Ionizing radiation is one of the most effective tools in cancer therapy. In a previous study, we reported that protein tyrosine kinase (PTK) inhibitors modulate the radiation responses in the human chronic myelogenous leukemia (CML) cell line K562. The receptor tyrosine kinase inhibitor, genistein, delayed radiation-induced cell death, while non-recepter tyrosine kinase inhibitor, herbimycin A (HMA) enhances radiation-induced apoptosis. In this study, we focused on the modulation of radiation-induced cell death by genistein and performed PCR-select suppression subtractive hybridization (SSH) to understand its molecular mechanism. We identified human thymidine kinase 1 (TK1), which is cell cycle regulatory gene and confirmed expression of TK1 mRNA by Northern blot analysis. Expression of TKl mRNA and TK1 enzymatic activity were parallel in their increase and decrease. TK1 is involved in G1-S phase transition of cell cycle progression. Utility of a deep learning model and a clinical model for predicting bleeding after endoscopic submucosal dissection in patients with early gastric cancer

BACKGROUND Bleeding is one of the major complications after endoscopic submucosal dissection(ESD)in early gastric cancer(EGC)*** are limited studies on estimating the bleeding risk after ESD using an artificial intelligence *** To derivate and verify the performance of the deep learning model and the clinical model for predicting bleeding risk after ESD in EGC *** Patients with EGC who underwent ESD between January 2010 and June 2020 at the Samsung Medical Center were enrolled,and post-ESD bleeding(PEB)was investigated *** split the entire cohort into a development set(80%)and a validation set(20%).The deep learning and clinical model were built on the development set and tested in the validation *** performance of the deep learning model and the clinical model were compared using the area under the curve and the stratification of bleeding risk after *** A total of 5629 patients were included,and PEB occurred in 325 *** area under the curve for predicting PEB was 0.71(95%confidence interval:0.63-0.78)in the deep learning model and 0.70(95%confidence interval:0.62-0.77)in the clinical model,without significant difference(P=0.730).The patients expected to the low-(<5%),intermediate-(≥5%,<9%),and high-risk(≥9%)categories were observed with actual bleeding rate of 2.2%,3.9%,and 11.6%,respectively,in the deep learning model;4.0%,8.8%,and 18.2%,respectively,in the clinical *** A deep learning model can predict and stratify the bleeding risk after ESD in patients with EGC.

Comparison of efficacy and safety between standard-dose and modified-dose FOLFIRINOX as a first-line treatment of pancreatic cancer

AIM To directly compare the efficacy and toxicity of standarddose FOLFIRINOX(sFOLFIRINOX) and modified-dose FOLFIRINOX(mFOLFIRINOX, 75% of standard-dose) for pancreatic *** One hundred and thirty pancreatic cancer patients who received sFOLFIRINOX(n = 88) or mFOLFIRINOX(n = 42) as their first-line chemotherapy from January 2013 to July 2017 were retrospectively reviewed. For efficacy analysis, the objective response rate(ORR),disease control rate(DCR), progression-free survival(PFS), and overall survival(OS) were evaluated and compared using Pearson's chi-square test, Kaplan-Meier plot and log-rank test. The adverse events(AEs) were evaluated, and severe(≥ grade 3) AEs rates of the two groups were compared for toxicity *** The mFOLFIRINOX group included more female patients(30.7% vs 57.1%; P = 0.004) and older patients [age(median), 57 vs 63.5; P = 0.018] than the sFOLFIRINOX group. In the efficacy analysis, the ORR and DCR were not significantly different between the two groups(ORR: 39.8% vs 35.7%; P = 0.656; DCR: 80.7% vs 83.3%; P = 0.716). The median PFS and OS were also not different between the groups(PFS: 8.7 mo vs 8.1 mo, P = 0.272; OS: 13.9 mo vs 13.7 mo, P = 0.476). In the safety analysis with severe AEs, the rates of neutropenia(83.0% vs 66.7%; P = 0.044), anorexia(48.9% vs 28.6%; P = 0.029) and diarrhea(13.6% vs 0.0%; P = 0.009) were markedly lower in the mFOLFIRINOX *** m FOLFIRINOX showed comparable efficacy but better safety compared to sFOLFIRINOX. If clinically necessary, initiating FOLFIRINOX with 75% of the standard-dose can alleviate toxicity concerns without compromising efficacy.

