Is cholecystectomy a reasonable treatment option for simple gallbladder polyps larger than 10 mm?

AIM: To determine the relevance of the 10-mm size criterion of the generally accepted surgical indication for gallbladder polyps(GBPs).METHODS: We collected data of patients who were confirmed to have GBPs through cholecystectomy at Samsung Medical Center between January 1997 andDecember *** the patients who underwent cholecystectomy for GBP, those with a definite evidence for malignancy such as adjacent organ invasion, metastasis on preoperative imaging studies, polyp larger than 20 mm, absence of preoperative imaging study results, and patients having gallstones were *** retrospectively collected and analyzed information on patient's clinical characteristics, symptoms, ultrasonographic findings, and blood laboratory ***: A total of 836 patients who had undergone cholecystectomy were retrospectively *** hundred eighty patients(93%) had benign polyps, whereas 56 patients(7%) had malignant *** the 56 patients with malignancy, 4 patients(7%) had borderline GBP(10-12 mm) and a patient had small GBP(< 10 mm) with T2 *** conducted an ROC curve analysis to verify the 10-mm size criteria(AUC = 0.887, SD = 0.21, P < 0.001).In the ROC curve for polyp size and malignancy, sensitivity and specificity of the 10-mm size criterion was 98.2% and 19.6%, *** specificity of the 11-mm and 12-mm size criteria was 44.6% and 56%, respectively, whereas the sensitivity of these two size criteria was *** defined the GBPs of 10 to 12 mm as a borderline-sized GBP, which were found in 411 patients(49%).In this group, there was a significant difference in age between patients with benign and malignant GBPs(47 years vs 60 years, P < 0.05).CONCLUSION: GBPs larger than 13 mm need immediate excision whereas for borderline-sized GBPs detected in young patients, careful medical observation can be a rational decision.

For the global commercialization of highly efficient and stable perovskite solar cells(PSCs),it is necessary to effectively suppress the formation of various defects acting as nonradiative recombination sources in perovskite light-harvesting *** defects between the charge-selective layer and the perovskite are easily formed in the solution process used to fabricate perovskite *** addition,owing to the difference in thermal expansion coefficients between the substrate and the perovskite film,internal residual tensile stress inevitably occurs,resulting in increased nonradiative ***,a simple compositional engineering scheme for realizing efficient and stable PSCs,which incorporates acetamidinium bromide(AABr)as an additive into the MAPbI_(3) lattice,is *** an additive,AABr has been found to provide synergistic multiple passivation for both internal and interfacial *** was found to effectively release the tensile strain of the MAPbI_(3) film by forming a structure stabilized by NH-I hydrogen bonds,as evidenced by calculations based on density functional theory(DFT).Furthermore,the incorporated AABr additives created a charge carrier recombination barrier to enhance charge collection capability by reducing interfacial ***,a power conversion efficiency(PCE)of 20.18%was achieved using a planar device employing AABr-incorporated MAPbI_(3).This was substantially higher than the 18.32% PCE of a pristine MAPbI_(3)-based ***,unencapsulated PSCs using AABr-incorporated MAPbI_(3) absorbers exhibited excellent long-term stability,maintaining>95% of initial PCE up to 1200 hours in ambient air.

