Better timing for HoLEP: a retrospective analysis of patients treated with HoLEP over a 10-year period with a 1-year follow-up

The aim of this study was to explore the optimal timing of holmium laser enucleation of the prostate(HoLEP)in patients presenting benign prostatic hyperplasia(BPH)with lower urinary tract symptoms(LUTS).A retrospective analysis was conducted based on the perioperative and postoperative outcome data of 1212 patients who underwent HoLEP in Shanghai Ninth People’s Hospital(Shanghai,China)between January 2009 and December *** to the preoperative International Prostate Symptom Score(IPSS),all patients whom we analyzed were divided into Group A(IPSS of 8–18)and Group B(IPSS of 19–35).Peri-and postoperative outcome data were obtained during the 1-year *** changes were the main postoperative *** postoperative IPSS,quality of life,peak urinary flow rate,postvoid residual,and overactive bladder symptom score(OABSS)improved *** IPSS improved further in the group with severe LUTS symptoms,but the postoperative IPSS was still higher than that in the moderate LUTS *** showing moderate and severe cases after follow-up were more frequent in Group B(9.1%)than in Group A(5.2%)(P<0.05).There were no significant intergroup differences in the intraoperative American Society of Anesthesiologists or hospitalization expense scores,and the medication costs,as well as the total costs,were significantly higher in Group *** this retrospective study,HoLEP was an effective treatment for symptomatic *** patients with LUTS,earlier surgery in patients with moderate severity may result in a marginally better 12-month IPSS than that in men with severe symptoms.

China′s urban reforms have brought social progress and development, but a comprehensive national system of social welfare (for example, unemployment insurance, pensions, medical care and public housing) for new migrants from rural areas is lacking. One of the most remarkable changes in Chinese cities in the last decade was a change in social "equality", with the rise of new poverty both in individual communities and some social groups in urban society. However, there is little social assistance and public infrastructure for the migrants. Governments or communities or individual should pay attention to the control of new urban poverty and new slums. This paper consider that it is necessary to launch a successful policy, which include mainly: 1) accommodating urban growth through low cost investment projects; 2) urban economy depends heavily on successful macroeconomic policy; 3) to broaden the three channels linking adjustment to the incidence of urban poverty; 4) to restructure urban economic based on the high or new technology; 5) to coordinate relationship between urban economic growth and environment management for sustainable development of Beijing′s metropolitan fringe.

With advances in imaging technology and surgical instruments,hepatectomy can be perfectly performed with technical precision for hepatocellular carcinoma(HCC).However,the 5-year tumor recurrence rates remain greater than 70%.Thus,the strategy for hepatectomy needs to be reappraised based on insights of scientific *** evidence has suggested that the main causes of recurrence after hepatectomy for HCC are mainly related to underlying cirrhosis and the vascular spread of tumor cells that basically cannot be eradicated by *** transplantation and systemic therapy could be the solution to prevent postoperative recurrence in this ***,determining the severity of liver cirrhosis for choosing the appropriate surgical modality,such as liver transplantation or hepatectomy,for HCC and integrating newly emerging immune-related adjuvant and/or neoadjuvant therapy into the strategy of hepatectomy for HCC have become new aspects of exploration to optimize the strategy of *** this new area,hepatectomy for HCC has evolved from a pure technical concept emphasizing anatomic resection into a scientific concept embracing technical considerations and scientific advances in underlying liver cirrhosis,vascular invasion,and systemic *** introducing the concept of scientific hepatectomy,the indications,timing,and surgical techniques of hepatectomy will be further scientifically optimized for individual patients,and recurrence rates will be decreased and long-term survival will be further prolonged.

Dynamic behavior of laterally loaded caisson foundations based on different cushion types: an experimental and theoretical study