Weekend and nighttime effect on the prognosis of peptic ulcer bleeding

AIM:To evaluate whether weekend or nighttime admission affects prognosis of peptic ulcer bleeding despite early ***:Retrospective data collection from four referral centers,all of which had a formal out-of-hours emergency endoscopy service,even at weekends.A total of 388 patients with bleeding peptic ulcers who were admitted via the emergency room between January 2007 and December 2009 were *** parameters included time from patients' arrival until endoscopy,mortality,rebleeding,need for surgery and length of hospital ***:The weekday and weekend admission groups comprised 326 and 62 patients,*** were no significant differences in baseline characteristics between the two groups,except for younger age in the weekend *** patients (97%) had undergone early endoscopy,which resulted in a low mortality rate regardless of point of presentation (1.8% overall vs 1.6% on the weekend).The only outcome that was worse in the weekend group was a higher rate of rebleeding (12% vs 21%,P = 0.030).However,multivariate analysis revealed nighttime admission and a high Rockall score (≥ 6) as significant independent risk factors for rebleeding,rather than weekend ***:Early endoscopy for peptic ulcer bleeding can prevent the weekend effect,and nighttime admission was identified as a novel risk factor for rebleeding,namely the nighttime effect.

Biomass-derived electrodes inherently containing redox-active species have gained extensive attention recently due to their availability,eco-friendliness,sustainability,and low *** report novel binder-free faradic surface redox onion-derived carbon positive electrode with nano regime particles by hydrothermal synthesis and Na^(+)and Cl^(−)ions diffused porous carbon negative electrode via a carbonization ***-dispersive X-ray spectroscopy and X-ray photoelectron spectroscopy confirmed the presence of oxidized sulfur and(N-6)pyridinic N-based redox groups inherently present in the as-prepared *** electrochemical analysis of the positive electrode revealed its faradic redox type of energy storage mechanism with an excellent specific capacitance of 1805 Fg^(-1) at the current density of 3 Ag^(-1) as well as appreciable long-term cycling stability(76.8%retention after 10000 charge-discharge cycles).Meanwhile,the negative electrode exhibited a maximum specific capacitance of 373 Fg^(-1) at 1 Ag^(-1) with outstanding long-term cycling stability(100.7%retention after 10000 cycles).The fabricated polyvinyl alcohol-potassium hydroxide gel electrolyte-based quasi-solid-state hybrid supercapacitor(QHSC)delivered excellent energy density and power density of 19.94 Wh kg^(-1) and 374.99 W kg^(-1),respectively with an ultralong cyclic life(102.3% retention)over 10000 ***,the QHSC was connected to a solar panel to store renewable *** charged QHSC effectively powered a speedometer,enlightening its potential application in advanced sustainable energy storage systems.

Usefulness of colonic tattooing using indocyanine green in patients with colorectal tumors

AIM To prove that tattooing using indocyanine green(ICG) is feasible in laparoscopic surgery for a colon *** From January 2012 to December 2016, all patients who underwent laparoscopic colonic surgery were retrospectively screened, and 1010 patients with colorectal neoplasms were included. Their lesions were tattooed with ICG the day before the operation. The tattooed group(TG) included 114 patients, and the non-tattooed group(NTG) was selected by propensity score matching of subjects based on age, sex, tumor staging, and operation method(n = 228). In total, 342 patients were enrolled. Between the groups, the changes in [Delta(Δ), preoperative-postoperative] the hemoglobin and albumin levels, operation time, hospital stay, oral ingestion period, transfusion, and perioperative complications were compared. RESULTS Preoperative TG had a shorter operation time(174.76 ± 51.6 min vs 192.63 ± 59.9 min, P < 0.01), hospital stay(9.55 ± 3.36 d vs 11.42 ± 8.23 d, P < 0.01), and postoperative oral ingestion period(1.58 ± 0.96 d vs 2.81 ± 1.90 d, P < 0.01). The Δ hemoglobin(0.78 ± 0.76 g/dL vs 2.2 ± 1.18 g/dL, P < 0.01) and Δ albumin(0.41 ± 0.44 g/dL vs 1.08 ± 0.39 g/dL, P < 0.01) levels were lower in the TG. On comparison of patients in the "N0" and "N1 or N2" groups, the N0 colon cancer group had a better operation time, length of hospital stay, oral ingestion period, Δ hemoglobin, and Δ albumin results than those of the N1 or N2 group. The operation methods affected the results, and laparoscopic anterior resection(LAR) showed similar results. However, for left and right hemicolectomy, both groups showed no difference in operation time or hospital *** Preoperative tattooing with ICG is useful for laparoscopic colectomy, especially in the N0 colon cancer group and LAR.