Several studies have demonstrated that human umbilical cord blood-derived mesenchymal stem cells can promote neural regeneration following brain injury. However, the therapeutic effects of human umbilical cord blood-derived mesenchymal stem cells in guiding peripheral nerve regeneration remain poorly understood. This study was designed to investigate the effects of human umbilical cord blood-derived mesenchymal stem cells on neural regeneration using a rat sciatic nerve crush injury model. Human umbilical cord blood-derived mesenchymal stem cells （1 ~ 106） or a PBS control were injected into the crush-injured segment of the sciatic nerve. Four weeks after cell injection, brain-derived neurotrophic factor and tyrosine kinase receptor B mRNA expression at the lesion site was increased in comparison to control. Furthermore, sciatic function index, Fluoro Gold-labeled neuron counts and axon density were also significantly increased when compared with control. AIM： To confirm the predictive factors for interferon （IFN）-α and ribavirin combination therapy for chronic hepatitis patients with hepatitis C virus （HCV） genotype lb. METHODS： HCV RNA from 50 patients infected with HCV genotype lb was studied by cloning and sequencing of interferon sensitivity determining region （ISDR）, PKR- eIF2α phosphorylation homology domain （PePHD）. Patients were treated with IFN-α and ribavirin for 6 mo and grouped by effectiveness of the therapy. A variety of factors were analyzed. RESULTS： Our data showed that age, HCV RNA titer, and ISDR type could be used as the predictive factors for combined IFN-α and ribavirin efficacy. Characteristically, mutations in PePHD appeared only when the combination therapy was effective. Other factors, such as sex and alanine aminotransferase （ALT） level, were not related to its efficacy. Adjusting for age and HCV RNA titer indicated that the ISDR type was the most potent predictive factor. CONCLUSION： HCV RNA ISDR type is an important factor for predicting efficacy of IFN-α and ribavirin combination therapy in Korean patients.

The microstructures of the brazed joints for commercially pure Ti and stainless steel were investigated by the applications of various filler alloys including Ag-, Ti-, Zr- and Ni-based alloys. Generally, the dissimilar joints between Ti and stainless steel were dominated by the Ti-based intermetallic compounds （IMCs）, e.g. （Ti, Zr）2（Fe, Ni）, TiFe, TiCu, and Ti2（Fe, Ni）, due to a significant dissolution of Ti from the base metal. The （Fe-Cr） cr phase was also observed near the stainless steel due to a segregation of Cr into the interface region. This research demonstrates empirically that the brittleness of the Ti and stainless steel joint can not be avoided only by applying single braze alloy or single insert metal, and thus an introduction of additional suitable interlayer between the filler alloy and the base metal is necessary to prevent the brittleness of the joint.

The effect of ultraviolet-ozone(UVO)irradiation on amorphous(am)SnO_(2) and its impact on the photoconversion efficiency of MAPbI3-based perovskite solar cells were investigated in *** treatment was found to increase the amount of chemisorbed oxygen on the am-SnO_(2) surface,reducing the surface energy and contact *** changes in the am-SnO_(2) surface lowered the Gibbs free energy for the densification of perovskite films and facilitated the formation of homogeneous perovskite *** addition,the Fermi energy of the UVO-treated am-SnO_(2) shifted upwards to achieve an ideal band offset for MAPbI3,which was verified by theoretical calculations based on the density functional *** achieved a champion efficiency of 19.01% with a statistical reproducibility of 17.01±1.34% owing to improved perovskite film densification and enhanced charge transport/extraction,which is considerably higher than the 13.78±2.15% of the ***,UVO-treated,am-SnO_(2)-based devices showed improved stability and less hysteresis,which is encouraging for the future application of up-scaled perovskite solar cells.

The application of Li metal anodes in rechargeable batteries is impeded by safety issues arising from the severe volume changes and formation of dendritic Li ***‐dimensional hollow carbon is receiving increasing attention as a host material capable of accommodating Li metal inside its cavity;however,uncontrollable and nonuniform deposition of Li remains a *** this study,we synthesize metal–organic framework‐derived carbon microcapsules with heteroatom clusters(Zn and Ag)on the capsule walls and it is demonstrated that Ag‐assisted nucleation of Li metal alters the outward‐to‐inward growth in the microcapsule ***‐incorporated microcapsules are prepared via chemical etching of zeolitic imidazole framework‐8 polyhedra and are subsequently decorated with Ag by a galvanic displacement reaction between Ag^(+) and metallic *** introduced Ag significantly reduces the energy barrier and increases the reaction rate for Li nucleation in the microcapsule host upon Li *** combined electrochemical,microstructural,and computational studies,we verify the beneficial role of Ag‐assisted Li nucleation in facilitating inward growth inside the cavity of the microcapsule host and,in turn,enhancing electrochemical *** study provides new insights into the design of reversible host materials for practical Li metal batteries.
