AIM: To investigate the release of cyclodextrin-5-amino salicylic acid (CyD-5-ASA) in cecum and ***: An anti-inflammatory drug 5-ASA was conjugated onto the hydroxyl groups of α-, β- and γ-cyclodextrins (CyDs) through an ester linkage, and the in vivo drug release behavior of these prodrugs in rat' s gastrointestinal tract after the oral administration was ***: The 5-ASA concentration in the rat's stomach and small intestine after the oral administration of CyD5-ASA conjugate was much lower than that after the oral administration of 5-ASA alone. The lower concentration was attributable to the passage of the conjugate throughthe stomach and small intestine without significant degradation or absorption, followed by the degradation of the conjugate site-specific in the cecum and colon. The oral administration of CyD-5-ASA resulted in lower plasma and urine concentration of 5-ASA than that of 5-ASA ***: CyD-5-ASA conjugates may be used as prodrugs for colon-specific drug delivery system.

针对普通公路高架桥引起的地面振动问题提出了一种预测地面响应的有效方法。该方法假设高架桥的桥墩在交通荷载下是刚体运动,根据实测数据求得基础承台的输入荷载,并进而由一个准三维有限元方法所得基础承台中心的阻抗函数以及地面响应的传递函数,可以预测周围地面的响应。无论是试验车的情况还是通常的繁忙交通工况,计算的结果和地面响应的实测数据符合得很好。这证明了该方法的可靠性以及对高架桥交通引起的地面振动的通用性。

Clinical Impact of Bacteremia Due to <i>Staphylococcus saprophyticus</i>

Objective: This was a retrospective study to evaluate the clinical impact of bacteremia due to Staphylococcus saprophyticus and identify which factors influence it. Methods: We reviewed all patients with bacteremia due to S. saprophyticus over the last 12 years. This study was performed at Jichi Medical University Hospital in Japan, a key hospital in the northern Kanto area including Tochigi, Gunma, Ibagagi, and northern Saitama prefectures. We retrospectively reviewed the blood culture results and medical records of all patients with a history of visits or hospitalizations between April 2008 and September 2020. Results: During the study period, 4 blood culture specimens were considered to have S. saprophyticus bacteremia. Two of these were from subjects > 60 years old who had severe infection. A third case, 27 years old, was thought to have a catheter-related bloodstream infection;however, the only symptom was fever, which was not serious. The fourth case, a previously unreported pediatric patient, had non-severe fever. The mean time for a diagnosis of bacteremia by blood culture testing was 42 hours, whereas contamination appeared in cultures after another 50 hours. Conclusion: The pathogenicity of S. saprophyticus might be lower in the blood than in the urine due to its physiological function and activity. In older adults with underlying diseases, the severity of bacteremia was more pronounced, whereas in a young adult and a child, the disease was relatively mild. Age and underlying disease might be useful factors to consider when diagnosing bacteremia due to S. saprophyticus.

AIM:To determine the effect of oral sumatriptan on gastric emptying using a continuous 13 C breath test(BreathID system).METHODS:Ten healthy male volunteers participated in this randomized,2-way crossover *** subjects fasted overnight and were randomly assigned to receive a test meal(200 kcal/200 mL) 30 min after pre-medication with sumatriptan 50 mg(sumatriptan condition),or the test meal alone(control condition).Gastric emptying was monitored for 4 h after administration of the test meal by the 13 C-acetic acid breath test performed continually using the BreathID ***,using Oridion Research Software(β version),the time taken for emptying of 50% of the labeled meal(T 1/2) similar to the scintigraphy lag time for 10% emptying of the labeled meal(T lag),the gastric emptying coefficient(GEC),and the regression-estimated constants(β and κ) were *** statistical significance of any differences in the parameters were analyzed using Wilcoxon's signed-rank ***:In the sumatriptan condition,significant differences compared with the control condition were found in T 1/2 [median 131.84 min(range,103.13-168.70) vs 120.27 min(89.61-138.25);P = 0.0016],T lag [median 80.085 min(59.23-125.89) vs 61.11 min(39.86-87.05);P = 0.0125],and β [median 2.3374(1.6407-3.8209) vs 2.0847(1.4755-2.9269);P = 0.0284].There were no significant differences in the GEC or κ between the 2 ***:This study showed that oral sumatriptan significantly delayed gastric emptying of a liquid meal.

A family history of diabetes is not associated with arterial stiffness in non-diabetic Japanese population

Prevalent diabetes is at high risk for cardiovascular diseases and has a high familial inheritance. However, little is known whether a non-diabetic subject with a family history of diabetes is at high risk for vascular damage or not. The purpose of this study was to evaluate the association between a family history of diabetes and arterial stiffness in adult non-diabetic Japanese population. We analyzed eligible 787 non-diabetic subjects (502 men and 285 women) aged 35-69 years who enrolled in the baseline survey of a cohort study in Tokushima Prefecture, Japan and who underwent a brachial-ankle pulse wave velocity (ba-PWV) measurement. Information on individual life-style characteristics including medical history and treatment for diseases and a first-degree family history of diabetes was obtained through a structured self-administered questionnaire. Analysis of covariance and logistic regression analyses were used to evaluate the association between a family history of diabetes and ba-PWV. We found no differences in age-and-systolic blood pressure-adjusted and multivariate-adjusted means of ba-PWVs between subjects of both sexes with and without a family history of diabetes. Logistic regression analyses including both sexes also revealed that subjects with a family history of diabetes showed no differences in age-and-systolic blood pressure-adjusted and multivariate-adjusted odds ratios for high ba-PWV compared to those without that trait. Our results suggest that a family history of diabetes itself is not associated with arterial stiffness in adult non-diabetic Japanese population.