8-Hydroxydeoxyguanosine:Not mere biomarker for oxidative stress,but remedy for oxidative stress-implicated gastrointestinal diseases

Reactive oxygen species (ROS) attack guanine bases in DNA easily and form 8-hydroxydeoxyguanosine (8-OHdG),which can bind to thymidine rather than cytosine,based on which,the level of 8-OHdG is gen-erally regarded as a biomarker of mutagenesis conse-quent to oxidative *** example,higher levels of 8-OHdG are noted in Helicobacter pylori-associated chronic atrophic gastritis as well as gastric ***,we have found that exogenous 8-OHdG can paradoxically reduce ROS production,attenuate thenuclear factor-κB signaling pathway,and ameliorate the expression of proinflammatory mediators such as interleukin (IL)-1,IL-6,cyclo-oxygenase-2,and induc-ible nitric oxide synthase in addition to expression of nicotinamide adenine dinucleotide phosphate oxidase (NOX)-1,NOX organizer-1 and NOX activator-1 in vari-ous conditions of inflammation-based gastrointestinal (GI) diseases including gastritis,inflammatory bowel disease,pancreatitis,and even colitis-associated *** recent finding that exogenous 8-OHdG was very effective in either inflammation-based or oxidative-stress-associated diseases of stress-related mucosal damage has inspired the hope that synthetic 8-OHdG can be a potential candidate for the treatment of inflammation-based GI diseases,as well as the pre-vention of inflammation-associated GI *** this editorial review,the novel fact that exogenous 8-OHdG can be a functional molecule regulating oxidative-stress-induced gastritis through either antagonizing Rac-guanosine triphosphate binding or blocking the signals responsible for gastric inflammatory cascade is introduced.

OBJECTIVE:To develop and validate a Seven Emotions Impairment questionnaire(SEIQ),to define an optimum cut-off point for the SEIQ,and to examine whether SEI was predictive of Phlegm and Blood Stasis(BS).METHODS:Two hundred outpatients and 75 college students were asked to complete the SEIQ,the Profile of Mood States(POMS),Phlegm Pattern Questionnaire(PPQ),and BS Questionnaire(BSQ).Twelve clinicians determined whether the outpatients exhibited *** data were used to examine the internal consistency and determine validity for the ***,POMS,PPQ,and BSQ data were used to examine concurrent validity and predictability of SEI for Phlegm and BS in the college *** SEIQ scores and the clinicians' diagnoses of the outpatients were considered to define an optimum cut-off score for the ***:The 18-item SEIQ had satisfactory internal consistency(α = 0.905) and concurrent *** the construct validity test,four factors(chest-anxiety,fatigue-depression,working-family-troubles,and sleep-memory) were *** the receiver operator characteristic curve curve analysis,the sensitivity,specificity,and area under the curve of the SEIQ were 67.2%,72.1%,and 73%,*** optimum cut-off score was defined as nine *** scores were strongly predictive of Phlegm and BS(β = 0.862 and 0.673,respectively).CONCLUSION:Based on our results,we concluded that the SEIQ is a reliable and valid instrument for evaluating SEI,and is strongly predictive of Phlegm and BS.
