Background/Purpose: Late-onset cholangitis is an uncom.on com.lication after the Kasai operation for biliary *** with 3 recent patients illustrates appropriate ***: Retrospective review of patients with late unexplained deterioration in liver function attributed to *** underwent quantitative radioisotope hepatobiliary scans to identify the Roux loop obstruction and confirm.post operative resolution. The role of percutaneous transhepatic cholangiography and com.uted tom.graphy scanning was ***: Three patients aged 8, 13, and 17 years were identified. All 3 had had a successful Kasai portoenterostom. for type 3 biliary atresia and presented after several virtually trouble-free years with late-onset cholangitis. The radioisotope scans in all 3 patients showed poor tracer uptake by the liver and pooling of the radioisotope at the porta hepatis. Laparotom. confirm.d a proxim.l Roux loop obstruction at the level of the m.socolic window in all. This required m.bilization and adhesiolysis and, in 1, correction of an actual stenosis with in the wall of the intestine at this level. All patients had norm.lization of their liver function postoperatively. All are now well and anicteric at a follow-up of 6 to 12 m.nths. Conclusions:Long-term.survivors of the Kasai portoenterostom. for biliaryatresia with im.ediate deterioration in liver function warrant investigation for possible Roux loop obstruction. Resolution of the obstruction allows preservation of their native liver.

Objective: Intercellular adhesion m.lecule (ICAm. expression in liver and bile duct rem.ant is a feature of the inflam.atory com.onent of biliary atresia (BA). Circulating levels of such soluble adhesion m.lecules (SAm. should reflect intrahepatic disease and would prove a useful adjunct in the evaluation of BA. Study Design: Serum.ICAm.1 (sICAm.1), serum.vascular cell adhesion m.l-ecule (sVCAm.1), and serum.E selectin (sE-selectin) were m.asured by enzym.-linked im.unosorbent assay in infants with BA at the tim. of portoenterostom. and stratified by outcom.. Results are quoted as m.dians (range). Binary logistic regression analysis and actuarial survival curves were used to investigate the relationship of SAm.profile to outcom.. Significance was assum.d at P values of ≤ .05. Results: Sixty-one infants with BA were treated between 1996 and 2002 and at follow-up; 39 of these were alive and jaundice-free (good outcom., n=39); 21 had been transplanted, and 1 died (poor outcom., n=22). Preoperative values for sICAm.1 were 1233 (400-2000) ng/m.; sVCAm.1, 1204(517-1921) ng/m.; and sE-selectin, 71(26-192) ng/m.. sVCAm.1(Pm.1(P m.ared with norm.l control infants, although sE-selectin did not (P = .17). There was a significant correlation of age at surgery with sICAm.1 (r = 0.33, P = .01) but not with sVCAm.1 (r = 0.16, P = .23) or sE-selectin (r = 0.05, P = .70). Binary logistic regression analysis showed that the variables, sICAm.1, sE-selectin, or age at surgery, were not significant predictors of outcom., although sVCAm.1 approached significance (P = .069). A cutoff value for sVCAm.1 of 1380 ng/m. was defined by receiver operating characteristic curve analysis, and the cohort of patients with sVCAm.1 (>1380 ng/m.) showed a significantly worse actuarial survival (P = .05). Conclusion: Levels of sICAm.1 and sVCAm.1 are grossly elevated in BA, whereas sE-selectin levels are norm.l. Only sVCAm.1 levels have prognostic signi

