Robot-assisted laparoscopic radical cystectomy with complete intracorporeal urinary diversion

Robot-assisted radical cystectomy with intracorporeal urinary diversion(RARCICUD)has only recently been explored as a viable surgical option for patients with muscle-invasive bladder cancer seeking satisfactory oncologic control while benefiting from minimally invasive surgical *** by earlier open and laparoscopic work,initial descriptions of RARC-ICUD were published in 2003,and have since been followed by multiple larger case series which have suggested promising outcomes for our ***,the rate of adoption has remained relatively slow when compared to other robotassisted procedures such as the radical prostatectomy,likely owing to longer operative times,operative complexity,costs,and uncertainty regarding oncologic *** operative technique for RARC-ICUD has evolved over the past decade and several high-volume centers have shared tips to improve efficiency and make the operation possible for a growing number of *** there are still questions regarding economic costs,effectiveness,and generalizability of outcomes reported in published data,a growing dataset has brought us ever closer to the ***,we present our current operative technique for RARC-ICUD and discuss the state of the literature so that the urologist may hold an informed discussion with his or her patients.

mitigating osteonecrosis of the jaw(ONJ) through preventive dental care and understanding of risk factors

It is well established that alterations in phosphate metabolism have a profound effect on hard and soft tissues of the oral cavity. The present-day clinical form of osteonecrosis of the jaw(ONJ) was preceded by phosphorus necrosis of the jaw, ca. 1860. The subsequent removal of yellow phosphorus from matches in the early 20 th century saw a parallel decline in "phossy jaw" until the early 2000 s, when similar reports of unusual jaw bone necrosis began to appear in the literature describing jaw necrosis in patients undergoing chemotherapy and concomitant steroid and bisphosphonate treatment. Today, the potential side effect of ONJ associated with medications that block osteoclast activity(antiresorptive) is well known, though the mechanism remains unclear and the management and outcomes are often unsatisfactory. much of the existing literature has focused on the continuing concerns of appropriate use of bisphosphonates and other antiresorptive medications, the incomplete or underdeveloped research on ONJ, and the use of drugs with anabolic potential for treatment of osteoporosis. While recognizing that ONJ is a rare occurrence and ONJ-associated medications play an important role in fracture risk reduction in osteoporotic patients, evidence to date suggests that health care providers can lower the risk further by dental evaluations and care prior to initiating antiresorptive therapies and by monitoring dental health during and after treatment. This review describes the current clinical management guidelines for ONJ, the critical role of dental-medical management in mitigating risks, and the current understanding of the effects of predominantly osteoclast-modulating drugs on bone homeostasis.

Optimization of sugar production from Durian seeds via alkaline hydrolysis for second-generation bioethanol production

As one way to eliminate the issues found in the preceding generation,feedstock exploration in second-generation bioethanol production remains an issue,especially for a tropical country such as *** exotic fruit by-products,durian holds a promising perspective that rests on its abundance,superb carbohydrate content and limited usage until *** work presents the first-ever utilization of durian seeds for sugar production under optimized conditions through alkaline hydrolysis.A simple form of sugar was extracted by varying four parameters,namely substrate loading,NaOH concentration,hydrolysis time and hydrolysis *** surface methodology based on the Box-Behnken design was employed to outline the most optimum parameter *** of variance revealed that the quadratic model fit the data appropriately with the order of significance as substrate loading>hydrolysis time>NaOH concentration>hydrolysis *** optimized conditions for reducing sugar yield,as high as 2.140 g/L,corresponded tom NaOH,60 minutes of hydrolysis time and 80oC hydrolysis *** possible ethanol content of 1.094 g/L was also expected under optimized conditions,demonstrating great potential in second-generation bioethanol production.

An improved symmetry-based approach to reciprocal space path selection in band structure calculations

Band structures for electrons,phonons,and other quasiparticles are often an important aspect of describing the physical properties of periodic *** commonly,energy bands are computed along a one-dimensional path of high-symmetry points and line segments in reciprocal space(the“k-path”),which are assumed to pass through important features of the dispersion ***,existing methods for choosing this path rely on tabulated lists of high-symmetry points and line segments in the first Brillouin zone,determined using different symmetry criteria and unit cell *** we present a new“on-the-fly”symmetry-based approach to obtaining paths in reciprocal space that attempts to address the previous limitations of these *** a unit cell of a magnetic or nonmagnetic periodic solid,the site symmetry groups of points and line segments in the irreducible Brillouin zone are obtained from the total space *** elements in these groups are used alongside general and maximally inclusive high-symmetry criteria to choose segments for the final k-path.A smooth path connecting each segment is obtained using graph *** new framework not only allows for increased flexibility and user convenience but also identifies notable overlooked features in certain electronic band *** addition,a more intelligent and efficient method for analyzing magnetic materials is also enabled through proper accommodation of magnetic symmetry.

