Does carrier fluid reduce low flow drug infusion error from syringe size?

BACKGROUND Critically ill neonates and pediatric patients commonly require multiple low flow *** limitations are imposed by small body habitus and comorbidities like cardiopulmonary disease,renal failure,or fluid *** access is limited by diminutive *** fluids or parenteral nutrition in conjunction with actively titrated infusions such as insulin,fentanyl,prostaglandins,inotropes and vasopressors may necessitate simultaneous infusions using a single lumen to maintain vascular catheter *** requirement for multiple titratable infusions requires concentrated medications at low flows,rather than more dilute drugs at higher flows that in combination may volume overload small *** To determine whether carrier fluid reduces variability that variability of low flow drug infusions is proportional to syringe size in pediatric critical *** We assessed concentrations of orange“drug”in a 0.2 mL/h low flow clinical model with blue dyed carrier fluid at 5 mL/h,using 3-,10-,or 60-mL syringes.A graduated volumetric pipette was used to measure total *** time to target concentration was 30,21,and 46 min in 3-,10-,and 60-mL syringes,respectively(P=0.42).After achieving target concentration,more dilute drug was delivered by 60-mL(Plinear mixed effects models demonstrated lesser delivered drug concentrations in the initial 30 min by 3-mL compared to 10-and 60-mL syringes(P=0.005 and P<0.001,respectively)but greater drug concentrations and total infused drug in the subsequent 30-60 and 60-90 min intervals with the 3-and 10-mL compared to 60-mL *** With carrier fluid,larger syringes were associated with significantly

＜正＞ In recent years, many U.S. nuclear plants have applied and received approval for the riskinformed extension of the Allowed Outage Time （AOT） for Emergency Diesel Generators （EDGs）. These risk-informed applications need to meet the regulatory guidance on the risk criteria. This paper discusses in detail insights derived from the risk-informed analyses performed to support these applications. BACKGROUND Although much information is available regarding hepatitis C virus infection and diabetes,less is known about the relationship between hepatitis A virus(HAV)infection and *** To examine the roles of HAV in diabetes *** This cross-sectional study population included data from the National Health and Nutrition Examination Survey collected between *** subjects(≥20 years old)with available body mass index measurements,defined diabetes status,history of HAV vaccination,and HAV serology were *** vaccination was based on self-reported *** HAV immunization was defined as the presence of both vaccination and anti-HAV *** infection was defined by the absence of vaccination but presence of anti-hepatitis A *** odds ratio(OR)for diabetes with 95%confidence intervals(95%CI)was calculated for each HAV status and then adjusted for *** tests,based on different definitions of diabetes,were performed to verify the *** Among 19942 subjects,4229 subjects(21.21%)received HAV vaccination and HAV antibody was present in 9224 subjects(46.25%).Although HAV infection was associated with an increased risk of diabetes(OR:1.13;95%CI:1.08-1.18),HAV vaccination was not associated with diabetes(OR:1.06;95%CI:0.95-1.18),and successful HAV immunization had no impact on the risk of diabetes(OR:1.11;95%CI:0.97-1.27).Thus,HAV infection was an unlikely cause of ***,in non-vaccinated subjects,diabetes increased the risk of HAV infection by 40%(OR:1.40,95%CI:1.27-1.54).CONCLUSION An association between HAV infection and diabetes is observed which is best explained by an increased risk of HAV infection in diabetic *** subjects are more susceptible to ***,HAV vaccination is highly recommended in diabetic patients.

Assessing the yield and safety of endoscopy in acute graft-vs-host disease after hematopoietic stem cell transplant

BACKGROUND Acute gastrointestinal(GI)graft-vs-host disease(aGVHD)is the most complication of hematopoietic stem cell transplant(HSCT)in patients with hematologic *** data exists on endoscopic evaluation of GVHD in post-HSCT patients with differing GI ***,the diagnostic value of gross endoscopic findings as well as the safety of endoscopy in this commonly thrombocytopenic and neutropenic patient population remains *** To understand the diagnostic value of symptoms and gross endoscopic findings as well as safety of endoscopy in aGVHD *** We analyzed 195 endoscopies performed at City of Hope in patients who underwent allogeneic HSCT less than 100 d prior for hematologic malignancy and were subsequently evaluated for aGVHD via *** yield,sensitivity,and specificity of diagnosing aGVHD were calculated for upper and lower endoscopy,various GI tract locations,and presenting *** Combined esophagogastroduodenoscopy(EGD)and flexible sigmoidoscopy(FS)demonstrated a greater diagnostic yield for aGVHD(83.1%)compared to EGD(66.7%)or FS(77.2%)alone with any presenting *** upper and lower GI tract demonstrated similar yields regardless of whether patients presented with diarrhea(95.7%vs 99.1%)or nausea/vomiting(97.5%vs 96.8%).Normalappearing mucosa was generally as specific(91.3%)as abnormal mucosa(58.7%-97.8%)for the presence of *** events such as bleeding(1.0%),infection(1.0%),and perforation(0.5%)only occurred in a small proportion of patients,with no significant differences in those with underlying thrombocytopenia(P=1.000)and neutropenia(P=0.425).CONCLUSION Combined EGD and FS with biopsies of normal and inflamed mucosa demonstrated the greatest diagnostic yield regardless of presenting symptom and appears to be safe in this population of patients.

Relative Bile Acid Binding Potential of Extruded Lentil Snacks

Previously we have reported that extrusion significantly improved healthful potential of cereals. It was hypothesized that snacks produced by extrusion would be more healthful than their raw formulations. Bile acid binding has been reported to indicate cholesterol lowering and cancer risk reduction potential of food and fractions. Bile acid binding potential of five lentil snack raw formulations and their extruded snacks were evaluated. The raw formulations were 100% lentils (F01), 69% lentils (F02), 57% lentils + 12% supplement (F03), F03 with 125 μg/100g Chromium (F04), F03 with 536 μg/100g Chromium (F05), and their respective extruded (E) snacks E01, E02, E03, E04 and E05. The in vitro bile acid binding on an equal dry matter basis relative to cholestyramine, was F01 (0.5%), E01 (3.7%), F02 (0.6%), E02 (3.0%), F03 (1.6%), E03 (5.1%), F04 (2.0%), E04 (4.2%), F05 (0.8%) and E05 (3.6%). Replacing 12% lentils with high protein supplements (F02 vs. F03) resulted in significantly higher bile acid binding, suggesting that the supplement appears to have higher bile acid binding capacity than that of lentils. All the extruded lentil snacks had significantly higher bile acid binding compared with their raw formulations. Extruding with added chromium containing yeast resulted in significantly lower bile acid binding in a dose dependent manner. Most healthful lentil snacks were produced with the addition of high protein supplement without added chromium-containing yeast (E03). Data proved the hypothesis that lentil snacks produced by extrusion are significantly more healthful than their raw formulations.
