Genitourinary melanoma:An overview for the clinician

Genitourinary (GU) melanoma is a rare presentation of melanoma accounting for approximately 0.5% of all *** melanomas include primary melanomas of the vulva,vagina,uterine cervix,ovary,penis,scrotum,urethra,bladder,ureter,and *** melanomas are often diagnosed in advanced stages and stigma is thought to contribute to delays in *** the likely diagnosing provider,it is imperative that dermatologists,urologists,and gynecologists are aware of these uncommon sites of *** there have been major advances in the treatment of melanomas as a whole in the last 10 years,their applications to GU melanomas have often been *** melanomas have not been included in many of the major phase II clinical trials which brought contemporary advanced treatments to market and the prognoses for GU melanomas remain *** to the rarity of GU melanomas,much of the literature provides gener alized recommendations across multiple different organs affected by GU melanomas or omits certain topics,making it difficult to appreciate the funda-mentals of the individual *** review aimed to provide background information on the pathogenesis and epidemiology of the different sites of GU melanomas and categorize data specific to the presentation,staging,treatment,and prognosis of each type of GU melanoma to guide the *** was also meant to encourage a multidisciplinary approach to the management of these patients as it spans the expertise of surgical oncologists,medical oncologists,radiation oncologist,dermatologists,urologists,and gynecologists.

Nutritional stimulation by in-ovo feeding modulates cellular proliferation and differentiation in the small intestinal epithelium of chicks

Nutritional stimulation of the developing small intestine of chick embryos can be conducted by in-ovo feeding(IOF).We hypothesized that IOF of glutamine and leucine can enhance small intestinal development by promoting proliferation and differentiation of multipotent small intestinal epithelial *** embryos(n=128)were subject to IOF of glutamine(IOF-Gln),leucine(IOF-Leu),NaCl(IOF-NaCl)or no injection(control)at embryonic d 17(e 17).Multipotent,progenitor and differentiated cells were located and quantified in the small intestinal epithelium between e 17 and d 7 after hatch(D 7)in all treatment groups by immunofluorescence of SRY-box transcription factor 9(Sox9)and proliferating cell nuclear antigen(PCNA),in-situ hybridization of leucine-rich repeat containing G-protein coupled receptor 5(Lgr5)and peptide transporter 1(PepT1)and histochemical goblet cell *** effects of IOF treatments at e 19(48 h post-IOF),in comparison to control embryos,were as follows:total cell counts increased by 40%,33%and 19%,and multipotent cell counts increased by 52%,50%and 38%,in IOF-Gln,IOF-Leu and IOF-NaCl embryos,*** IOF-Gln embryos exhibited a significance,36%increase in progenitor cell *** IOF treatments shifted Lgr5+ stem cell localizations to villus *** differentiated,PepT1+region of the villi was 1.9 and 1.3-fold longer in IOF-Gln and IOF-Leu embryos,respectively,while goblet cell densities decreased by 20%in IOF-Gln ***,crypt and villi epithelial cell counts were significantly higher IOF-Gln chicks,compared to control chicks(Pe conclude IOF of glutamine stimulates small intestinal maturation and functionality during the peri-hatch period by promoting multipotent cell proliferation and differentiation,resulting in enhanced compartmentalization of multipotent and differentiated cell niches and expansions of the absorptive surface area.

Probing the subcellular nanostructure of engineered human cardiomyocytes in 3D tissue

The structural and functional maturation of human induced pluripotent stem cell-derived cardiomyocytes(hiPSC-CMs)is essential for pharmaceutical testing,disease modeling,and ultimately therapeutic *** 3D-tissue platforms have improved the functional maturation of hiPSC-CMs,but probing cardiac contractile properties in a 3D environment remains challenging,especially at depth and in live *** small-angle X-ray scattering(SAXS)imaging,we show that hiPSC-CMs matured and examined in a 3D environment exhibit a periodic spatial arrangement of the myofilament lattice,which has not been previously detected in *** contractile force is found to correlate with both the scattering intensity(R^(2)=0.44)and lattice spacing(R^(2)=0.46).The scattering intensity also correlates with lattice spacing(R^(2)=0.81),suggestive of lower noise in our structural measurement than in the functional ***,we observed decreased myofilament ordering in tissues with a myofilament mutation known to lead to hypertrophic cardiomyopathy(HCM).Our results highlight the progress of human cardiac tissue engineering and enable unprecedented study of structural maturation in hiPSC-CMs.