Bridge foundations located in deep water are usually subjected to horizontal dynamic loads and moments which may be caused by the wind, waves, earthquake, and the possibility of boat crashing or vehicle braking. Caisson foundations based on gravel or sand cushions are a new type of deep-water foundation for bridges, suitable for meizoseismal areas. In this paper, harmonic horizontal excitation tests for the study of the lateral dynamic response of caisson foundations based on cushion layers are described. Different lateral loads and two different cushion types are considered. The results show that the lateral dynamic responses of caisson foundations based on sand and gravel cushions both show strong nonlinear characteristics, and the resonant frequency of the foundation decreases with the increase of the excitation force. The dynamic displacement of a foundation based on a sand cushion is far less than that based on a gravel cushion, and the rate of decrease of the resonant frequency of a foundation based on a gravel cushion is faster than that of a foundation based on a sand cushion under the same conditions. Under dynamic loading the gravel cushion can more effectively dissipate vibration energy and isolate the vibration, than the sand cushion can. A simplified nonlinear analysis method is proposed to simulate the lateral dynamic response of caisson foundations, and the predicted response shows a reasonable match with the results observed in laboratory tests. Scaling laws have also been applied in this small-scale vibration model test to predict the dynamic behavior of the prototype foundation.

AIM： To investigate the relationship between insulin resistance （IR）/β-cell dysfunction and diabetic retinopathy （DR） in Chinese patients with type 2 diabetes mellitus （T2DM）, and to explore further whether there were differences in the relationship among diabetic patients with higher and lower body mass index （BMI）. METHODS： Cross-sectional study. A total of 1466 subjects with T2DM were recruited in a local Desheng Community of urban Beijing from November 2009 to June 2012 for the cohort of Beijing Desheng Diabetic Eye Study. Standardized evaluation was carried out for each participant, including questionnaire, ocular and anthropometric examinations, and laboratory tests. Seven fields 30° color fundus photographs were used for DR grading according to the Early Treatment Diabetic Retinopathy Study protocols. Homeostatis Model Assessment （HOMA） method was employed for IR and β-cell function assessment. RESULTS： After excluding those participants who were treated with insulin （n=352） or had missing data of fasting insulin （n=96）, and further excluding those with poor quality of retinal photographs （n=10）, a total of 1008 subjects were included for the final analysis, 406 （40.3%） were men and 602 （59.7%） were women, age ranging fiom 34 to 86 （64.87±8.28）y. Any DR （levels 14 and above） was present in 278 （27.6%） subjects. After adjusting for possible covariates, the presence of any DR did not correlate with HOMA IR [odds ratio （OR） 1.51, 95% confidence interval （Cl） 0.87-2.61, P=0.14] or HOMA β-cell （OR 0.71, 95%CI 0.40-1.26, P=0.25）. After stratification by BMI, the presence of any DR was associated positively with HOMA IR （OR 2.46, 95%CI： 1.18-5.12, P=0.016）, and negatively with HOMA β-cell （OR 0.40, 95%CI： 0.19-0.87, P=0.021） in the group of patients with higher BMI （225 kg/m2）. In the group of patients with lower BMI （〈25 kg/m2）, the presence of any DR was not associated with HOMA IR （OR 1.00, 95%C1： 0.43-2.33, P=I.00） or HOMA β-cell （OR 1.41, 95%CI： 0.60-3.32, P=

Land use/cover and landscape pattern changes in Manas River Basin based on remote sensing

Large-scale water and soil development in inland river basins in arid areas has made changes in landscape composition and structure,threatening the ecological *** order to study the trend of land use/cover and landscape pattern change and its relationship with water resources utilization in Manas River Basin,the land-use data of five periods in 1976,1990,2000,2010 and 2015 were *** results showed that:(1)During 1976-2015,farmland and construction land continuously increased,forest land and grassland continuously decreased,the water area initially reduced and then increased,the area of saline-alkali land initially increased and then reduced,the overall trend of unused land and the sandy area was decreasing and the area of different time periods is *** areas of land-use types either increased or *** indicates that the landscape pattern of the basin changes dramatically,and human activities are the main reasons for this phenomenon;(2)The oasis area increased from 3480.2 km2 in 1976 to 7982.0 km2 in 2015,with an obvious increasing *** oasis area clearly increased during 1976-1990,the growth rate was 40.6%,the growth rate of the oasis area was 129.4%.In the last 40 years,the degree of desertification fluctuated and *** increase of the oasis area directly leads to the increase of water resources utilization and aggravates the degree of water resources shortage;(3)The pattern of land-use types showed a non-equilibrium *** the region with increasing landscape heterogeneity,the overall landscape pattern was increasingly controlled by the majority of *** intensive land management model and drip irrigation under mulch have improved the utilization efficiency of water resources and saved water resources from engineering renovation and irrigation management.