Properties of alkylbenzimidazoles for CO_2 and SO_2 capture and comparisons to ionic liquids

To date, few reports have been concerned with the physical properties of the liquid phases of imidazoles and benzimidazoles- potential starting materials for a great number of ionic liquids. Prior research has indicated that alkylimidazole solvents exhibit different, and potentially advantageous physical properties, when compared to corresponding imidazolium-based ionic liquids. Given that even the most fundamental physical properties of alkylimidazole solvents have only recently been reported, there is still a lack of data for other relevant imidazole derivatives, including benzimidazoles. In this work, we have synthesized a se- ries of eight 1-n-alkylbenzimidazoles, with chain lengths ranging from ethyl to dodecyl, all of which exist as neat liquids at ambient temperature. Their densities and viscosities have been determined as functions of both temperature and molecular weight. Alkylbenzimidazoles have been found to exhibit viscosities that are more similar to imidazolium-based ILs than al- kylimidazoles, owed to a large contribution to viscosity from the presence of a fused ring system. Solubilities of CO2 and SO2, two species of concern in the emission of coal-fired power generation, were determined for selected alkylbenzimidazoles to understand what effects a fused ring system might have on gas solubility. For both gases, alkylbenzimidazoles were deter- mined to experience physical, non-chemically reactive, interactions. The solubility of CO2 in alkylbenzimidazoles is 10%-30% less than observed for corresponding ILs and alkylimidazoles. 1-butylbenzimidazole was found to readily absorb at least 0.333 gram SO2 per gram at low pressure and ambient temperature, which could be readily desorbed under an N： flush, a behavior more similar to imidazolium-based ILs than alkylimidazoles. Thus, we find that as solvents for gas separations, benzimidazoles share characteristics with both ILs and alkylimidazoles.

Whole-exome sequencing and microRNA profiling reveal PI3K/AKT pathway＇s involvement in juvenile myelomonocytic leukemia

Background： Clinical studies and genetic analyses have revealed that juvenile myelomonocytic leukemia （JmmL） is caused by somatic and/or germline mutations of genes involved in the RAS/mAPK signalling pathway. Given the vastly different clinical prognosis among individual patients that have had this disease, mutations in genes of other pathways may be involved. methods： In this study, we conducted whole-exome and cancer-panel sequencing analyses on a bone marrow sample from a 2-year old juvenile myelomonocytic leukemia patient. We also measured the microRNA profile of the same patient＇s bone marrow sample and the results were compared with the normal mature monocytic cells from the pooled peripheral blood. Results： We identified additional novel mutations in the Pi3K/AKT pathway and verified with a cancer panel targeted sequencing. We have confirmed the previously tested PTPNII gene mutation （exon 3 181G〉T） in the same sample and identified new nonsynonymous mutations in NTRKI, HmGA2, mLH3, mYH9 and AKT1 genes. many of the microRNAs found to be differentially expressed are known to act as oncogenic microRNAs （onco^microRNAs or oncomiRs）, whose target genes are enriched in the PI3K/AKT signalling pathway. Conclusions： Our study suggests an alternative mechanism for JmmL pathogenesis in addition to RAS/mAPK pathway. This discovery may provide new genetic markers for diagnosis and new therapeutic targets for JmmL patients in the future.

Dose-dependent associations of joint aerobic and muscle-strengthening exercise with obesity:A cross-sectional study of 280,605 adults

Background:Emerging epidemiological evidence suggests that compared to engaging in 1 activity mode alone,a combination of moderate-tovigorous physical activity(mVPA:brisk walking/jogging,cycling)and muscle-strengthening exercise(mSE:push-ups/sit-ups,using weight machines)has more favorable associations with optimal weight ***,few studies have examined the dose-dependent and joint associations of mVPA and mSE with ***:Based on cross-sectional analyses of the European Health Interview Survey Wave 2(2013-2014),we examined prevalence ratios(PRs)of joint and stratified associations between mVPA(4 categories:(ⅰ)0 min/week,(ⅱ)1-149 min/week,(ⅲ)150-299 min/week,and(ⅳ)≥300 min/week)and mSE(3 categories:(ⅰ)0 day/week,(ⅱ)1 day/week,and(ⅲ)>2 days/week)with body mass index-defined obesity(body mass index of>30.0 kg/m2)using Poisson regression with robust error *** were examined unadjusted and adjusted for sociodemographic and lifestyle characteristics(e.g.,sex,age,education,income,and smoking status).Results:Data were available for 280,456 adults(≥18 years),of which 46,166(15.5%)were *** interaction mVPA x mSE guideline adherence was statistically significant for obesity(p≤0.05).The joint mVPA-mSE analysis showed that compared to the reference group(i.e.,no mVPA and no mSE),the PRs followed a dose-dependent pattern,with the lowest observed among those reporting≥150 mVPA min/week and≥1 mSE days/week(PR:0.43;95%confidence interval:0.41-0.46).When stratified across each mVPA strata,the PRs were mostly lower among those engaging in mSE 1 day/week,as compared to those doing mSE≥2 days/***:"There was evidence for a dose-dependent association between joint mVPA-mSE with a reduced prevalence of *** health strategies for the prevention and management of obesity should recommend both mVPA and mSE.