Mainstream silicon photonic integrated circuits are based on compact and low-loss silicon-on-insulator(SOI)waveguide platforms. However, monolithic SOI-based photonics provides only a limited number of functional device types. Here, to extend the on-chip capabilities, we propose a general heterogeneous integration approach to embed highly nonlinear Ⅲ-Ⅴ(Al Ga As) photonics into the SOI platform. We develop low-loss AlGaAs-on-SOI photonic circuits with integrated Si waveguides and showcase sub-milliwatt-threshold(-25 mW) Kerr frequency comb generation in ultrahigh-QAlGaAs microrings(Q over 10^(6)) at the telecom bands. Our demonstration complements existing mature Si photonics technology with efficient nonlinear functionalities provided by Ⅲ-Ⅴ and propels conventional Si photonics into emerging nonlinear photonic applications towards fully chip-based nonlinear engines.

Cesarean Operative Reports: We Can Do Better!

Background: The operative report is an essential component of communication, patient care, and safety;it is a JCAHO requirement and legal document. Almost 80% of residency program directors surveyed agreed on the importance of the operative report, yet less than 20% of programs provide formal resident training. Documentation has important implications in obstetrics as route and timing of future deliveries are often influenced by operative findings. Purpose: Examine completeness of cesarean operative reports using a validated instrument adapted from general surgery. Assess the impact of an educational resident in-service on the required elements and documentation in resident generated cesarean operative reports. Methods: A quality improvement project identified significant deficiencies in resident generated cesarean operative reports. We conducted a retrospective chart review of cesarean operative reports from November 2019 to May 2020 at Jersey Shore University Medical Center. Our study was conducted in three phases. In phase 1, a cohort of cesarean operative reports was examined using a structured evaluation tool. In phase 2, an educational resident in-service on standardization of operative notes was provided. In phase 3, another cohort of cesarean operative reports was examined using the same evaluation tool after the resident in-service. Analysis for improvement was performed. Results: Fifty pre and post-in-service cesarean operative report evaluations weree="font-family:Verdana;"> compared (N = 100). Quantifying the relationships between plant functional traits and ecosystem services has been promoted as an approach to achieving multifunctional grassland systems that balance productivity with other regulating,supporting and cultural *** trade-offs and synergies between traits and services has largely relied on meta-analyses of studies from different systems and *** study demonstrated the value of focused studies of long-term experiments in grassland systems that measure traits and services in the same space and time to better understand the ecological constraints underlying these trade-offs and *** analysis is presented that uses data from the Park Grass experiment at Rothamsted Research on aboveground productivity,species richness and soil organic carbon stocks to quantify the relationships between these three outcomes and the power of variance in plant functional traits in explaining *** was a trade-off between plots with high productivity,nitrogen inputs and soil organic carbon and plots with high species richness that was explained by a functional gradient of traits that are indicative of contrasting strategies of resource acquisition of resource *** were identified of using functional traits to identify opportunities for mitigating these trade-offs and moving toward more multifunctional systems.

Seepage and stress analysis of anti-seepage structures constructed with different concrete materials in an RCC gravity dam

This study used the finite element method （FeM） to analyze the stress field and seepage field of a roller-compacted concrete （RCC） dam, with an upstream impervious layer constructed with different types of concrete materials, including three-graded RCC, two-graded RCC, conven- tional vibrated concrete （CVC）, and grout-enriched vibrated RCC （GeVR）, corresponding to the design schemes S 1 through $4. It also evaluated the anti-seepage performance of the imperious layer in the four design schemes under the normal water level and flood-check level. Stress field analysis of a retaining section and discharge section shows that the maximum tensile stress occurs near the dam heel, the maximum compressive stress occurs near the dam toe, and the stress distributions in the four schemes can satisfy the stress control criteria. Seepage field analysis shows that the uplift pressure heads in schemes S3 and S4 descend rapidly in the anti-seepage region, and that the calculated results of daily seepage flow under the steady seepage condition in these two schemes are about 30%-50% lower than those in the other two schemes, demonstrating that CVC and GeVR show better anti-seepage performance. The results provide essential parameters such as the uplift pressure head and seelga^e flow for physical model tests and anti-seepage structure selection in RCC dams.

Highly efficient nonlinear optical（NLO） materials with well-defined architectures in the wavelength and subwavelength length scales are of particular importance for next generation of integrated photonic circuits. Fluorenone analogues have been demonstrated to be promising candidates as building blocks for assembly of organic NLO materials thanks to their synergistic supramolecular interactions and brilliant optical properties. Here we have studied the polymorphs of a phenylethynyl functionalized fluorenone derivative, and their controlled self-assembly for microstructures with different morphologies. These polymorphic microcrystals exhibit very distinctive NLO properties, highly related to their supramolecular and electronic structures.