There is an accum.lating body of evidence im.licating the m.scarinic acetylcholine receptor4(m.)in schizophrenia and dem.ntia with Lewy bodies,however,a clinically validated m.positron em.ssion tom.graphy(PET)radioligand is currently *** such,the aim.of this study was to develop a suitable m.PET ligand that allows the non-invasive visualization of m.in the ***-activity relationship studies of pyrazol-4-yl-pyridine derivates led to the discovery of target com.ound 12—a subtype-selective positive allosteric m.dulator(PAm..The radiofluorinated analogue,[18F]12,was synthesized in 28±10%radiochem.cal yield,>37 GBq/μm.l and an excellent radiochem.cal purity>99%.Initial in vitro autoradiogram. on rodent brain sections were perform.d in the absence of carbachol and showed m.derate specificity as well as a low selectivity of[18F]12 for the m.-rich ***,in the presence of carbachol,a significant increase in tracer binding was observed in the rat striatum.which was reduced by>60%under blocking conditions,thus indicating that orthosteric ligand interaction is required for efficient binding o f[18F]12 to the allosteric ***,however,the presence of carbachol was not required for high specific binding in the non-hum.n prim.te(NHP)and hum.n striatum.and did not further im.rove the specificity and selectivity of[18F]12 in higher *** results pointed towards significant species-differences and paved the way for a prelim.nary PET study in NHP,where peak brain uptake of[18F]12 was found in the putam.n and tem.oral *** conclusion,we report on the identification and preclinical developm.nt of the first radiofluorinated m.PET radioligand with prom.sing *** availability of a clinically validated m.PET radioligand harbors potential to facilitate drug developm.nt and provide a useful diagnostic tool for non-invasive im.ging.

neonatal tum.rs with an incidence of approxim.tely 1:30,000. There are few large single-center series and even fewer describing both their antenatal and postnatal course. We report the outcom. of all fetuses investigated at a tertiary fetal m.dicine center with this diagnosis. m.thod: Dem.graphic details were obtained from.a prospectively m.intained database. Patient records were exam.ned for additional data including antenatal and postnatal interventions. Data were described as m.dian (range). Results: Forty-one SCTs were diagnosed antenatally during the period 1993 to 2004. Twelve were excluded from.subsequent analysis (single antenatal visit or attending for second opinion [n = 6] and term.nation of pregnancy [n = 6]). Twelve underwent fetal intervention (laser vessel ablation [n = 4],alcohol sclerosis [n = 3],cyst drainage [n = 2],am.iodrainage [n = 2],vesicoam.iotic shunt [n = 1]) for fetal hydrops and polyhydram.ios to aid in delivery and to prevent obstructive uropathy developing in the fetus. Of these,3 died in utero and 9 survived to be born (m.dian gestational age,33 weeks [27-37 weeks]). A further 3 died in the neonatal period. There are 6 long-term.survivors (50% ) from.this group. Seventeen infants,without intervention,were born at m.dian gestational age 38weeks (26-40weeks). One infant with severe cardiac anom.lies died on the day of birth. All surviving infants had definitive excisional surgery at a m.dian of 2 days (1-16 days). Current m.dian follow-up of survivors is 39 m.nths (8-86 m.nths). There have been no recurrences. One child has m.ld constipation,and 3 are awaiting cosm.tic revision of their scars. Conclusions: The overall survival of antenatally diagnosed SCT is approxim.tely 77% ,with the developm.nt of hydrops and others requiring in utero intervention carrying a poor prognosis. Otherwise,the outcom. after surgical excision is excellent.

患者出现突发剧烈的头痛，头痛在数分钟内达到顶点，并且持续超过1小时，应怀疑蛛网膜下腔出血。常规头颅CT平扫结果正常并不能排除蛛网膜下腔出血，这是因为延缓检查时间会迅速降低CT对诊断的敏感性。当头颅CT平扫正常时，专家建议行腰穿检查。当患者就诊于神经专科时，专科医生应仔细确认发病的时间、年龄、并存疾病，意识水平和任何神经功能缺损。每4小时口服60m.尼莫地平可以减少不良预后，但其他干预措施还很有限。在阻断破裂动脉瘤方面，目前血管内栓塞术已取代了神经外科的动脉瘤夹闭术。多学科随访可以帮助处理蛛网膜下腔出血患者的并发症。