ARID1A expression in gastric adenocarcinoma:Clinicopathological significance and correlation with DNA mismatch repair status

AIM:To analyze the mismatch repair(MMR)status and the ARID1A expression as well as their clinicopathological significance in gastric ***:We examined the expressions of MMR proteins and ARID1A by immunohistochemistry in consecutive 489 primary gastric *** results were further correlated with clinicopathological ***:The loss of any MMR protein expression,indicative of MMR deficiency,was observed in 38cases(7.8%)and was significantly associated with an older age(68.6±9.2 vs 60.4±11.7,P<0.001),a female sex(55.3%vs 31.3%,P=0.004),an antral location(44.7%vs 25.7%,P=0.021),and a differentiated histology(57.9%vs 39.7%,P=0.023).Abnormal ARID1A expression,including reduced or loss of ARID1A expression,was observed in 109 cases(22.3%)and was significantly correlated with lymphatic invasion(80.7%vs 69.5%,P=0.022)and lymph node metastasis(83.5%vs 73.7%,P=0.042).The tumors with abnormal ARID1A expression more frequently indicated MMR deficiency(47.4%vs 20.2%,P<0.001).A multivariate analysis identified abnormal ARID1A expression as an independent poor prognostic factor(HR=1.36,95%CI:1.01-1.84;P=0.040).CONCLUSION:Our observations suggest that the AIRD1A inactivation is associated with lymphatic invasion,lymph node metastasis,poor prognosis,and MMR deficiency in gastric adenocarcinomas.

Background:We sought to clarify the prevalence of occult hepatitis B virus(HBV)infection(OBI)and to determine whether OBI affects the surgical outcomes in curatively resected Japanese patients with hepatitis C virus(HCV)-related hepatocellular carcinoma(HCC).Methods:A total of 257 patients with HCV-related HCC who underwent curative surgical resection were *** enrolled patients were serologically negative for HBV surface antigen and positive for HCV *** was extracted from formalin-fixed paraffin-embedded liver *** was determined by the HBV-DNA amplification of at least two different sets of primers by TaqMan real-time polymerase chain *** outcomes were evaluated according to overall survival(OS),disease-specific survival(DSS),and disease-free survival(DFS).Results:OBI was identified in 15 of the 257(5.8%)*** the multivariate analyses,the factors significantly correlated with OS were BMI>25(P=0.0416),portal vein invasion(P=0.0065),and multiple tumors(P=0.0064).The only factor significantly correlated with DSS was T-stage(P=0.0275).The factors significantly correlated with DFS were liver fibrosis(P=0.0017)and T-stage(P=0.0001).The status of OBI did not show any significant correlation with OS,DSS or DFS,but a weak association with DSS(P=0.0603)was ***:The prevalence of OBI was 5.8%in 257 cases of HCV-related *** a weak association between DSS and OBI was observed,and statistical analyses were limited by small number of OBI cases,no significant correlation between OBI and surgical outcomes was detected.

AIM:To assess clinical outcomes of laparoscopic hepatectomy(LH) in patients with a history of upper abdominal surgery and repeat ***:This study compared the perioperative courses of patients receiving LH at our institution that had or had not previously undergone upper abdominal *** the 80 patients who underwent LH,22 had prior abdominal surgeries,including hepatectomy(n = 12),pancreatectomy(n = 3),cholecystectomy and common bile duct excision(n = 1),splenectomy(n = 1),total gastrectomy(n = 1),colectomy with the involvement of transverse colon(n = 3),and extended hysterectomy with extensive lymph-node dissection up to the upper abdomen(n = 1).Clinical indicators including operating time,blood loss,hospital stay,and morbidity were compared among the ***:Eighteen of the 22 patients who had undergone previous surgery had severe adhesions in the area around the ***,there were no conversions to laparotomy in this *** the 58 patients without a history of upper abdominal surgery,the median operative time was 301 min and blood loss was 150 m *** patients with upper abdominal surgical history or repeat hepatectomy,the operative times were 351 and 301 min,and blood loss was 100 and 50 m L,*** median postoperative stay was 17,13 and 12 d for patients with no history of upper abdominal surgery,patients with a history,and patients with repeat hepatectomy,*** were five cases with complications in the group with no surgical history,compared to only one case in the group with a prior *** were no statistically significant differences in the perioperative results between the groups with and without upper abdominal surgical history,or with repeat ***:LH is feasible and safe in patients with a history of upper abdominal surgery or repeat hepatectomy.

Background: Low back and pelvic pain (LBPP) as a physical symptom may cause psychological symptoms during pregnancy and the postpartum period. We examined the association of LBPP with mental health condition in women from pregnancy to puerperium in a prospective study. Methods: This study was conducted from March 2015 to December 2016 in a birth center in Kagawa Prefecture in Japan. We recruited 89 pregnant women who responded to questionnaires in all four stages (first, second and third trimesters and one week postpartum). We designed a self-administered questionnaire including a visual analog scale (VAS) of LBPP and Edinburgh postnatal depression scale (EPDS). Results: There were significant differences in EPDS scores and VAS scores among the 4 stages. The EPDS score at the first trimester showed significant positive correlations with VAS scores at the second and third trimesters, and the EPDS score at the second trimester showed a significant positive correlation with VAS score at the third trimester. The VAS scores at the second trimester showed significant positive correlations with EPDS scores at the third trimester and at one week postpartum. Conclusion: It was shown that the mental health condition at early pregnancy was related to an increase in LBPP at mid-pregnancy and that LBPP at mid-pregnancy was associated with the mental health condition thereafter.
