergence of infections caused by Enterococcus faecium and extended-spectrum β-lactamaseproducing Enterobacteriaceae,which are resistant to the first line antimicrobial agents used for *** emergence of resistant bacteria,underlines the need to restrict the use of prophylactic antibiotics to patients with the greatest risk of *** infections should be treated with wide spectrum *** studies of early diagnosis,prevention and treatment are needed to improve the outcomes in patients with decompensated cirrhosis.

Grain yield losses in yellow-rusted durum wheat estimated using digital and conventional parameters under field conditions

The biotrophic fungus Puccinia striiformis f. sp. tritici is the causal agent of the yellow rust in wheat. Between the years 2010–2013 a new strain of this pathogen(Warrior/Ambition),against which the present cultivated wheat varieties have no resistance, appeared and spread rapidly. It threatens cereal production in most of Europe. The search for sources of resistance to this strain is proposed as the most efficient and safe solution to ensure high grain production. This will be helped by the development of high performance and low cost techniques for field phenotyping. In this study we analyzed vegetation indices in the Red,Green, Blue(RGB) images of crop canopies under field conditions. We evaluated their accuracy in predicting grain yield and assessing disease severity in comparison to other field measurements including the Normalized Difference Vegetation Index(NDVI), leaf chlorophyll content, stomatal conductance, and canopy temperature. We also discuss yield components and agronomic parameters in relation to grain yield and disease ***-based indices proved to be accurate predictors of grain yield and grain yield losses associated with yellow rust(R2= 0.581 and R2= 0.536, respectively), far surpassing the predictive ability of NDVI(R2= 0.118 and R2= 0.128, respectively). In comparison to potential yield, we found the presence of disease to be correlated with reductions in the number of grains per spike, grains per square meter, kernel weight and harvest index. Grain yield losses in the presence of yellow rust were also greater in later heading varieties. The combination of RGB-based indices and days to heading together explained 70.9% of the variability in grain yield and 62.7% of the yield losses.
