Severe morbidity af.er pancreatectomy is accurately pre dicte d by preoperative pancreatic resection score(PREPARE): A prospective validation analysis f.om a medium-volume center

Background: Maj.r morbidity in pancreatic surgery remains high. Dif.erent scores f.r predicting complications have been described. Preoperative pancreatic resection(PREPARE) score is based on obj.ctive preoperative variables and of.ers good predictive accuracy f.r Clavien ≥ III complications. This study aimed to validate this score and analyze other preoperative variables in a prospective study perf.rmed in a medium-volume center. Methods: A total of.50 pancreatic resections were included. Preoperative variables were registered and PREPARE was calculated. The main outcome was severe morbidity(Clavien ≥ III) up to 30 days af.er discharge. The secondary outcomes were length of.stay(LOS) and readmission. Statistical validation was perf.rmed to compare severe morbidity rate among the scores categories. Association with other preoperative variables(not included in PREPARE) was also tested. Results: Of.the 50 pancreatic resections, the severe morbidity was 34.0%, with median LOS of.11 days. Readmission rate was 25.5%. Severe morbidity rates according to PREPARE categories were 18.5% in lowrisk group, 41.7% in intermediate-risk group, and 63.6% in high-risk group, respectively( P = 0.023). The accuracy was 72%(Hosmer–Lemeshow, P = 0.86). ROC curve was obtained both f.r PREPARE score expressed as incremental values and categorized as the three risk groups, showing an area under curve(AUC) of.0.736(95% CI: 0.586–0.887; P = 0.007) and 0.712(95% CI: 0.555–0.869; P = 0.015), respectively. PREPARE was signif.cant in multivariate analysis. Median LOS was statistically higher as PREPARE category increases(9, 11 and 15 days in low-, intermediate-and high-risk groups, respectively; P = 0.009). Readmission was not associated with any variables. Conclusions: PREPARE behaves as an independent risk f.ctor f.r severe morbidity af.er pancreatic surgery. Score validation shows good accuracy prediction. Increasing PREPARE category is also associated with longer LOS.

Photosynthetic Regulation of.the Cyanobacterium Synechocystis sp. PCC 6803 Thioredoxin System and f.nctional Analysis of.TrxB （Trx x） and TrxQ （Trx y） Thioredoxins

The expression of.the genes encoding the f.rredoxin-thioredoxin system including the f.rredoxin-thioredoxin reductase （f.R） genes f.rC and f.rV and the f.ur dif.erent thioredoxin genes trxA （m-type; sir0623）, trxB （x-type; sir1139）, trxC （sll1057） and trxQ （y-type; sir0233） of.the cyanobacterium Synechocystis sp. PCC 6803 has been studied according to changes in the photosynthetic conditions. Experiments of.light-dark transition indicate that the expression of.all these genes except trxQ decreases in the dark in the absence of.glucose in the growth medium. The use of.two electron transport inhibitors, 3-（3,4-dichlorophenyl）-1,1-dimethylurea （DCMU） and 2,5-dibromo-3-methyl-6-isopropyl-p- benzoquinone （DBMIB）, reveals a dif.erential ef.ect on thioredoxin genes expression being trxC and trxQ almost unaf.ected, whereas trxA, trxB, and the f.r genes are down-regulated. In the presence of.glucose, DCMU does not af.ect gene expression but DBMIB still does. Analysis of.the single TrxB or TrxQ and the double TrxB TrxQ Synechocystis mutant strains reveal dif.erent f.nctions f.r each of.these thioredoxins under dif.erent growth conditions. f.nally, a Synechocystis strain was generated containing a mutated version of.TrxB （TrxBC34S）, which was used to identif. the potential in-vivo targets of.this thioredoxin by a proteomic analysis.

The inf.uence of.small calcium additions on the high-temperature mechanical behaviour in an extruded Mg-6Zn-l Y(wt.%)alloy reinf.rced by the I-phase has been *** promotes the f.rmation of.the intermetallic Mg6Zn3Ca2 phase instead of.1-phase,which results in a noticeable improvement of.the yield strength and ultimate tensile strength of.the alloy above 100℃.The strength of.the alloys was analysed taking into account the contribution due to the grain size,the crystallographic texture and the volume f.action and nature of.second phase *** situ synchrotron radiation dif.raction experiments have been used to evaluate the load partitioning between the magnesium matrix and the second phase particles(1-and MgeZgCa?phases)in both *** load transf.r f.om the magnesium matrix towards the MgeZihCa?phase is markedly more ef.ective than that f.r the I-phase over the entire temperature range,especially at 200°C,temperature at which the reinf.rcement ef.ect of.the I-phase is null.

The ef.ects of.applying an electromagnetic interaction of.low intensity (EMILI) on the microstructure and corrosion resistance of.7075-T651 Al alloy plates (13 mm in thickness) during modif.ed indirect electric arc (MIEA) welding were investigated. The welding process was conducted in a single pass with a heat input of.~1.5 kj.mm. The microstructural observations of.the welds were correlated with the ef.ect of.EMILI on the local mechanical properties and the corrosion resistance in natural seawater by means of.microhardness measurements and electrochemical impedance spectroscopy, respectively. Microstructural characterization of.the welds revealed a grain ref.nement in the weld metal due to the electromagnetic stirring induced by EMILI of.3 mT during welding. In addition, observations in the scanning electron microscope showed that the precipitation of.Cu-rich phases and segregation of.eutectics were reduced in the heat af.ected zone (HAZ) also as an ef.ect of.EMILI. The high corrosion dissolution of.the 7075-T651 welds in natural seawater and the extent of.overaging in the HAZ were reduced when welding with EMILI of.3 mT. Thus, EMILI along with the MIEA technique may lead to welded j.ints with better microstructural characteristics, improved mechanical properties in the HAZ and reduced electrochemical activity.
